Introduction The Repeat Management System (RMS) within is designed to cater for pharmacists who operate a repeat prescription collection service for their patients. Currently there are a variety of methods in which the repeat collection service can operate within pharmacies. RMS can cater for most of these methods, including Repeat Dispensing (batch dispensing) as stated in the Pharmacy Contract, where doctors can issue batches of prescriptions for patients on stable chronic conditions. RMS can be a useful tool in managing the repeat service workload, thereby allowing you to provide an efficient service to your patients. In, the Repeat Management process is split into two sections: 1) Setting up a Repeat Request which includes: Printing consent forms; Selecting the type of service required (collection only, repeat reminder, full collection and delivery); Selecting items to be requested; Printing a repeat request form to be sent to a surgery (if required); Setting the repeat reminder type (reminder, request, batch etc.); Setting a reminder date (which reminds you to request medications, process a batch etc.). 2) Processing a Repeat Request which includes: Using the Repeat Manager to process reminder scripts or track the progress of a request; Updating the status of the request so that you know its progress right from creating the request up until the point of collection or delivery of the items to the patient; Send an SMS text reminder to let a patient know that their medication is ready for collection (if enabled); Using reports and filters to simplify your workload (optional). 3 Rx Systems 2013

Setting up a Repeat Request The first part of the repeat management process in is setting up a repeat request, which includes: 1. Obtaining a patient s consent; 2. Adding repeatable items into the RMS Register; 3. Setting up the reminder type and date. To begin setting up a repeat request, type in the name of the patient from the Main Screen and select the patient from the list to enter his or her patient medication record (PMR). Tip: Before setting up a repeat request, ensure that all patient details and GP details (i.e. name and surgery) are correct. This will ensure that the reports generated are done so correctly. Patient Consent Once you have entered the PMR, select the [F4 Repeat Request] button: If you are setting up a repeat request for the first time, will display the following alert to print a consent form (as well as save the consent information): Note: As a legal requirement, pharmacies are required to obtain a signed consent form from a patient in order to handle prescriptions on their behalf 4 Rx Systems 2013

Selecting [Yes] will display the Patient Consent window (shown below) which allows you to: Select the type of repeat service the patient has signed up to: o Collection Service Only o Repeat Reminder Service o Full Collection and Delivery Service Save consent status Print the consent form and save the consent status at a later date Print the consent form and save the consent status Revoke consent Select the type of service here Click here if you only want to save the consent status and do not want to print the consent form Click here to print the consent form but not save the consent status Click here to print the consent form and save the consent status Tip: You may have generated your own paperwork to capture patient consent and may not require the consent form. You should still say [Yes] to the alert above and select [F8 Save] in order to acknowledge that consent has already been obtained. Selecting either [F8 Save] or [F10 Print and Save] will save the consent status and take you to the Repeat Items window, where you will be able to mark items that have been requested. 5 Rx Systems 2013

Selecting [F9 Print] to print the consent form will display a preview of the consent form (as shown below). After printing the form, you will be returned to the Patient Consent window. Click here to print the form If you chose to escape from the Patient Consent window without saving consent, you will still be taken to the Repeat Items window. Please note that pharmacies are required to obtain a patient s signed consent in order to handle their prescription requests. Tip: s consent form in RMS caters for receiving the patient s prescription in both paper and electronic format. 6 Rx Systems 2013

Repeat Items After printing a consent form and/or saving the consent status, the next window displayed is the Repeat Items window. The status column Use these two checkboxes to print or show only repeatable items (items with an R in the status column) The Repeat Items window above shows all the drugs dispensed to the patient in the last 365 days based on their PMR history. You can set the status of any item in the list as one of the following: R Requested (item is to be requested) S Stopped (item is no longer required; removed from the request in RMS) A Acute (one-off item; won t be added to repeat) When entering the Repeat Items window, the status column is automatically highlighted. Simply use the up and down arrow keys on your keyboard to move through the drugs and set each item to either R, S or A. Alternatively, you may click on the box to the left of each item and type in the relevant letter using the keyboard. 7 Rx Systems 2013

Repeat Items Function Keys The functions keys to the left-hand side of the window can be used to perform the following functions: Add an item to the request that has not been previously dispensed for the patient. This will not make an entry in the PMR Automatically set the status of all undefined items to R Change the status of repeatable items ( R ) to a blank status and vice versa Print a medication request form for all the repeatable items Add repeatable items to the RMS Register, where you can set the reminder date and type Displays the Patient Consent window (see Patient Consent) Displays the Repeat Manager window (see Repeat Manager) Amend dosage instructions or quantity if the selected item has changed since the last dispensing. This will not amend items in the PMR. Tip: When you send the initial repeat request for a patient, it is useful to set the status for Acute ( A ) and Stopped ( S ) items. This will ensure that incorrect items are not requested. After setting the status for all the repeatable items, save your changes by selecting [F10 Save]. 8 Rx Systems 2013

