1. Welcome Welcome to Digital SlideBox, a multimedia content management system for web-based microscopy. This manual will guide you through the functionality of the system. IMPORTANT: SlidePath applications are not cleared by the FDA, Health Canada, or in the EU for diagnostic or clinical use. All applications are intended solely for use in the research or educational setting, such as university or pharmaceutical development. These applications are described as Research Applications or Research Use Only. 1.1 Restricted Access Please note, the features and functionality available will depend on your access level. As a result, some features and functionality reviewed in this manual may not be available to all users. 3. Access Levels This section outlines the different access levels available in Digital SlideBox. There are four access levels: Access level Browse Tab Search Tab Administration Tab User Management System Tab Administrator Creation Administrator Management User Administrator Super User Global Administrator 3.1 Visibility Levels Administrators and Super Users can set the visibility to users of various pieces of content in Digital SlideBox. Note: If content that is not currently visible is linked to an annotation, narrative, questionnaire or quiz, the link will not function for users. Visibility Key Invisible Visible Becomes visible after a questionnaire is completed 4 of 61

4. Logging in to Digital SlideBox Log in to the Digital SlideBox system from this page. A. Enter username / password B. Login C. Enter username / email address to retrieve your login details Note: Username must be at least 3 characters long to use this function D. Register as a new user, using an activation key provided by an administrator E. Register as an existing user, using an activation key provided by an administrator 4.1 Banner The banner in Digital SlideBox is visible throughout the system. There are three primary tabs, each with specific functions. A. Display Digital SlideBox in one of the following languages: English German French Spanish Italian Japanese B. Link to help page C. Contact SlidePath customer support D. Manage profile name, password and email address E. Logout F. User tab G. Search tab (access level = administrator or higher) H. Admin tab (access level = administrator or higher 5 of 61

5. User Tab After logging in, a user can view all modules they have access to. A. Navigation bar (can be used to return to parent folders at any time) B. Select a folder icon to enter that folder Green = folder with questionnaire / quiz that has been answered Orange = folder with questionnaire /quiz that has been partly answered Red = folder with questionnaire /quiz that has not yet been answered White = folder does not contain an accessible questionnaire / quiz 6 of 61

5.1 Accessing Content On entering a folder, users can access content which has been added to that folder by an administrator. Note: When navigating within DSB, please utilise the internal software links instead of browser back / forward buttons, as these may cause errors in some cases. A. Return to parent folder. B. Open this image in the digital image viewer C. Select a multimedia file to download to your local computer D. Click on weblink to access an external website E. Greyed-out content is only accessible after the questionnaire has been answered F. View questionnaire / quiz statistics (only available after answering the associated questionnaire or quiz) 7 of 61

5.2 Answering a Questionnaire or Quiz If a questionnaire or quiz is associated with a folder, access it using the Answer Case button from the information side panel in the Viewer. 5.2.1 Answering a Questionnaire A questionnaire may contain questions with various answer formats, and may be displayed as a single list, or as individual questions. A. Navigate to previous question Note: This functionality may be disabled by an administrator B. Navigate to next question C. Blue icon indicates the question links to an image feature or multimedia content D. Input answers to the field provided, in the appropriate format: Text Numeric Multiple choice Measurement of an image feature E. Save questionnaire progress to this point and exit 8 of 61

5.2.2 Answering a Quiz A quiz will consist of multiple choice questions only, which will be displayed individually. A. Continue to next question B. Blue icon indicates the question links to an image feature or multimedia content C. Select correct answer from the list of choices D. Blue icon indicates the answer choice links to an image feature or multimedia content E. Panel displays what question is currently being displayed, and total number of questions 5.2.3 Quiz Answers After completing a quiz, the Answer Case panel may display your percentage of correct results and a list of correct answers, if this feature is enabled by your administrator. A. Answer choice selected by user (green indicates answer is correct, red indicates answer is incorrect) B. Correct answer for this question will be indicated by a green tick-mark, while incorrect answers will be indicated by a red cross-mark 9 of 61

