Enterprise Identity Management-EIDM EIDM account with an associated user role: Sign in and submit data to the Quality Payment Program. Provides a single identification to use across multiple CMS systems. Receive real-time performance feedback for data submitted via the portal. Integrated with CMS Enterprise Portal. Assures only authorized/registered users can access protected information and systems. Access Performance Feedback Reports. NOTE: EIDM has retained the PQRS naming convention for roles associated with QPP submission 3

2 Steps to Access QPP Portal 1. Create an EIDM Account EIDM Account would exist if you ve accessed: Quality Reporting Use Reports (QRUR) PQRS Feedback Reports in past Submitted PQRS via Qnet Portal in past 2. Register for the Physician Quality and Value (PV) Application Roles: Approvers and Submitters Remote Identity Proofing (RIDP) Multi-Factor Authentication (MFA) *Call QPP Helpdesk at 866-288-8292 and provide group s TIN/name of practice to inquire IF an EIDM account exists. 4

New User Registration: Before You Start Review Provider(s) in Provider Enrollment Chain and Ownership System (PECOS). PECOS updates needed? o Call 866-484-8049 or https://pecos.cms.hhs.gov/pecos/login.do#headinglv1 5

New User Registration: Before You Start Legal Business Name (LBN) Medicare Billing TIN(s) Clinician s Full Name(s) Rendering Physician s NPI and PTAN Basic demographics: address, city, state, zip and phone Security Official role requires NPIs and PTANs for 2 different clinicians who bill under TIN *Access the EIDM User Guide for further details 6

What is a PTAN? 7

Provider Transaction Access Number-PTAN Medicare-only number issued by Medicare Administrative Contractors (MACs) upon enrollment in Medicare. o MACs issue an approval/notification letter, including PTAN information, when an enrollment is approved. While only the National Provider Identifier (NPI) is submitted on claims, the PTAN is a critical number directly linked to a provider s or supplier's NPI. A PTAN's use should generally be limited to a provider's communication with their MAC. 8

NPI/PTAN Relationships If a group NPI enrolled in Medicare, the group would be issued a PTAN. If an individual NPI enrolled in Medicare, the individual would be issued a PTAN. If an individual NPI enrolled in Medicare under multiple TINs, multiple PTANs would be issued by the MAC under those TINs. When creating a new, individual provider association in the Physician Quality and Value Program in the EIDM portal, the PTAN issued to the individual NPI should be entered. 9

Multiple PTANs-Locality More than one PTAN may be issued depending upon the reasonable charge locality, under the Medicare Physician Fee Schedule (PFS) locality configuration. o Example: If a NPI practices under a single TIN but has a practice in John Wayne County (PFS Locality 01) and a practice in Green County (PFS Locality 99), multiple PTANs may be issued. 10

How to Find a PTAN Option 1: Find PTAN on the notification letter sent by the MAC when the Medicare enrollment was approved Option 2: Find PTAN in the Medicare Provider Enrollment, Chain and Ownership System (PECOS) 11

PECOS Step 1 Login into PECOS: https://pecos.cms.hhs.gov/pecos/login.do#headinglv1 NOTE: The password security has been updated at the I&A level. If you cannot login to PECOS, try logging into the I&A system to update the password. Once updated, the user should by able to access PECOS. 12

Creating an EIDM Account: Choose Your Application Select Physician Value- PV: Physician Quality and Value Programs from the drop down list Click the box I Agree to The Terms and Conditions and click NEXT 21

EIDM Account: Password Challenge Questions Set up your Username and Password Create Challenge Questions Review Registration Summary screen, and select Submit User to continue with registration process Note: Highly recommended to screenshot or write down in a secure location your username/password and challenge questions/responses 23

EIDM Account: Confirmation - Success! Your EIDM account setup is now complete! You ll receive an email confirmation acknowledging your successful account creation. Next, log into the CMS Enterprise Portal by clicking here link. 24

Physician Quality and Value (PV) Application Roles 25

PV Application: Selecting the Appropriate Role PV Application requires selection of appropriate role: Provider Approver: Master account (must be set up first) o Individual Practitioner o Security Official PQRS Provider: Requires approval of Master o Individual Practitioner Representative o PQRS Submitter o Web-Interface Submitter Establishing a role requires identity verification (Experian credit report) 26

Log In and Add Apps Log into www.qpp.cms.gov with your EIDM credentials Click the Request/Add Apps button 31

Request Access for PV Enter P in Access Catalog search box to filter applications Click Request Access button for the Physician Quality and Value Program (PV) 32