If you have not already added the items to the RMS Register, a prompt will appear as shown: Selecting [Yes] will add items with a repeatable status into the Repeat Manager (see Repeat Manager). Selecting [No] will simply save the repeat status and return to the PMR. Tip: In order to use the full RMS functionality effectively, select [Yes] to add the repeat request to the RMS Register. If you have not already printed the repeat request, a prompt will appear as shown below. Selecting [Yes] will display the following print preview: 9 Rx Systems 2013

Repeat Cycle Once you have saved the repeatable items and added them to the RMS Register, you will see the Repeat Cycle window. This window will allow you to manage your workload by setting reminder dates for the prescriptions. It also allows you to tailor the RMS to the type of service being provided. For training purposes, the following screen has been split into three sections: 1 2 3 Section 1: this is where you set the reminder date Section 2: this is where you set the reminder type Section 3: displays the patient details 10 Rx Systems 2013

Setting a Reminder Date (Section 1) Section 1 is where you are able to set a reminder date. This is the date you will be reminded in the Repeat Manager that a prescription is due. You can set the reminder date either by using the on-screen calendar or the function keys located to the right-hand side of the calendar. Tip: By default, the function keys have been set to +21, +28, +84, +1 and -1 but these can be changed by contacting our Service Desk. 11 Rx Systems 2013

Setting the Repeat Service Type (Section 2) Section 2 requires you to set the type of repeat management service you would like to use for this patient, which are explained below: Set the repeat management service type here Enter any notes here. These notes will appear beside the reminder in the RMS Register R Select this option to create a reminder. This option can be useful if you want to be reminded when a prescription is due for a patient so that you can contact them and ask if they wish for the repeat prescription to be collected on their behalf X Select this option to remind you that a repeat prescription is due, and that a repeat request form needs to be sent to the GP B Select this option if you are processing a batch of prescriptions. When this radio button is selected, you will see a small set of options as shown below: These options require you to enter details of the batch prescription. Use the up and down arrows beside each option to set the batch number, the total number of prescriptions in the batch, and lastly the prescription period. Delivery Mode The delivery mode section is used to set whether the medications will be collected or delivered to the patient. Tip: The delivery mode is automatically set if Collection or Delivery is selected under Additional Services. This can be found in the PMR under [F1 Patient Details]. 12 Rx Systems 2013

Patient Details (Section 3) Section 3 displays the patient s details for reference purposes. You can view a preview of the prescription listing only the repeatable items by selecting the [F8 View Prescription] button. Selecting [F10 OK] will save any changes you have made in the Repeat Cycle window to the RMS Register and will return you to the PMR screen. Note: If you have marked any items with a status of S (stopped), you will see the following prompt: The PMR will look as follows: If you have marked items with S, they will appear in red in the PMR Items marked with an R will be highlighted in purple in the PMR This completes the process of adding items to the Repeat Manager. 13 Rx Systems 2013

Setting Reminders for Patients Follow the steps below if you wish to be reminded at the end of a dispensing to either request items for a patient, or to offer the service to a patient who may benefit from you managing their repeat requests. In the patient s PMR, select [F1 Patient Details]. Tick this checkbox to set reminder messages to appear for this patient at the end of a dispensing Select [F10 Save]. The next time you complete a prescription, you will be prompted with the following message: If you select [Yes], you will be taken through the process of setting up reminders as discussed previously (see Repeat Cycle). Selecting [No] will return you to the Main Screen. 14 Rx Systems 2013

Processing a Repeat Request After adding items to the RMS Register, you will need to access the Repeat Manager daily to either: View prescriptions that are due, or Create reports on actions that are required to complete a repeat request Processing a repeat request or a repeat reminder usually requires the following steps: Sending the repeat request form(s) to a surgery (if required); Collecting the repeat prescriptions from the surgery and marking the items as booked in; Processing the prescriptions (this involves dispensing and generating labels for the items); Marking the records as collected or delivered; Marking the records as completed. The Repeat Manager screen can be accessed by selecting the [RMS] button from the Main Screen, as shown below. Alternatively, you can enter [Alt+R] using your keyboard. 15 Rx Systems 2013

