Completing a CAFAS in KaleidaCare Solutions Child and Adolescent Functional Assessment Scale Introduction KaleidaCare is pleased to have worked with Dr. Kay Hodges to make the Child and Adolescent Functional Assessment Scale - CAFAS available in KaleidaCare Solutions. Users must receive reliability training on the CAFAS before utilizing the CAFAS Instrument. In addition, payment for use of the CAFAS must be made directly through Dr. Hodges. (See below for contact information.) To access the CAFAS go to Outcomes > CAFAS > Add. If this is not available on a user s menu, it can be added by the agency system administrator through Menu Manager. CAFAS Sections The CAFAS Screen is broken into multiple sections. This manual will cover each of the sections along with its specific functionality. Add CAFAS This section provides information on how to contact Dr. Hodges at the Functional Assessment Systems office. In addition, there is information at the bottom of the section that displays how many CAFAS Instruments an agency has remaining. These usages must be purchased directly from Functional Assessment Systems. 3 of 14

Client Association This is where the user associates a client with the instrument. Use the First and Last Name Filter to reduce the list of available clients, and then click on Refresh Clients to populate the Selected Client dropdown. The appropriate client must be selected in the dropdown in order to save the CAFAS document. CAFAS Information The CAFAS Information section is where the following information is recorded: Assessment Date Next Due Date Period Being Rated (Last Month, Last 3 Months, Last 6 Months, Other) CAFAS Administration (1 st Evaluation, Exit Evaluation, 3 Month Evaluation, 6 Month Evaluation, as so on) Client s Event Tracking Event. The required fields are marked with document can be saved. and must be completed before the CAFAS Select a Trained CAFAS Rater The trained CAFAS Rater dropdown will default to the User s Name if they have the Role of Trained CAFAS Rater. 4 of 14

To ensure the user have the Trained CAFAS Role, go to Administration > Staff/Professional > Role. If the Trained CAFAS Rater is not in the Available Roles List it may be added through the Configuration Manager. Caregivers/Informant Sources This section identifies the client s Caregivers and the Informant Sources for the Instrument. Clicking on either link will open a new window that will allow for additional information to be entered on the Caregivers and Informant Sources. Caregivers When the Caregivers link is selected the user is able to identify the Primary, Non- Custodial, and/or Surrogate Caregiver. Once selected, all three fields are required. To add a caregiver, click on the Add button next to the type and fill in the required information. 5 of 14

Multiple caregivers can be added for each type by continuing to click Add. To delete a caregiver, click on the next to the item needing to be deleted. Once all of the caregivers have been entered, the user must click on Save to add the caregiver information to the CAFAS. If the user clicks Cancel or simply closes the window, the information entered will not be saved. Informant Sources When the Informant Sources link is selected, the informant and mode of information gathering can be identified in this section. There is a separate dropdown for each type of Informant and under each one there are the different modes of information gathering. Multiple items can be selected in each dropdown by holding down the Control Key. 6 of 14

Assessment Sections There is the ability to enter CAFAS Assessment Scores as well as Caregiver Assessment Scores. Below each of these there is a list of links identifying the individual sections. To enter a score, click on the corresponding Section Name. The lists all default to NR (Not Rated), but once a score is entered and saved, it will appear in place of the NR for each section. Scoring a Section To score the section the user can check the appropriate box. To view additional information regarding the specific item, hover the cursor over the item and the entire description will be displayed at the top of the list. Hoover the mouse of the selection item and view the entire description in the blue area. 7 of 14

Exceptions There is an option for Exceptions under each heading. When that option is selected there is a text box provided for the user to enter a description of the exception Enter Exceptions by checking the last item in the column and enter the description in the text field Could Not Score If a section cannot be scored, there is a Could Not Score check box at the top that must be selected. If the section can not be scored check the Could Not Score Checkbox 8 of 14

Completing Sections The individual sections can be completed by returning to the main screen and clicking on the links, or by clicking on the tabs along the top of the window once the first section has been selected. Complete each Section by clicking on the Section Heading Saving Sections Regardless of how the user completes the sections the user must select Save before closing the window or none of the information will be saved. 9 of 14

Editing Sections Once the information has been saved, the user can return to the sections in the same manner and is able to edit them at that time. Strengths and Goals Strengths and Goals for the Client and Caregiver are entered in this section. Click on the corresponding link under each heading to complete each section. Identifying Strengths and Goals To complete the section Strengths and/or Goals are identified by checking the appropriate corresponding check box. Completing Sections The individual sections can be completed by returning to the main screen and clicking on the additional links, or by clicking on the tabs along the top of the window once the first section has been selected. However, the user must select Save before closing the window or none of the information will be saved. Complete each Section by clicking on the Section Heading 10 of 14

Saving Sections Regardless of how the user completes the sections the user must select Save before closing the window or none of the information entered will be saved. CAFAS Reports The same reporting functionality is available on the CAFAS Report as with all KaleidaCare Solutions Reports: Inserting Page Breaks Sorting Inserting Signature Lines Downloading (if permission has been granted by System Administrator). Report Format The Report Formats includes: CAFAS Client Assessment Compressed CAFAS with Strengths and Goals CAFAS Progress Chart 11 of 14

CAFAS Progress Chart Report This Report compares two CAFAS Scores Additional Printing Graph Instructions If you have a problem printing the CAFAS Graph, please follow these instructions: In the Toolbar, go to Tools, >Internet Options Click on the Advanced Tab Scroll down to Printing and check Print Background Colors Click Apply, then click OK Note: Changing this setting will affect coloring when printing other reports.