5.2.4 Questionnaire Statistics After scoring a questionnaire or quiz, users can access related statistics. Note: Visibility of statistics is controlled by the administrator. A. View answers from other members of the user group, displayed anonymously B. Open a graph of answers from the user group C. Return to the folder 5.2.5 Resetting a Questionnaire If enabled by an administrator, questionnare responses can be reset from the viewer so that the questionnaire can be completed again. A. 6. The Digital Image Viewer UPDATED The image viewer allows users to view, navigate and zoom digital images, and contains a variety of tools and options. Access the viewer by clicking on a digital image thumbnail. A. Navigate to the next / previous image in this folder B. Close the viewer C. Navigate around this image Navigate around the image using the joystick Use mouse to click and drag image Use keyboard arrows (up, down, left, right) to navigate Flip image vertically / horizontally, or change orientation by dragging the arrow around the wheel Zoom in and out using magnification tool Zoom up and down through focal planes (only available for multifocal images) Use the mouse wheel to zoom in and out Return to overview and original orientation with centering tool Right click on screen for preset magnification options 11 of 61

6.1 Viewer Tools A number of tools can be accessed from the top bar of the viewer. A. Show / hide information panel B. Scale bar Displays in um / mm, or in pixels if no base magnification is set C. Image overview Shows an overview of the image, with crosshairs indicating orientation and field of view Users can navigate around the image by selecting a point on the overview The image overview has the capacity to record a viewer s analysis of a image by way of heat mapping, with corresponding colours used for various magnifications D. Magnifier tool NEW View an area of the image at up to 2x the base scan magnification Note: This tool uses a digital zoom, and may display at lower resolution than the base scan E. Set image default view (access level = administrator or higher) NEW Lock display settings and field of view, so that this will be the default view for all users opening this image. F. Display settings Change colour balance, brightness and contrast, or display image in negative or greyscale. Changes will persist for this session only, unless locked using the Default View. G. Take a snapshot. This will generate a new window with three options: Jpeg image Static link: a url that will allow users to access the current field of view without logging into DSB Dynamic link: a url that will allow users to view this full image, including navigation functionality, without logging into DSB (access level = administrator or higher). Note: Snapshot links are persistent H. Annotation tool 12 of 61

6.2 Information Panel Digital SlideBox 5.0 Select images / media files and manage annotations, narratives and questionnaires within the viewer. A. Open an image from the current folder in the viewer, or use the + icon to open up to 4 images simultaneously NEW B. View / edit the folder narrative (editing access level = administrator or higher) C. Edit the folder questionnaire (access level = administrator or higher) D. Manage annotation layers (access level = administrator or higher) E. View annotation information for this image F. Select multimedia files from this folder for download NEW 13 of 61

6.2.1 Working with Multiple Images Work with up to four images at a time in the viewer. A. Green icon indicates which image is currently active B. Remove image from viewer C. Overlay the first image opened in the viewer (master image) onto this image D. Lock all images currently open in the viewer. When images are locked, navigation / zoom actions on the active image will be reflected by all other images. 14 of 61

6.2.2 Narrative Editor The narrative tool allows you to build up a review of the images in the current folder, associating external links and multimedia from keywords within the text. A. Save narrative details Note: Be sure to click SAVE on the narrative screen each time before you close it, to ensure your changes are not lost. B. Cancel changes to narrative C. Delete narrative D. Enter narrative text into text box E. Format narrative text Change text size NEW Set text to Bold / Italic / Underlined NEW Align text left / centre / right / justified NEW Set bullet points beside selected text 15 of 61

6.2.2.1 Linking Content to a Narrative Link selected text in the narrative to an image feature or external content. A. Link to a field of view in the current image B. Link to a web page Note: When entering URL, do not include http:// in the address C. Link to an annotation layer from any image in the current folder D. Link to an annotation from any image in the current folder E. Link to a file Select from a list of files in the current folder, or upload a new file from your local computer Note: New files will be added to the current folder, automatically set to not visible F. View the text that has been selected for this link 16 of 61