Repeat Manager The Repeat Manager is designed to simplify the repeat prescription request process by indicating which steps are outstanding in order to complete a repeat request that has been added to the RMS Register. It does this by displaying the stage the prescription request is currently at and the steps that are required in order to complete the record in the RMS Register. For training purposes, the screen has been split into 4 sections: C B A D These sections are as follows: Section A: displays the date ranges and filters Section B: displays a list of the repeat requests (i.e. the RMS Register) Section C: displays the functions keys Section D: displays additional features that can be used These are explained in further detail below. 16 Rx Systems 2013

Date Ranges and Filters (Section A) The options at the top of this section are used to apply a date range to the RMS Register. By default, it will show reminders for today s date but you can expand or narrow this range by either: Using the predefined date ranges; Using the calendar icons; or Free typing the date in the From and To sections. The checkboxes beneath the date ranges allow you to include or exclude different records. Simply tick or un-tick the ones you require and the RMS Register will update accordingly. RMS Register (Section B) Section B is the RMS Register. It shows a list of reminders for the date range and filters specified: This column indicates the type of repeat management service being used (R, X, B) This column is only used for batch prescriptions. In this example, the repeat is for script 1 of 6 The columns A, B, L, C, D, and S (highlighted by the green box) show the actions required to complete the repeat request. These stand for: A At Surgery B Booked In L Labelled C Collected D Delivered S SMS To simplify the process, only the letters shaded in blue are applicable to the repeat reminder selected. To perform an action, i.e. mark a reminder as at surgery, you will need to use the function keys, which are explained later. 17 Rx Systems 2013

As mentioned, only the actions required by you will be shaded in blue to simplify the process. To demonstrate, note the three patients in the screenshot below: Mr Daniel Test needs to have A, B, L, and D columns ticked; as he was set up as a delivery patient, he has the D Delivered column shaded. Mrs Fiona Test already has the A and B columns ticked; this is because this record is a batch reminder and all the prescriptions in the batch should already be in the pharmacy, therefore, do not need to be requested. The only columns that need to be ticked are L and C. Mr Patient RMS needs to have A, B, L, and C columns ticked; as he was set up as a collection patient, he has the C Collected column shaded. Note: If a patient is marked for delivery and wants to collect their medication (or vice versa), simply press the Collection (or Delivery) function key and you will be prompted to confirm the change. 18 Rx Systems 2013

Repeat Manager Function Keys (Section C) The function keys are displayed in Section 3; in most cases you will be using them while processing a repeat request. Each key reflects a phase that needs to be carried out in order to successfully complete a repeat request and are usually used in sequence; they are explained below: Access the PMR screen for the selected patient After sending a repeat request to the surgery, mark the request as At Surgery ; this adds a tick in the A column Once a prescription has been received back from the surgery, mark the request as Booked In ; this adds a tick in the B column Use this button to take you through the RMS Dispensing Process (see Dispensing a Repeat Request) If you have processed a prescription request using the [F4 Process] button, you will have been prompted to mark the script as ready. If you selected [Yes], a tick would have been placed in the L column. If you prefer dispensing via the patient s PMR screen, you can do so and return to the Repeat Manager, highlight the prescription request and use this button to place a tick in the L column Once a patient has collected their medication, select this option to place a tick in the C column Once a patient has had their medication delivered, select this option to place a tick in the D column Mark the repeat request record as completed and remove it from the RMS list thereby completing the repeat service for the patient Amend the notes, reminder date, reminder type, and delivery mode. You can also remove a tick by clearing the date and time stamp adjacent to the phase carried out (see below) 19 Rx Systems 2013

Edit the reminder date here Edit the repeat management service here Select any of the Clear buttons to clear the corresponding action and remove the tick in Section A. You can also amend the date by clicking on the relevant box and editing the date Summary As mentioned previously, most of the function keys (with the exception of [F1 PMR] and [F9 Edit Record]) are usually used in sequence to mark the stage the prescription is currently at. Below is a typical example of the sequence these keys would be used in: 1) The repeat request form has been sent to the surgery (i.e. the request forms have left your pharmacy). Select [F2 At Surgery]. 2) The surgery has sent a prescription for the items requested. Select [F3 Booked In]. 3) You are now ready to process and endorse the prescription. Select [F4 Process]. 4) The prescription has been endorsed and the items have been labelled. Select [F5 Labelled]. 5) The prescription has been collected by or delivered to the patient. Select [F6 Collected] or [F7 Delivered] respectively. 6) All required actions have been completed, and you wish to mark the repeat prescription request as complete and remove it from the RMS Register. Select [F8 Completed]. 20 Rx Systems 2013

