1.3 Navigating Cleansing CAT4 Menu s Click to access Cleansing CAT Menu Bar Predefined Report Selection Filters View or Hide additional menu items Click on the Export button to save or print your report Click on any heading to sort your data Double click on a patient to display their record in your clinical software Interactive Reports Cleansing CAT will provide access to a number of predefined reports indicating missing patient data. By double clicking on any of the patient details or the red box (indicator of missing data), you can access the patient record in Medical Director, Best Practice or Zedmed clinical software to update the required information. 1.4 Navigating Reports A report produced in CAT4 and Cleansing CAT will display to your screen. You can save your report in Excel, Word or PDF formats or print a hardcopy. 2. Getting Started 2.1 Collecting a Data Extract Data collection via the Collect button in CAT4, is available for Medical Director, Best Practice, Zedmed and Communicare. 1. To start a new collection click on the Collect button from the menu bar 2. Click OK in the dialog box that appears to continue with the collection 3. The data collection process will begin. OUTCOME: To produce current and up-to date reports, it is recommended to perform a data collection if the last extract is older than one month. Page 8 CAT4 Road Map

2.2 Opening a Data Extract 1. To select a data extract, click on the View Extracts button from the menu bar. 2. Select the required extract from the list on the left side. TIP: The newest extract will appear at the top of the list. Data Extracts Page 10 CAT4 Road Map

2.3 Using a filter 1. To view the filters, click on the View Filter button from the menu bar. 2. This will open a whole range of options for you to use, to filter your data for specific topics and requirements. Filters Page 11 CAT4 Road Map

2.6 Producing a report of Active Patients Option Two You can specify your own date range to categorise if a patient is active. This will produce a report of patients who have visited your practice at least once within the date range selected. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. Under the Date Range (Visits) tab in the filter panel, select the required date range. This can be via the predetermined time frames or by selecting a range from the from and to option. 3. Click on the Recalculate button from the menu bar 4. 3. Accreditation Preparation 3.1 Data Quality Dashboard This Report will display your practices current data quality in comparison to the RACGP standard. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. 3.2 No Gender Listed 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab select Other (This is located under the gender column) and Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. Click on the View Population button in the menu bar to view the results OUTCOME: This report can be used to identify and correct patient s records with no gender recorded. 3.3 Meet 90% of Allergies Recorded 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the Report section of the screen select the Allergies tab 5. Double click on the Nothing Recorded section of the pie graph to display a report of your patient details OUTCOME: The data produced will display patients with no allergies coded and/or recorded in their record. Using this list you can update incorrectly coded allergies or add a note to the patient s record for the GP to follow-up on their next appointment. Page 17 CAT4 Road Map

3.4 No Date of Birth Listed 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab select the No Age box under the Age column and select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the View Population button in the menu bar to view the results OUTCOME: This report would enable you to add a note into the patient s file as a prompt to update the missing information on their next appointment. 4.2 Missing Health Risk Factors via Cleansing CAT: Smoking 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. Click on the Cleansing CAT button from the menu bar 5. In the report section of the screen, select the Missing Clinical/Accreditation Items tab 6. Click on the heading Smoking within the table to sort the patients missing a smoking status to the top 7. OUTCOME: You will now have a list of all patients who do not have a smoking status recorded in your clinical software. There are a number of activities you could do: Refer to the new patient form to check if their smoking status was collected A note could be added to the patient s record to prompt the healthcare provider at their next appointment Contact your ehealth team at Brisbane South PHN to see if your clinical software is compatible with Topbar Page 22 CAT4 Road Map

4.3 Missing Health Risk Factors via CAT4: BMI 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the report section of the screen, select the Measures tab 5. Select BMI and Incomplete sub tabs 6. OUTCOME: Use this report to check the patient s record and see if their weight, height and/or BMI has been recorded, but hasn t been saved into the right location in your clinical software. If you have a lot of patients missing this information, try narrowing down your search to only include high risk patients and then gradually complete the rest. Page 24 CAT4 Road Map

4.4 Missing Health Risk Factors via Cleansing CAT: BMI 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. Click on the Cleansing CAT button from the menu bar 5. In the report section of the screen, select the Missing Clinical/Accreditation Items tab 6. Click on the heading Height or Weight within the table to sort the patients missing this measurement to the top 7. OUTCOME: You can use this tool to access the patient s record to see if their height, weight or BMI has been recorded, but hasn t been put into the correct location within the clinical software. You can select the Export button to view a patient report. Page 26 CAT4 Road Map

4.5 Missing Health Risk Factors via CAT4: Alcohol Consumption 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab enter in a Start Age of 18 under the age column and select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the report section of the screen, select the Alcohol tab 5. Double click on the Nothing Recorded section of the pie graph to display a report of your patient details Page 27 CAT4 Road Map

4.6 Missing Health Risk Factors via Cleansing CAT: Alcohol Consumption 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab enter in a Start Age of 18 under the age column and select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. Click on the Cleansing CAT button from the menu bar 5. In the report section of the screen, select the Missing Clinical/Accreditation Items tab 6. Click on the heading Alcohol within the table to sort the patients missing this information to the top 7. Double click on the patients details to open up their record in Medical Director, Best Practice or Zedmed (refer to the Getting Started section for more details on Cleaning CAT ) Page 29 CAT4 Road Map