6.3 Annotations Annotations are visible areas of interest superimposed on the image that can be created without changing the underlying image. A. Select annotation name to zoom to that annotation, show details and hide all other annotations B. Hide / show all annotations on this image C. Box D. Ellipse E. Arrow F. Freehand G. Measurement Note: Measurement is calibrated based on the image magnification and the original capture conditions. Once the marker is drawn, the length of the measurement is filled in automatically and scales automatically from micrometres to millimetres to centimetres as you draw. H. Text box (access level = administrator or higher) I. Right clicking on an annotation offers the following options: Edit Redraw Note: The original annotation will need to be manually deleted using the eraser tool Reposition Delete Add new annotation 17 of 61

6.3.1 Creating an Annotation Select the annotation tool from the toolbar, or right click in the viewer window, to begin annotating. A. Open annotation toolbox B. Select annotation tool Note: When drawing mode is activated, the cursor will change from a hand to a cross-hair. Warning: If the annotation tool is opened, right-clicking on the screen to change magnification will disable drawing mode. Avoid this by using the magnification slide-bar to change magnification while drawing mode is active. C. Select annotation colour, visibility and fill D. Save annotation drawing E. Cancel annotation drawing F. Enter annotation title (mandatory) G. Enter annotation description (optional) H. Browse the current folder to associate a file with the annotation (access level = administrator or higher) I. Browse your local computer to associate a file with the annotation (access level = administrator or higher) Note: Files added from your computer are uploaded to the current folder, with their visibility status set by default to not visible J. Enter a URL to associate to this annotation (access level = administrator or higher) K. Save / cancel annotation details Note: After saving an annotation, you can redraw, delete, reposition or edit details by doubleclicking or right-clicking on the annotation. Note: When redrawing an annotation, right-clicking will allow you to redraw the same annotation shape, while double-clicking will allow you to select an annotation tool and draw additional shapes as part of the same annotation. 18 of 61

6.4 Layer Manager Digital SlideBox 5.0 Use the layer manager to create groups of annotations. (access level = administrator or higher) A. Select an annotation layer from the list B. Create a new annotation layer C. Edit selected annotation layer D. Set visibility for all annotation layers. NEW There are four layer visibility settings: E. Delete selected annotation layer 19 of 61

6.4.1 Managing Annotation Layers Manage layer details and select annotations that will be included in this layer. A. Enter layer name B. Set visibility for this layer only NEW Note: If the folder questionnaire is set to invisible, any annotation layer set to visible after scoring will default to visible C. Select an annotation Double-click or drag-and-drop annotation to add / remove it from this layer D. Add / remove all available annotations from this layer Note: If all annotations are removed from a layer, the layer will remain visible in the Layer Manager, but will not be visible in the Annotation Information E. Save changes to this layer F. Cancel changes to this layer 20 of 61

7. Search Tab Search Digital SlideBox for images, file, or folders. (access level = administrator or higher) 7.1 Image Search Search for images by choosing a parameter and inputting the search terms. A. Choose an operator from the dropdown menu. The choices are: = Exact Match!= Does Not Match > Greater Than (numbers and letters) >= Greater Than or Equal to (numbers / letters) < Less than (numbers / letters) <= Less Than or Equal to (numbers / letters) *wild Matching term after the asterisk wild* Matching term before the asterisk *wild* Matching term between asterisk B. Enter search parameter C. Select a image attribute to search for D. Select an annotation attribute to search for E. Submit search 21 of 61

7.2 Files and Folders Search Search for multimedia files and folders by choosing a parameter and inputting the search terms. A. Select a parameter B. Enter input C. Run search 22 of 61

8. Admin Tab After logging in, an administrator will be directed to the Admin tab. The workflow within Digital SlideBox allows administrators to set up projects in a stepwise fashion, from creating folders through to monitoring user activity. (access level = administrator or higher) A. Navigation bar (use to return to parent folders at any time) B. Sub tabs for adding content, managing users and viewing statistics C. Click to enter the folder of choice 8.1 Folder Contents Tab Create and access new folders and content. The number of folders per administrator is unlimited. Folders can be nested in order to structure content as desired. A. Create new folder 23 of 61