Additional Features (Section D) This section contains several buttons that can be of use in performing tasks such as editing/deleting a patient s RMS record, re-printing a repeat request form, sending SMS messages, and searching through the register for a specific patient. It also contains several printable reports. These buttons are explained in detail below. Print the repeat request form for the selected patient Search for a specific patient in the Repeat Manager Send an SMS alert for the selected patient when the prescription is ready (if SMS is enabled in ) Allows you to edit the selected patient, print a consent form or display all RMS records for that patient If you are viewing records for a selected patient, click on the dropdown arrow again to display all patients Delete the selected record (or a batch of records using the drop down button) from the RMS Register Generate a variety of reports to help manage your workload (see Repeat Management Reports) 21 Rx Systems 2013

Sending an SMS Alert in The SMS feature allows you to send a text message from your computer to a patient s mobile phone, and in turn, they can reply to the text which would then be received to an email address of your choice. This feature can be useful for reminding a patient that their medications are ready for collection, or simply to offer the collection and delivery service to a patient who may otherwise be unaware of the existence of the service. Tip: If you would like this functionality activated, please contact our Operations Team who will be able to provide you with more information. If you already have the functionality enabled, follow the steps below to use it within RMS. Ensure that you have a mobile number populated for the patient in the Patient Details window accessed from the PMR. This will enable the SMS button in RMS. If a mobile number has not been populated, the SMS button will remain inactive. Enter the mobile number here In order to send a message in RMS, ensure this checkbox is selected SMS messages can be sent from here but a tick will not be placed in the S column in RMS In the Repeat Manager, ensure you have highlighted the correct patient. Click on the [Send SMS] button and you will see the Send Text Message window. Select here to schedule the text to be sent at a specific date and time Select here if you would like replies from the patient to be sent to an email address you specify Type your message here. By default, a message will be pre-populated but this can be amended. Contact our Service Desk if you wish to change the default message 22 Rx Systems 2013

Batch Processing Some of the function keys described above allow for batch processing. These features allow you to perform an action for multiple reminders. The batch processing feature can be useful as many of the actions you would usually carry out on a day-to-day basis would be required for more than one reminder. Press ALT with the relevant function key using your keyboard or use the drop down arrow beside the function keys to access the batch facility For example, when you send a bundle of repeat request forms to a surgery, you are likely to be sending more than one request form at any one time. Instead of marking each record as At Surgery one by one using [F2 At Surgery], use [Alt+F2 Batch] or the drop down button. This will then display the Batch Processing window: Add or remove specific records using these checkboxes Use these buttons to either select/de-select all records or to toggle the selection Selecting this button will then mark items with a tick as At Surgery 23 Rx Systems 2013

Dispensing a Repeat Request After receiving a prescription from the surgery (in response to sending a repeat request), you will need to label and endorse it. The Repeat Manager includes a utility for processing these prescriptions so that all repeat dispensing can be done from within the RMS module. There are two ways to process repeat requests added to the RMS Register: Access the Repeat Manager using the RMS button from the Main Screen. Highlight the patient s repeat request from the RMS Register and select [F4 Process]; Access the patient s PMR. If they have any outstanding actions that are to be completed in RMS, you will see an alert. Select [Yes] to this alert to view outstanding records for only this patient in the RMS Register. Use [F4 Process] to process accordingly. Note: If there are any outstanding actions in the RMS and you select [No], you will be taken to the PMR. You can still dispense the items but you will need to access the RMS module to mark the relevant actions on the request (i.e. labelled, collected, delivered, or completed). In either of these methods, you will see the Process Prescription screen: If you wish to add any notes, you may do so by entering them into this box. These will appear in the Repeat Manager under the notes column If the quantity expected is different to the quantity you have received, click here to highlight the received quantity and type in the actual quantity received This window is used to confirm that all items requested have been issued on the prescription you have received. By default, all items will be ticked. If an item is not on the prescription received from the surgery, remove the tick beside the item. The patient may have requested a large number 24 Rx Systems 2013

of items which have been prescribed over a number of prescription forms. Process each form individually by selecting only the items listed on the form. Select [F10 Process] to begin the dispensing process. Note: Once you have selected [F10 Process] and begun the dispensing process, all items selected on the repeat request will be marked as processed. These will be greyed out in the Process Prescription window. There may be instances where you have begun dispensing but have to abort the dispensing process whilst in the PMR screen e.g. wrong patient selected, item(s) on the repeat request do not match the prescription received, need to access another patient s PMR, etc. If this occurs, you can re-activate items in the Process Prescription window using the [F8 Activate All Records] button (see screenshot above). To reactivate all records, select this button. If you only want to re-activate some of the items, highlight the item, use the drop-down button beside [F8 Activate All Records] and then select [F9 Activate Selected Record]. Alternatively, highlight the item and enter [F9] on your keyboard. You will then see the Prescription Comparison window. Depending on your configuration settings, this window may have a different appearance. You may either see the following Prescription Comparison window: (Prescription Comparison window A) This button will allow you to change the order of the items This button will display the PMR history on the right-hand side of this window 25 Rx Systems 2013

