BluesEnroll User Guide How to use this guide The most recent version of this BluesEnroll User Guide was created on October 23 rd, 2006, and is available to view in BluesEnroll. You may use the table of contents to locate the topic that you need assistance with. Printing this guide as a reference manual may also be helpful. Login to BluesEnroll 1. You may receive a Member Letter in the mail containing your Login ID and initial Password. Contact your Human Resources Department if you haven t received this information. 2. On the Internet, locate the BluesEnroll login screen as indicated by your Human Resources Administrator or go to www.bluesenroll.com. 3. Enter your BluesEnroll ID and Password. 4. Click the Login button or press enter on your keyboard. 5. The first time you login to BluesEnroll you will have to change your initial Password. 2006 Benefitfocus.com 3
Change your initial password Your password should be considered personal and confidential. 1. Create a new password following the criteria below. Your Password Must Contain: 6-15 characters At least 1 letter and 1 number Your Password Cannot: Be the same as your old Password Have more than 2 of the same characters in a row Contain your BluesEnroll ID 2. Reenter your password in the Reenter new password field, click Save. FAQ: I forgot my Login ID and password. Who can tell me what they are? If you received a Login ID card in the mail, you can find your Login ID listed on the card. Or, you can get your Login ID and Password by contacting a Benefitfocus representative at 1-877-336-8083 or your HR Administrator. 4

Logout Step 1: Authorizations and Terms and Conditions Page(s) If required by your Insurance Carrier, you may have to review and agree to Authorization to Disclose Protected Health Information and/or Terms and Conditions of Benefit Enrollment. If your carrier does not require this step you will begin on Step 2, The Welcome Page. Review the information presented on the screen and click the box left of the agreement statement. Step 2: My Home Page The My Home page will tell you what items need to be completed. Click the Start Enrollment button to move to the My Basic Information page. 6
BluesEnroll User Guide Step 3: My Basic Information The My Basic Information page presents you with the opportunity to enter your personal information. Click the Next button to begin entering your personal information. Review the basic information process and click the Start button to begin entering your personal information. 2006 Benefitfocus.com 7
Click the Next button to move through the basic information screens and update your personal information. Review your personal information and click the Save button. 8
BluesEnroll User Guide FAQ: How do I change information entered by my HR Administrator? Click the Edit button on the Review page. The first information entry page will display. Make changes as you proceed through each page. FAQ: If my Personal Information has a check mark beside it, do I need to do anything? You will want to review the information entered by your HR Administrator and perhaps add any optional information that is incomplete. FAQ: My Social Security Number is wrong. How do I change it? If your Social Security Number is wrong, inform your HR Administrator to have it corrected. 2006 Benefitfocus.com 9
Step 4: My Benefits The My Benefits page will display the benefits that you are eligible for and the time frame for enrollment. Click the Next button to move to begin entering your initial benefit elections. Review the basic enrollment process and click the Start button to begin entering in benefit elections. 10
BluesEnroll User Guide Step 5: Complete Your Current Benefits The benefits you are eligible for will be displayed under Your Benefits section on the Home page. You will need to complete, review, and save one benefit at a time. Required information is indicated with a bold, blue label. The first of several enrollment pages will display for you to enter information. Proceed from page to page by clicking the Next button. When you have entered all required information, a review page will display. On the Review page, look over your information closely. You will have the option to Edit or Save the information. If you need to change any information click Edit beside the section you would like to edit. Otherwise, click the Save button and you will be returned to the Home page. Step 6: Benefit Summary. Review your current benefit elections. Once you have completed entering in your benefit elections, review your benefit information then click the Next button. 12
BluesEnroll User Guide Step 7: Summary. The Summary Page allows you to print your Employee Detail Report. In the My Reports section of the Summary page click on the link for the Employee Detail Report. Print this report for your own records. 2006 Benefitfocus.com 13
FAQ: How much will my benefits cost me? Benefit costs are listed on the Review page for each benefit. FAQ: How do I change information I entered during online enrollment? Select Edit on the Review page for the item you want to change. The information entry page for that item will display. Make your changes and click the Next button. You will be directed through the necessary pages until you are returned to the Review page. FAQ: When will my benefits become effective? An effective date for each of your benefits is listed on each introductory and review page. FAQ: I entered information incorrectly for one of my dependents. How do I correct their information? From the Main Menu page, click the appropriate benefit listed under Your Current Benefits. On the Review page for that benefit, click the Edit button next to the dependent(s) you want to correct. You will be directed to the necessary page to edit your dependent(s). On that page, click on the dependent s name whose information you want to correct, and the make necessary changes. When you re done, click the Next button until you reach the Review page. Step 8: Your Paycheck Information (if present) Depending on your company you may or may not have a Your Paycheck Information section. If you do, you will have links to a W4 and/or a Direct Deposit section. You will complete, review, and save one section at a time. Required information will be indicated with a bold, blue label. If you have completed enrolling in all benefits listed on the Main Menu page, click the Next button and the W4 or Direct Deposit Introduction page will display. From the Introduction page, click the start button. The first of several payroll pages will display for you to enter information. Proceed from page to page by clicking the Next button. When you have entered all required information, a review page will display. On the Review page, look over your information closely. You will have the option to Edit or Save the information. If you need to change any information click Edit beside the section which you would like to edit. Otherwise, click the Save button and you will be returned to the Main Menu. Repeat the steps above for each benefit listed. 14
BluesEnroll User Guide FAQ: What do I do if I need to change information I entered? On the W4 Review page, click the Federal or State link. On the Direct Deposit Review page, click the link for the account that you need to update. In both cases, the information entry page for that item will display. Make the necessary changes and click the Next button. You will be directed through the necessary pages until you reach the Review page again. FAQ: When will direct deposit for my paycheck begin? Typically, direct deposit will begin within 2 pay periods. Please contact your HR Administrator for a definitive answer. Step 9: Enrollment Confirmation After you have completed the benefit enrollment process, a confirmation page will be displayed, click Next to return to the Home page. If you need to stop during the enrollment process, click the Stop button and then Logout. To log out of BluesEnroll, click the Logout button. Step 10: Log Out After you have completed the benefit enrollment process, click the Logout button on any page. If you need to stop during the enrollment process, click the Stop button and then Logout. 2006 Benefitfocus.com 15