8.1.1 Managing Folder Content Folders can be populated with subfolders, digital images and multimedia content. A. Return to parent folder (this can also be accessed by clicking on the orange navigation bar) B. Drag content up and down to rearrange order C. Enter folder D. Navigate to previous folder E. Select / deselect all F. Edit folder details G. Delete this file / folder. A blue icon indicates that the folder for deletion is empty, while a grey icon indicates that the folder contains files. Warning: This action cannot be undone. H. Toggle folder visibility to users I. Select / deselect individual files / folders J. Navigate to next folder K. Create new folder / subfolder L. Reset original order M. Delete selected items. Warning: This action cannot be undone. N. Copy selected items Note: In order to protect user identities, user groups, scoring data and statistics are NOT copied along with other folder content. If there are sub-folders in the directory you are copying, the same constraints apply. O. Paste copied items here (becomes selectable after contents have been copied) 24 of 61

8.1.2 Edit Folder Details Click on the Details icon beside a folder to edit details associated with this folder. These details will be visible at folder level. A. Enter folder name B. Format description text C. Enter folder description Warning: Copying content directly from a webpage or word processor document may introduce hidden XML tags that affect display of the folder contents. For best results, input text manually or copy from a plain text editor. D. Return to folder view E. Save values 25 of 61

8.1.2.1 Viewer Configuration UPDATED Set behaviour of side panel and heat maps for images in this folder. Note: Changes apply to the current folder only. Viewer behaviour in subfolders will be default unless they are manually changed. A. Set whether viewer will load with side panel open or closed B. If side panel is set to be automatically visible, select which information panel will automatically load C. Set whether user heat maps will be generated for images in this folder Note: Heatmaps will be set off by default D. Return to browse page E. Reset to default values 26 of 61

8.1.3 Slide Details Select the Details icon beside an image to view / edit image metadata and configuration. 8.1.3.1 Slide Metadata View metadata associated with the current image. A. Click on thumbnail to open the image in the image viewer B. Navigate to the previous / next image in the folder C. Return to the folder contents 27 of 61

8.1.3.2 Edit Slide Details Modify the name and description of an image, and create new metadata attributes. 28 of 61

A. Edit image name B. Edit image description C. Select magnification at which image was scanned from drop down menu (where available, this will be set by default using scanner metadata) D. Restrict magnification at which the current image can be viewed E. View the user who imported this image F. Set stain for this image G. Set image quality Note: The image quality effects the compression of the image and is designed to provide marginal improvements for low bandwidth users when reduced. Increasing this value to maximum may have a negative impact on the speed at which the image loads. If unsure, leave at the default setting. H. Save image metadata details I. Delete metadata category J. Enter an attribute value for this image K. Enter name for this attribute L. Select this attribute M. Add a new metadata attribute to this category N. Delete selected attribute O. Enter a name for a new category P. Select a category that has previously been created for another image Q. Add category to this image R. Return to previous page 29 of 61

8.1.3.3 Slide Configuration The slide configuration field allows you to specify whether or not metadata and annotations will be visible for this image. A. Select to review / delete image annotations in the viewer B. Select to review annotation and layer visibility in the viewer C. Set visibility of annotations and metadata for this image Note: When a questionnaire is associated with the image, this can also be set to be invisible until questionnaire is scored. D. Return to previous page 30 of 61

8.2 Add Slides Tab Digital SlideBox 5.0 Add images to a folder from an image server. Note: Only Digital Image Hub image servers are supported for use with Digital SlideBox. A. Select one of the available links to an image server 31 of 61

8.2.1 Adding Slides Select the desired images from the image server to add to the current folder. A. Click on the icon to open image in a viewer B. Toggle between browsing and searching server for images C. Select desired images D. Select to import images with or without annotations and / or metadata added via Digital Image Hub E. Add the selected images 32 of 61

8.3 Add Multimedia Tab Add a variety of multimedia types or hyperlinks to a folder. A. Enter file name B. Browse your computer s folders to select the desired file C. Enter a description for the file (optional) D. Upload file Note: The maximum file size that can be uploaded is 20 MB Note: Digital SlideBox is not limited by file type, however users will only able to access content if their computer has the relevant applications installed to access files E. Enter name for the hyperlink F. Enter the hyperlink URL G. Enter a description for the hyperlink (optional) H. Upload the hyperlink 33 of 61

8.3.1 Adding Content After Scoring Content added to a folder after users have completed an associated questionnaire will be visible to users by default. A. Continue B. Cancel 34 of 61