OUTCOME: The information produced will show you patients 18 years and older who do not have alcohol consumption recorded in their clinical record. 4.7 Missing Health Risk Factors via CAT4: Physical Activity 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the report section of the screen, select the Measures tab 5. Select the Physical Activity sub tabs 6. OUTCOME: You can use this report to target patients with no physical activity recorded. If you have a large amount of patients, you could put together a patient update form and get all patients, new and existing, to complete it and bring it in with them when they see their clinician or the nurse, to help prompt the conversation. 4.8 Correctly Coding Health Conditions via Cleansing CAT: Diabetes This search produces a list of all the patients who are not coded as having Type 2 Diabetes but who have risk factors associated with Type 2 Diabetes. The information shown is indicative only. It is intended to be used as a way of searching a patient s record to see if they could have a chronic condition, based on the search terms, but have not been correctly coded. The search terms are generic and may not include every person with a condition. The search terms could also indicate that a person has a condition, when in fact they do not. Correctly coding a patient as having a chronic condition should be done with the patient s general practitioner. The list produced is designed to give a starting point to correctly coding and cleaning your database. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. 4.10 Correctly Coding Health Conditions via CAT4: Asthma This search will produce a list of patients who are listed as taking respiratory medications but are not coded as being asthmatic. This report can be used to help you find and correctly code patients with Asthma. The information shown is indicative only. It is intended to be used as a way of searching a patient s record to see if they could have a chronic condition, based on the search terms, but have not been correctly coded. The search terms are generic and may not include every person with a condition. The search terms could also indicate that a person has a condition, when in fact they do not. Correctly coding a patient as having a chronic condition should be done with the patient s general practitioner. 5. At Risk Groups 5.1 People at Risk of Developing Chronic Disease 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. From the filters panel click on the Conditions tab 5. Under the Chronic sub tab, select the option No for the chronic conditions you wish to include in your report (you can select one or more conditions, for the purchase of this example we will be selecting No to Type 2 Diabetes, Hypertension and CHD) 6. In the report section of the screen, select the risk factors that relate to the chronic disease you are filtering by (For the purchase of this example we will be using smoking, alcohol and BMI). 8. Select the Smoking tab 9. Select Daily Smoker, Irregular Smoker and Ex-Smoker, this can be done by single clicking on each section within the pie graph, the words or the legend. 10. OUTCOME: The report will show all patients that have no chronic condition record for the conditions selected in the filters, but who do have risk factors associated with chronic disease. It might be a good idea to promote a Health Check to these patients if they are eligible, and you could also make a note in their record for a discussion about these risk factors at their next appointment. 6.2 Chronic Disease: MBS Eligibility This report can help you to find patients who have a chronic condition (i.e. CHD, CKD, COPD etc.), but have not been billed a related MBS item. This search is indicative only and cannot take into account if an item has been billed at another practice. It is designed to give an initial reference point. For the purpose of this example we will be using COPD. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. From the filters panel click on the Conditions tab 5. Under the Chronic sub tab, select the option Yes for COPD 6. Click on the Recalculate button from the menu bar Page 46 CAT4 Road Map

7. In the report section of the screen, select the MBS Eligibility tab 8. Select the GPMP/TCA Eligibility sub tab 9. Double click on the section of the bar graph that you would like produce a report on. Page 47 CAT4 Road Map

7. Mental Health 7.1 Searching for People with a Mental Health Condition This search can help you to identify patients with at least one coded mental health condition. The information shown is indicative only. The search terms are generic and may not include every person with a mental health condition. If a mental health condition has not been correctly coded, the patient will not show up in this search. The list produced is designed to give a starting point only. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. 7.2 MBS Eligibility 9. In the report section of the screen, select the MBS Items tab 10. Select the Not Recorded sub tab 11. On the bar graph shown, you have two options a. Select all related MBS items by clicking on them once: GP MHTP, 2712 (GP-MHTP Review) and 2713 (GP-MHTP Consult). And then selecting the Report button from the menu bar b. Select each MBS items ( GP MHTP, 2712 (GP-MHTP Review) and 2713 (GP-MHTP Consult) ) one at a time by double clicking on each to produce 3 separate reports OUTCOME: The reports produced will display patients who have been correctly coded as having a mental health condition, but who have not been billed the specific item numbers at your practice. 8. Health Promotion 8.1 Health Check: 75+ Health Check Use this search to generate a report of possible eligible patients that have not had a 75+ health check at your practice. As this search uses information from your billing software it cannot distinguish if a patient has had a 75+ health check at another practice. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel under the General tab select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the report section of the screen, select the MBS Eligibility tab 5. Select the Health Assessment Eligibility sub tab 6. Double click on the HC 75+ not claimed in last 12 months section of the bar graph to produce a report. Page 52 CAT4 Road Map

OUTCOME: The information shown on this report is for all 75+ patients in your practice who have not been billed for a75+ health check in the last 12 months. This information could be used to create a mail out list to patients. Page 53 CAT4 Road Map

8.2 Health Checks: 45-49 Health Check This search can be used to find patients who are aged between 45 and 49 with at least one risk factor for chronic disease and have not been billed a 45 to 49 year old health check at your practice. This search will not be able to distinguish if the patient has had a 45 to 49 year old health check at another practice as it draws information from your billing software only. Note: For the purpose of this example we will be using daily smoker as a risk factor. You can select one or multiple risk factors. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. 8.3 Health Checks: 715 Health Assessment This search can be used to find patients who recorded as Aboriginal and/or Torres Strait Islander and who have not been billed a 715 health check at your practice. This search will not be able to distinguish if the patient has had a health check at another practice as it draws information from your billing software only. 1. Click on the Clear Filters button from the menu bar before starting a new search 2. From the filters panel select Active Patients (refer to Producing a report of Active Patient section for instructions) 3. Click on the Recalculate button from the menu bar 4. In the report section of the screen, select the MBS Eligibility tab 5. Select the Health Assessment Eligibility sub tab 6. 8.5 Health Promotion: Pap Smear This search can be used to generate a report of females who are due or overdue for a Pap smear. TIP: You don t need to specifically filter for females, CAT4 has automatically filtered out the male population leaving just the female and other gender. 1.