Alternatively, you may see the following Prescription Comparison window: (Prescription Comparison window B) This option uses the dosage instructions from the repeat request and processes each item individually This option is the same as the [F2] button but applies any quantity changes made in the Process Prescription screen This option uses the dosage instructions from the repeat request and fast-tracks through the prescription Both of these windows can be used to either view or compare the prescription being processed to the last dispensing for the patient. After reviewing this screen, select: [F10 OK] in Prescription Comparison window A to continue Any of the options in Prescription Comparison window B to continue. 26 Rx Systems 2013

The next few windows you will see will require you to confirm the quantity and dosage instructions for each item on the prescription (see below). If you selected [F9 Process This Script Using Last Medication (Quick Dispense)] on Prescription Comparison window B, you will bypass the windows below. Confirm if the quantities and dosages are correct by selecting [F10 OK] otherwise amend the information accordingly as you would with a normal prescription. Enter F10 - OK Next Item When you have finished confirming/editing each item, the window will remain in the PMR screen with a red dot beside each item that is being endorsed (see below). At this stage, you may wish to: add additional items that have been included on the prescription, create owings, or mark items as Not Dispensed. Once you are ready to endorse your prescription, select [F8 Last Item] to display a preview of the prescription. Select [F10 Endorse Script] to endorse the prescription and generate labels. After this stage, you will be prompted with the following prompt. Selecting [Yes] will automatically tick the L column for Labelled. 27 Rx Systems 2013

Repeat Management Reports Selecting [F11 Reports] in the Repeat Manager will open the Repeat Management Reports window. From this window, you are able to generate and print several useful reports to aid you in effectively managing your workload. Each tab represents a different report that can be generated. Furthermore, at the end of running certain reports, you will be able to batch mark a group of repeat reminders if you choose to do so. Each of the tabs has a date range filter which defaults to Today however this can be changed depending on the date range you wish to view. All reports with the exception of the RMS Work Load Sheet can be filtered further by doctor, surgery and/or by a specific patient. Tip: As stated earlier, before setting up a repeat request, you must ensure that all patient details and GP details (i.e. name and surgery) are correct. This will ensure that the reports generated are done so correctly. If you amend any of the GP details after a repeat request has been created, the reports will use the original details. You may need to recreate the repeat request in order to run the report using the updated GP details. There are also filters in the bottom-left of each tab that are specific for each report. These are explained in more detail below. Note: If you attempt to run any of the reports and are told that there are no records to print, you may need to adjust your date filters based on the reminder dates you have set. 28 Rx Systems 2013

RMS Work Load Sheet This report lists all patients entered onto the RMS based on the date range specified and the type of repeat management service that they have been set up for. To print this report, access the Repeat Manager and select [F11 Reports]. In the Repeat Management Reports window, RMS Work Load Sheet is usually selected by default. If it isn t, simply left-click on the tab and it will be highlighted in red. In the bottom-left of the window, there are four additional filters that can be used. These can be useful if you wish to view patients for a specific repeat management service. Once you have set your filters, select [F10 Run Report]. A print preview of the report is displayed: 29 Rx Systems 2013

Surgery Drop Sheet The report lists all patients whose repeat request form has yet to be sent to a surgery. The report generates a list based on any patients who do not have a tick in the A / At Surgery column. Redeeming Overdue Prescriptions You may have a scenario where you have generated an overdue script (i.e. an emergency supply at the request of a patient), and then created a prescription request for the item(s) using the RMS module. Tip: When generating the request, you may notice that the quantity of the items supplied may need adjusting. Using the RMS Module If you choose the RMS module to redeem the prescription, process the prescription as described above (see Dispensing a Repeat Request). However, before you endorse and label the prescription, delete the item that was supplied using the [Alt+D Delete Item] button. You will then need to select the item that was supplied from the PMR list. You will see the following alert where you should follow the prompts on screen: This method will ensure the outstanding RMS actions are completed. Using PMR or Overdue Scripts Button If you choose to redeem the prescription directly from the PMR or using the [F8 Overdue Scripts] button on the Main Screen, you will need to manually access the RMS module and mark the request as completed. 35 Rx Systems 2013

The request has been labelled and marked as completed but the PMR still shows the item in purple. What should I do? Answer: There are two options here: If the prescriber has stopped the item, access the patient s PMR, select [F4 Repeat Request] and replace the R with an S.
