Med Supp e-app Signature Process Help ensure your applicants complete and submit the e-app by taking a moment to familiarize yourself with the process. Email with Signature Options Once you submit applications to people for review and signature, those who gave you an email address will receive an email with two important items: authorization number and signature option(s). Depending on the situation, applicants receive one of these emails with their signature option(s): e-signature Only e-signature and Voice Signature If applicants don t give you an email address, direct them as follows: To see initial documents, send them to medsuppdocs.com To sign documents, you must provide applicants their authorization number and send them to signyourmedsuppapp.com 1

To e-sign This screen appears when applicants click the link in the email or on signyourmedsuppapp.com. Applicants complete these fields: Authorization Number. The authorization number appears for those who clicked the email link. Those coming from the website enter their authorization number. Date of Birth. Everyone enters their date of birth in this format: MM/DD/YYYY. It must match what you entered on the application. Applicants then hit Sign In. After sign in, the next page applicants see is either: Initial Payment if they want to enter their Social Security number and/or payment method themselves, then they get the Consent & Verification page OR Consent & Verification if you entered the above information on the application 2

Initial Payment If applicants want to provide their Social Security number and/or the payment method (only e-signature will be available) themselves, they see this screen. (In this case, the applicant provided the SSN to the producer for the application, so the field doesn t appear at the top.) Applicants who choose credit card are asked if the billing address is the same as the residence address provided for the application. Encourage them to answer correctly to ensure the credit card payment goes through: YES: If the address provided for the application matches the address appearing on their credit card statement. NO: If the address given for the application doesn t match the address appearing on their credit card statement. If no, they enter the billing address on this page. 3

Consent and Verification To e-sign, your applicant must follow these instructions: 1. Click Terms and Disclosures and e-signature Consent. An Adobe window opens. After review, applicants return to the Consent & Verification screen by either minimizing (-) or closing (x) the Adobe window, not the Web browser screen (keep it fully open). 2. Open and review each additional link. Return to the Consent & Verification screen by either minimizing (-) or closing (x) the Adobe window, not the Web browser screen (keep it fully open). 3. Click the box under the documents called I have read, received and kept a copy of the above documents. indicating the documents have been received, reviewed and /or read. 4. Click the I Accept box. 5. The Submit e-signature button is enabled and the applicant clicks it to submit the application. 1. The check mark indicates the applicant clicked the Terms and Disclosures and e-signature Consent form. Now the other documents appear for the applicant to click and open 2. Open each document until they all have a check mark. 3. The applicant clicks the box I have read, received and kept a copy of the above documents. 4. After the applicant clicks the I Accept box, the Submit e-signature button is enabled. 5. The applicant clicks this button to submit the application. 4

To Voice Sign Applicants call the number provided in the email and follow the prompts. They must provide their authorization number and date of birth as given for the application, MM/DD/YYYY. Tips and Help Contacts Here are a few simple things you can do to help your clients as they sign the Med Supp e-app: Stay on the phone. If you can t, give applicants your phone number and ask them to contact you if they have questions or concerns. Refresh your dashboard to verify completion. While you re expecting applicants to sign, verify they complete the process by refreshing your dashboard to see submitted in the status column. Encourage applicants to voice sign the app the same day it s completed. Rates and product changes are based on the date the app is signed. By signing/submitting it as soon as possible, it s less likely an application will need to be re-submitted. Remember, not all applicants are eligible for voice signature. The Thank you page tells you each applicant s option(s). Check your dashboard often. If you see applicants with pending signature status, contact them to find out why and remind them of the reasons to be timely. Above all, familiarize yourself with the Med Supp e-app before taking one. That way you know what s happening and can service the business before the sale.