8.4 Adding a Questionnaire or Quiz UPDATED Create and manage questionnaires and quizzes based on images and other folder content. Note: The questionnaire builder can be accessed via the Add / Edit MCQ button on the folder contents page, or from the questionnaire editor in the View Tools menu in the image viewer (access level = administrator or higher). IMPORTANT: Each folder is an individual case and all the slides / multimedia in the folder are part of that case. When a questionnaire / quiz is created in a folder, it applies to all slides within the folder. If users answer the questionnaire / quiz from any of the slides, it counts as the folder being scored. A. Add a questionnaire to the folder B. Add a quiz to the folder NEW 35 of 61

8.4.6 Dependent Questions UPDATED Set a question to be dependent on the answer to a previously created question, by clicking on Add Rule when creating a question. A. Set answer to the parent question that will cause dependent question to be displayed Note: Dependent questions will not be displayed to students unless they have completed the parent question according to the dependency rule, either in List View or Individual View. 8.4.6.1 Dependency Types Dependency rules can be created from three types of source question. Question Type Multiple Choice Numeric Text Input Dependency Rule Question appears when a specific option is selected from the list. Question appears when a specific value or a value within a specified range is entered Question is displayed when an exact word for word match to a specified phrase is entered. 41 of 61

8.4.7 Adding a Quiz NEW A quiz uses multiple choice questions only, with correct answers set when the quiz is created. Create a new quiz or add an existing one in the same way as adding a questionnaire. 8.4.7.1 Quiz Display Options Set display options for this quiz. A. Set whether results will be displayed when quiz is completed B. If results are displayed, select whether only student score will be displayed, or correct answer will also be displayed C. 8.4.8 Questionnaire Options On the folder contents page, an icon will display illustrating that there is a questionnaire associated with an image or folder. The visibility, move and deletion options are available here. (access level = administrator or higher) A. Open questionnaire for editing / preview B. Set visibility and permissions for questionnaire components C. Remove questionnaire from this folder Note: Removing a questionnaire does not delete it from the system permanently. It can still be selected when adding an existing questionnaire. Warning: A grey icon indicates that answers have been submitted to this questionnaire. Removing the questionnaire will permanently delete any submitted answers. This action cannot be undone. A CSV of any submitted answers should be downloaded before proceeding. D. Toggle questionnaire visibility to users E. Select this questionnaire for copying 8.4.8.1 Questionnaire Status Set user visibility and permissions for the current questionnaire. A. Set whether users can reset their answers to a questionnaire after submission B. Set visibility to users of various statistics: Gold Standard / Correct Answers, as selected by administrator Note: For questionnaires visibility of Gold Standard will be set, for quizzes visibility of Correct Answers will be set. Consensus statistics (percentage consensus between users on correct answer) Graphs (graphical representation of consensus statistics) Group Answers (other users answers displayed anonymously) C. 8.4.9 Editing Scored Questionnaires A questionnaire can be edited after users have scored. On the folder contents screen, the Add MCQ option becomes Edit MCQ when there is a questionnaire attached. 8.5.6 Adding an Existing User Group Existing user groups can be added to a current folder. Multiple administrators of the same Administrator group on the system can share user groups and access shared user groups. A. View this user group (click on name of group) B. Toggle sharing settings C. Delete this group (this does not delete any users associated with the group) D. Select user group Note: Shared groups created by another administrator cannot be altered. 8.6 Statistics Tab Using the statistics function, it is possible to review the activity of individual users within your group, for the folder to which the group is assigned. 8.6.3 Morph The morph shows a read-only view of individual user's image viewer allowing an administrator to see any annotations the user made, along with reviewing their diagnostic heat map of the image. This unique feature enables following the virtual diagnosis pathway of the user. Note: The heat map is switched off by default. It can be switched on for a specific image by rightclicking on the image overview window and selecting Generate Heatmap. Note: When viewing a user s heat map, right-clicking reveals two options: hide heat map, or clear heat map data (wipes the current heat map and resets it for this user s next visit). The varying colours correspond to areas viewed at various magnifications Blue = Maximum Magnification (e.g. 40x) Green = High Magnification (e.g. 20x) Red = Intermediate Magnification (e.g. 10x) 53 of 61
