What is this product about? The above is a Prepaid Card, a general purpose payment Visa Prepaid Card. This prepaid card is an E-Money instrument, which contains a monetary value which has been pre-loaded by the cardholder. The value will be deducted from the amount stored in the card whenever purchases are made. You are required to pre-load the card before making purchases. 2. What do I get from this product? 1x Treatspoints for every RM2 spend on retail transactions. 5x Treatspoints when Manchester United wins a Premier League Match. (5x TreatsPoints is applicable to ALL purchases on the match day and there is no additional bonus Treatspoints for petrol and groceries purchases) - Fixtures are based on UK match day 10x Treatspoints at United Direct Online Megastore. - Visit www.manutd.com and shop for your favourite Manchester United player's jersey or other items from United Online Megastore. 10% Discount Not applicable for any sales item. Not eligible to use additional 10% for Megastore Members when using the prepaid card. 3. What are my obligations? Applicant must be minimum 18 years of age Applicant must provide a valid mailing address in Malaysia and photocopy of IC/passport for those require higher purse limit(up to RM10,000) The cardholder should notify Maybank within 20 days from the closing date of billing period as stated on the statement of the account should there be disputes or discrepancies Cardholder is to take all reasonable precaution to prevent loss or theft of the Prepaid Card. The Cardholder shall notify Maybank by telephone, fax or email immediately upon discovery of lost or theft and confirm the same in writing to Maybank. 4. What are the fees and charges I have to pay? Annual fee Domestic ATM withdrawal Fee ATM withdrawal fee at CIRRUS/PLUS Card replacement fee Sales draft retrieval fee Statement request fee Starter pack Goods and Services Tax (GST) RM6/- Zero fee through Maybank s ATMs RM10/ - per withdrawal Nil Cardholder need to purchase a new starter pack for every card replacement Original Sales draft at RM15/- per copy/photocopy of sales draft at RM5/- per copy. RM2/- per month statement RM5/- for each statement more than 6 months *E-Statement is free of charge RM20/- The Malaysian Goods & Services Tax (GST) will be imposed on all fees/charges charged by the Bank to our cardmembers where applicable at the current prevailing rate. 1 P a g e

What if I fail to fulfill my obligations? Liability for unauthorized transactions: The cardholder shall be liable for all charges and advances whatsoever arising from all transactions, whether authorized or unauthorized, affected with the prepaid card. If you fail to abide by the terms and conditions of the prepaid card, we have the right to terminate your card. 7. What are the major risks? You should notify us immediately after having found that your credit card is lost or stolen. 8. What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a timely manner. Apakah yang saya dapat daripada produk ini? 1x TreatsPoints untuk setiap perbelanjaan RM2 ke atas urus niaga runcit 5x TreatsPoints apabila Manchester United menang perlawanan liga Premier Inggeris ( 5x TreatsPoints boleh digunakan untuk semua pembelian pada hari perlawanan dan tiada TreatsPoint bonus tambahan untuk petrol dan pembelian barangan runcit). Berdasarkan kepada jadual perlawanan di UK 10x Treatspoints di United Direct Online Megastore Lawati www.manutd.com dan beli jersi Manchester United pemain kegemaran atau barang-barang lain dari United Online Megastore Diskaun 10% tidak boleh digunakan untuk mana-mana barangan jualan promosi. Tidak layak untuk mendapat tambahan 10% diskaun untuk Ahli Megastore apabila menggunakan kad prabayar. 3. Apakah kewajipan yang saya perlu laksanakan sekiranya saya memohon produk ini? Pemohon mestilah berumur sekurang-kurangnya 18 tahun. Pemohon perlu memberikan alamat surat-menyurat di Malaysia yang sah dan salinan Kad Pengenalan/Pasport bagi yang memerlukan had yang lebih tinggi (sehingga RM10,000). Sekiranya terdapat sebarang pertikaian atau percanggahan, Ahli Kad perlu memberitahu Maybank dalam masa 20 hari dari tarikh tutup bil seperti yang dinyatakan dalam penyata akaun. Ahli Kad hendaklah mengambil segala langkah berjaga-jaga yang munasabah untuk mencegah kehilangan atau kecurian Kad. Ahli Kad hendaklah memaklumkan kepada pihak Bank menerusi telefon, faks atau e-mel dengan serta merta selepas kehilangan atau kecurian Kad dan mengesahkan kehilangan atau kecurian tersebut secara bertulis kepada pihak Bank. 4. Apakah caj dan yuran yang perlu dibayar? Liabiliti ke atas sebarang transaksi: Ahli Kad hendaklah menanggung liabiliti untuk segala caj dan pendahuluan walau apa sekalipun yang timbul daripada segala transaksi sama ada dibenarkan atau tidak dibenarkan, yang dilakukan menggunakan Kad anda.