Log Out Step 1: Review and Update Your Personal Information (optional) If available, you have the opportunity to review and update personal information in BluesEnroll. On the Home Page click the Personal and Work Information link under the View and Edit Information section. Your New Benefits Use the edit button to update your Personal Information. 16
BluesEnroll User Guide After updating your personal information click the Save button and you will be taken back to the Home page. FAQ: If my Personal Information has a check mark beside it, do I need to do anything? Required information will be indicated with a bold, blue label. Please note that New Benefits may or may not be the same as your Current Benefits. Even if you are enrolled in Current Benefits, you must enroll in the New Benefits you want for the upcoming period. From the Home page, click the Next button. The introduction page for the first benefit listed will display. If shown, click the A Note from Your Administrator link in the upper left corner of the introduction page. 2006 Benefitfocus.com 17

Repeat the steps above for each benefit listed. FAQ: If I want to renew my Current Benefits without changes, do I have to fill out the Open Enrollment benefits section? Yes. You must complete the new benefits section to be covered for the upcoming coverage period. FAQ: How do I change information I entered during online enrollment? Step 4: Print Your Employee Detail Report Your Summary section contains a report of all the information you entered in Your Personal Information, Your Current Benefits, and Your Paycheck Information (if present). You may print a copy of this report for your records. Depending on whether or not you have life benefits, you may or may not have a Beneficiary Form. Your Beneficiary Form contains all the information you entered about your beneficiaries. You may also print a copy of this form for your records. After you have completed enrolling in all your eligible benefits and entering your Paycheck Information (if present) you will be returned to the Main Menu. From this page, click the Next button. If you enrolled in life benefits and added beneficiaries you may print the Beneficiary Form. 2006 Benefitfocus.com 19
On the Home page click the Employee Detail Report link located in the My Reports section. A new window will open and display your Employee Detail Report. Click the Print button in the upper right corner of the report to print your report. Your report will print to your default printer. Step 5: Log Out After you have completed the benefit enrollment process, click the Logout button on any page. If you need to stop during the enrollment process, click the Stop button and then Logout. To exit completely from the Internet, click the X on the top, right corner of your web browser window. 20
