If you do not have at least Internet Explorer 7, you may download the newest available version on your home computer for free at www.microsoft.com. Your password expires every 90 days. You will be able to login using your old password but will be prompted to change it. Always use the links in the Menu window on the left side of the screen. If you use your browser s back button you may receive a message that says Warning: Page has expired. Use your browser s refresh feature, and the Employee Connect menu will appear. When you finish using the Employee Connect site, always sign out instead of simply closing your browser window. October 21, 2015 Page 1 of 9
Things to know when enrolling online You do not have to complete your enrollment in one sitting. Once you have viewed the screen, made a selection and clicked, the system will save that selection. You are able to make elections up to the end of Open Enrollment. October 21, 2015 Page 2 of 9
Section 1: Logging In Login to Employee Connect at https://employeeconnect.osfhealthcare.org from any computer. Please be sure to use https not http. Enter your User ID and password. Your User ID is your 5-digit employee ID number. Your password the first time you login is made up of: last 4 digits of your SSN, first letter of your first name (capitalized), first letter of your last name (capitalized), and 4-digit birth year. Sample password: 5678TD1960. If this is your first time logging in, the system will prompt you to change your password. Your new password must be a minimum of eight (8) characters and have at least one number. If you have difficulty logging in, you may use the Forgot your password? link to have your password reset and an email will be sent to your OSF email address. Once logged in, left click on the Main Menu link on the top toolbar and then navigate to Benefits Enrollment. October 21, 2015 Page 4 of 9
Section 2: Benefits Enrollment From the Open Enrollment screen, click the button to begin enrolling in your benefits. Contact the Benefits Help Center if you do not have an event. October 21, 2015 Page 5 of 9
Each benefit you are eligible for is listed, and your participation in that benefit shown as Current. If you choose NOT to make any changes, your participation, shown as New, will look the same. To review each benefit and make any changes, click the button next to it. If you make a change, your new election will show as New. October 21, 2015 Page 6 of 9
On each of these benefit screens, you will have the option to enroll in or waive coverage. You must click the button to save your selection once you have made your choice. At that point, you will see a confirmation screen that lists the coverage you just selected and the premium amount. Clicking does NOT complete your enrollment. Section 3: Enrolling and Selecting Dependents and Beneficiaries Dependents are individuals who are allowed to be covered on some of the OSF Benefit offerings. Beneficiaries are individuals who are allowed to receive death benefits under the Employer Paid Life and Accidental Death/Dismemberment Insurance and Supplemental Life Insurance. On the screens containing a benefit that dependents are eligible to enroll in, you will see the dependents listed in our HR system who are eligible to be enrolled. If there is a dependent that is eligible that you do not see listed, you may add an eligible dependent by clicking the button. If a dependent is listed, this does NOT mean they are enrolled in the benefit. To enroll them, check the check box in the Enroll column. To unenroll them, uncheck the check box in the Enroll column. On the Employer Paid Life and Accidental Death/Dismemberment Insurance and Supplemental Life Insurance screens, you will see listed everyone in our HR system who is eligible to be a Beneficiary. This does NOT mean they will receive a death benefit. o o To select them as a beneficiary, enter a percentage in the New Primary Percent if you want them to be a primary beneficiary, or the New Secondary Percent column if you want them to be a secondary beneficiary. To deselect them, delete the percentage in the appropriate column. If someone is missing in your lists click the button to add them. o OSF needs to keep records of anyone who has ever been your Dependent or Beneficiary, though, so it is VERY important that you please do not delete or type over a currently existing person. Section 4: Completing your enrollment You are not finished with your enrollment until you have clicked the button on BOTH the Enrollment Summary AND Submit Benefit Choices screens and the Submit Confirmation screen is displayed, stating your benefit choices have been successfully submitted. If you have done everything correctly, you will receive a confirmation email to your OSF email address with 48 hours. If you do not receive an email, you may need to take additional steps to finalize your enrollment, such as completing an Evidence of Insurability form (required for Supplemental Life Insurance elections for employee or spouse). The Evidence of Insurability form will be emailed to you by VOYA in December. If you would like to view your new elections, navigate to Benefits Summary 48 hours after you make your elections, change the date to January 1 of the following year (i.e. in November, 2015 enter 01/01/16), and click. Enter the last 2 digits of the next calendar year in place of the XX. (For example, in 2015, enter 16 for 2016) If you wish to make any changes or see something inaccurate, simply go back into benefits enrollment to change or update. October 21, 2015 Page 9 of 9
