1. About the application The Corin RPM Patient App helps you track and manage a wide range of health data from simple manual logging to syncing with connected health apps. It is packed with science-based rules that provide a reference range on all data that it collects for weight, steps, sleep and mood. Health Dashboard: See all your health trends in a colour-coded dashboard from science-based rules. This customisable dashboard makes it possible for you to have an easy and quick overview of what you need to track and manage to enjoy a healthy lifestyle. Connected Health: Embrace self-care with the Internet of Things (IoT) using popular cloud-based apps such as Fitbit and Garmin, and Bluetooth devices such as Terraillon, Thomson, A&D Medical and many more. Connected Logbook: See all your health data chronologically ordered and colour-coded by science-based rules. Filter your data in a couple of taps to see only what you would like to focus on. Generate a PDF report that can be shared to your healthcare professional. Engage Messages: Engage Messages will give you science-based feedback about your healthy behaviors and trends. The coaches remind you about things you should do or measure, best preventive health practices and analyse the data you log to coach you into making small behavior changes to improve your health. You can also send messages, pictures and web links to your healthcare professional by just finding his/her name in one of the coaches. 1
1.1 Dashboard On the dashboard you can view all your health indicators. On this main screen you can add a record, access the logbook, the engage section and the settings. You can choose and rearrange the health indicator tiles shown on the dashboard. Add record Dashboard filter Return to dashboard Logbook Engage Settings 2
1.2 Profile picture To change your profile picture, tap on the picture icon and select the source of the picture you wish to add. Profile picture 3
2. Adding data manually 2.1 Adding measurements manually You can manually enter all your data by simply tapping on the + in the top right corner and choosing which measurement you wish to add. 4
2.2 Adding waist size To add waist size, tap on Waist and enter the measurement as well as choosing your preferred units. You can specify the date and time recorded, you can also add a personal note on the measurement. When you re done, you can tap on Save. If not, you can tap on Cancel or on the arrow at the top left corner of the screen to go back. 5

2.3 Adding a note To add a personalised note, you can tap on the Note tab at the top of the Log page. You can write a text note, add a photo note or record an audio note. 7

3. Logbook On the logbook, you can view all your data in chronological order. You can choose which type of measurement you would like to view by filtering the logbook. Add record Logbook filter Logbook 9
3.1 Modify a measurement To modify a measurement, simply slide the measurement to the left and tap on the pencil icon. You can change all the data and tap on Save to save the change. If you want to cancel the modification, you can tap on Cancel or on the arrow at the top left corner of the screen to go back. 10
3.2 Delete a measurement To delete a measurement, simply slide the data to the left and tap on the red garbage icon. Confirm if you are sure you want to delete the record or tap on Cancel to dismiss the message. 11
4. Personal Health Report On the main page, at the bottom of the Dashboard, tap on Logbook to go to the Logbook page. To generate your Personal Health Report simply tap on the PDF icon. You can select the period that you want on the report. You can also share the report by tapping on the icon at the top right corner of the screen and then selecting how you want to share it. 12
5. Tracking weight 5.1 Tracking body weight You can track your body weight on this page, you can choose to return to the Dashboard or you can add a weight record. You can view graphs of your body weight, waist, Body Mass Index (BMI), height, body fat, and waist-toheight ratio. Change which measurement is graphed by tapping on the measurement in the list below the graph. The black box indicates which measurement is currently being viewed in the graph. 5.2 Body weight statistics You can view your body weight, your waist or your body fat statistic by sliding the page upwards. You can also tap on the icon beside each measurement to view the medical reference with your range and goals. 14
5.3 Graphs in panoramic view You can view your body weight measurements on the graph in panoramic view by tapping on the graph. To look at the statistics while staying on the graph, tap the Stats button and select Done to close the statistic screen. You can also zoom in/out by pinching the screen. Press Done to go back to the previous page. 15
6. Engage On the main page tap the Engage icon found at the bottom to go to the message page. 16
6.1 Auto-Engage You can tap on the coaches to view the messages. You can also view the conversations with your healthcare professionals. You can choose which coach you would like to have by filtering the coaches on All My Coaches. 17
6.2 Engage Messages (patient and healthcare professional) You can have a message conversation when your health professional initiates one. You can send them a text message, an image or a web link. To open the link, you can simply tap on the link and it will automatically open the page on the app. 18
6.3 Questionnaire notifications Questionnaires are accessed via the Engage section of the app. Simply select Questionnaires in the Engage section and press Start below the desired test. Once the test is completed a Done! message will appear in the conversation. 19
6.4 Lifestyle coach The Lifestyle coach will send you motivational messages and reminders to log entries. You can log measurements directly in the conversation by tapping the + icon inside the green box. 20
7. Settings In the settings page you can access many options like your account credentials, you can modify your profile and manage the connected health devices paired with your phone or tablet. You choose the language and the units you prefer. If you need more information on the app or if you need assistance, you can access the Info/Support page. For privacy and security, you can set a passcode lock and you can share your data by exporting them by email. 8. Connected Health The Connected Health interface enables the addition of accounts and Bluetooth devices to the app by automatically synchronising personal health data measurements. 8.1 Accounts If you have an existing account with any of our device partners, select it in the list and log in the account. When the connection is successful a pop up message will confirm it. To synchronise the data, you will first need to sync the data from the device to the manufacturer s app. Then, the data will automatically sync to the Corin app. For the Apple Health app, you will simply need to select the measurements and the date from which you wish the data to be synchronized with the Corin RPM app. 22

Apple Health app To synchronise with the Apple Health app, select it in the Accounts list and press Connect. Set a date for when to start syncing the data from and press Done. Once the app is successfully connected you will receive a pop up message. If you have an Apple Watch, you will first need to sync the data from the watch to the Health app on your iphone, ipod, or ipad. To check that your watch is connected to the Health app, open the app and click on the Sources icon. Click on your Apple Watch in the Devices list and verify which activities are being synced from the watch to the Health app. 24

8.2 Bluetooth The data can be transferred directly via Bluetooth devices to the Corin app. In the Connected Health page, tap on the Bluetooth tab to view the list of devices and select the one you would like to pair. Tap on Connect and follow the instructions to pair the device with your ios device. When a message pops up to request a pairing, tap on Pair. When the pairing is successful, you can start taking a measurement and it will automatically send it to the app. Don t forget to save the measurement. 26