Select Group and Role For Initial Registration, next to Select a Group, choose Provider Approver. Next to Select a Role Description, choose Individual Practitioner for solo practitioner OR Security Official for a Group (2+ EP s). Click NEXT. 33

Identity Verification Review the Identity Verification items listed below to keep in mind during the next steps. Click NEXT. 34

Identity Verification Read the Identity Verification terms and conditions Click I agree to the terms and conditions, then NEXT NOTE: You will not be able to complete the Identity Verification section if you have had a credit freeze in the past. Please see slide 45 for further details 35

Identity Verification Enter YOUR information accordingly. Click NEXT. 36

Manual Approver Process Identity verification cannot be completed for users with a credit freeze: Common issue due to recent Experian breach An EIDM Manual Identity form would need to be completed, with the following documents: Driver s license/state ID Passport Two statements from financial institutions addressed to the user at the address provided during registration For further questions, contact the QPP Helpdesk at 866-288-8292 38

Identity Verification Remote identity proofing is complete! Click NEXT to proceed to Multi-Factor Authentication registration process. Review Multi-Factor Authentication Information. Click NEXT. 39

Multi-Factor Registration (MFA) Read the Register Your Phone, Computer, or E- mail notification and then select an option from the MFA Device drop down menu. Click NEXT. 40

Multi-Factor Authentication Complete! Registration for the Multi-Factor Authentication is now complete. Select Next to proceed to request the role. 41

Select Role: Individual Practitioner For Initial registration of solo practice, select Group > Provider Approver and Select a Role > Individual Practitioner. Next to Create/Associate Individual, choose Create a New Individual Eligible Professional. 45

Associate to an Existing Organization Enter TIN to search for Practice/Organization you are associating with, may enter Reason for Request, and click NEXT. NOTE: If your practice/organization does not appear, contact the QPP Helpdesk by phone, 866-288-8292. 53

Completing Role Request Review request to confirm all the data was entered accurately If accurate, select Submit If a change needs to be made, select Edit and make appropriate changes 54

Important Tips Please be sure to create the new Organization OR Individual Practitioner using the provider(s) Individual NPI and PTAN combination(s). If needing to verify this information, contact your Medicare Part B Contractor (MAC). Approver(s) will receive an email notifying them of request approval. Approver will need to log into the CMS Enterprise Portal to approve or reject request. Notification of approval, denial, or other requests will be sent to the role requestor s email address on file for request. 59

EIDM FAQs Q. What if my EHR/Health IT Vendor is submitting my MIPS data for me, do I still need an EIDM Account? A. Yes. It is recommended that providers obtain an EIDM Account, to view their data submitted on their behalf by a Qualified Clinical Data Registry (QCDR), CMS Qualified Registry, or EHR/Health IT Vendor. 66

EIDM FAQs Q. Can my vendor or third party intermediary access my final performance feedback? A. It depends on their EIDM access. Yes, if your practice s Security Official approved an EIDM role request for your organization from a health IT vendor or thirdparty intermediary to submit data on behalf of your practice. No, if your vendor (qualified registry or Qualified Clinical Data Registry) had an EIDM role for their own organization to submit data on behalf of your practice. 67

EIDM FAQs Q. What if I have more than one TIN in my organization, do I have to create separate EIDM Accounts? A. No, you do not have to create separate EIDM Accounts if you have more than 1 TIN in your organization. Once you ve created your primary EIDM Account, you can add Associate to an Existing Organization. A Role Request will be sent to the Security Official for approval. 69

EIDM FAQs Q. What if CMS is unable to verify my identity? A. If your identity cannot be verified, you ll need to contact your QPP Helpdesk and complete a manual identity verification process. You would complete a special form provided to you by CMS, and submit one of the following as proof of identity: drivers license, Federal or State photo ID, or U.S. passport, and two financial institution official bills addressed to you at the address entered in the registration process. 70

EIDM FAQs Q. What if my providers NPI/PTAN cannot be identified in the EIDM application process? A. You are recommended to verify your providers enrollment information with PECOS for any errors. EIDM FAQs Q. What if I am in an ACO, do I still need an EIDM Account? A. Yes, groups participating in a Medicare Shared Savings Program (MSSP) ACO must obtain an EIDM Account and Role under their participant TIN to submit and meet ACI requirements under MIPS for purposes of MIPS APM scoring standard and MSSP ACO-11 quality measure. 72

EIDM FAQs Q. Do I need to update my password? A. Yes, EIDM requires users update their password once every 60 days. 73
