Experiencing a Workplace Issue? Don t delay. Take action within 30 days. AMAPCEO IS HERE TO HELP. 1 Jot down the details of your situation. Put together a written chronology of the matter. Collect documents supporting your position. 2 Find an AMAPCEO OPS Workplace Representative. You can find a trained volunteer Workplace Representative in your District on the AMAPCEO website at amapceo.on.ca. They don t have to be in your Ministry. 3 Contact a Workplace Representative about your matter. Give the Workplace Representative a brief, confidential background of your issue by phone or by email, and ask if they are able to assist you. 4 Report your workplace issue on RADAR. Login to your My Homepage on the AMAPCEO website and click on Report Workplace Issue to enter your case into RADAR, our secure web-based dispute tool.

My Homepage My Workplace Issues This is a closer look at the interface available to all OPS members. Here, you are able to see the status of the issues you ve already reported or seek assistance on a new issue. Of course, be sure to contact an OPS Workplace Representative first. My Homepage Report New Dispute When you click on the Report Workplace Issue button, a Help dialogue box will launch with instructions on how to use the tool, some frequently asked questions, and the ability to start a new online dispute report. A C A) A Start Here: Opens dispute portal to create new online record B) B Frequently Asked Questions: Expands to reveal answers B C) C Instructions: Please read carefully before starting A START HERE (REPORT NEW WORKPLACE ISSUE) When you click Start Here, a new page will appear with specific instructions and the ability to choose whether your issue is an Individual, Group or Classification dispute. If you select any other type of dispute, you will be advised that you must contact the AMAPCEO office. Note: there is a checkbox in E indicating the user has read this note. E) E Choose your dispute type and begin: Choose the dispute type from the dropdown and click NEXT. Completing the Form Completing the Form (Individual Dispute) Individual 1. From your AMAPCEO My Homepage, click Report Workplace Issue. 2. On the following screen, please read the section titled How to Use This Tool. Note that before you begin, you must have contacted a Workplace Representative who has agreed to take your case. 3. When ready, click Start Here. 4. On the following screen, please read the Important Note before beginning. 5. To start, use the dropdown to select Individual, check the box indicating you have read the Important Note, and click on Next. Please Note: if your dispute involves either Benefits or Surplussing/Job Security, you must contact an AMAPCEO Workplace Advisor. The next screen shows you the full form (next page). Completing the Form MEMBER INFORMATION You ll see that your core Member Information is already filled in. To edit this pre-populated information, click Edit my member information (top-right). Make sure to select how you would like to be contacted (either at work or at home). If you make any changes, be sure to click on Save Changes for them to take effect. OVERVIEW OF YOUR WORKPLACE ISSUE In the left column, you must include an overview or chronology of your issue. You may either upload a Word or PDF version OR use the text box to type in your content. Note that, after submission, you may work with your Workplace Representative to edit and update this information. In the right column, again, there is pre-populated information; complete at least the remaining required information (marked with a red asterisk). DATES AND DOCUMENTS The Date Submitted field will be auto-populated to today s date. Incident Date: Enter the date on which the incident occurred. This is important, as formal disputes must be filed 30 working days from this date. Note: If this dispute involves an ongoing or repetitive situation, please check the box provided and enter the most recent instance in the date field. Use the dropdown to select your dispute issue. NOTIFY A WORKPLACE REPRESENTATIVE From the dropdown menu, select the name of the Workplace Representative that you contacted and who has agreed to help you with this matter. It is the same process and form EXCEPT in the Dates and Documents section you will be asked to enter the names of everyone participating in your group dispute. Completing the Form (Classification) Completing the Form (Classification Dispute) From your AMAPCEO My Homepage, click Report Workplace Issue. On the following screen, please read the section titled How to Use This Tool. Note that before you begin, you must have contacted a Workplace Representative who has agreed to take your case. When ready, click Start Here. On the following screen, please read the Important Note before beginning. To start, use the dropdown to select Classification, then select whether its an Individual or Group dispute. PLEASE NOTE: The process and form for Individual and Group are the same EXCEPT, for the latter, you will be asked to enter all names of those involved in the dispute. Check the box indicating you have read the Important Note, and click on Next. The next screen shows you the full form. Please note that this form must be completed in a single session. MEMBER INFORMATION 1. You ll see that your core Member Information is already filled in. To edit this pre-populated information, click Edit my member information (top-right). 2. HAVE YOU FILED A STEP 1 CLASSIFICATION DISPUTE FORM YET? Select Yes or No if you select Yes, this means you have given your manager a Step 1 Classification Dispute Form. Important: If you select Yes, you must indicate the date on which you notified your employer; this date is important as if the issue is not resolved in Step 1, you and your Workplace Representative will have 30 working days from that date to advance your dispute to Step 2. Either now or in the near future, you can upload the Step 1 Classification Dispute form you submitted to your employer. You must click Continue to move forward with the form. INFORMAL CONTACT In the next section, enter any accounts of informal contact you ve had with your employer. Click Add a contact record, select the date of the contact and add your notes. This may be repeated for other days by clicking Add another contact record. For Members: Completing the Form (Classification) SELECTING DISPUTED ARTICLES Use the dropdown and Add Article button together to add as many Collective Agreement articles as you wish that are relevant to your dispute. As you go, if you make a mistake, simply click Remove Article. Note that you do not have to add Article 15.10 Classification, as this is a mandatory article for any dispute of this type. CURRENT JOB CLASSIFICATION INFORMATION Enter your current job class and indicate whether or not it is your home position. Use the file upload tools at right to add related documents. PROPOSED JOB CLASSIFICATION INFORMATION After entering your proposed job class, upload any related documents. PROVIDE A WRITTEN OVERVIEW This is required. You may either upload a Word or PDF OR use the text area to manually enter your overview. After submission, your Workplace Representative will still be able to replace or edit your overview, if necessary. NOTIFY A WORKPLACE REPRESENTATIVE From the dropdown menu, select the name of the Workplace Representative that you contacted and who has agreed to help you with this matter. Hit Submit when ready.
