Introduction About the National Food Safety Training Program (NFSTP) More than ever before, food safety is a top priority for foodservice operators and the customers they serve. In response to a growing demand for food safety training that meets the needs of operators from coast-to-coast, the Restaurants Canada developed the National Food Safety Training Program (NFSTP) in conjunction with Freeborn & Associates who offer a national training curriculum and certification for foodservice operators and employees. Based on Canada s Food Retail & Food Service Regulation, the NFSTP is broken down into eleven teaching modules covering key food safety topics: food safety hazards, facilities and equipment design, control of hazards following the flow of food, sanitation and pest control, and employee and visitor issues. The NFSTP is delivered to foodservice students and workers through a national network of certified trainers and online training. It is designed to meet the highest training standards in jurisdictions all across Canada. About NFSTP Examinations The databank of NFSTP examination questions is secure and copyrighted. Developed in conjunction with other NFSTP curriculum components (Trainer s Notes, Slides and Student Workbooks), it is based on the contents of NFSTP Student Workbook. The questions will vary from examination to examination and have been selected to provide a fair assessment of a student s food safety knowledge. From time to time, NFSTP may include pilot questions to check their relevance, format and accuracy. These pilot questions will be used for investigative purposes only, and examinee scores will be exclusive of any pilot questions. About this manual NFSTP Administration has prepared this manual to assist you in the proper administration of NFSTP training and certification process. Please review this manual carefully and complete and return all forms as required. Page 3 of 19

Your role as a certified NFSTP trainer/proctor An important part of your role as a certified NFSTP trainer/proctor relates to the testing of NFSTP students, and includes activities before, during, and after testing. NFSTP requires that you agree to conduct the testing activities in accordance with the procedures outlined in this document, so that tests may be administered in a uniform manner across Canada. Your agreement In order for your examinees to be certified, you must have a contractual agreement with NFSTP s administrators in which you agree to adhere to the specific training and testing requirements outlined in this guide. Your specific responsibilities in advance of, during, and after delivering the NFSTP examination are described throughout this document. NFSTP administration requires that the following steps be completed before you deliver your first training or testing session: 1. Complete and sign the NFSTP Examination Proctor Application and Agreement 2. Trainers must complete and sign the NFSTP Trainer Agreement 3. These agreements can be completed online and sent to NFSTP Administration. You may also complete and send them by mail, fax or email. Once your completed agreements have been received, you will receive an email or letter providing you with permission to begin NFSTP training and testing. As soon as NFSTP Administration has processed your agreements, you can begin to order NFSTP examination materials. Trainers / proctors eligible to administer NFSTP exams for a period of five years, after which time recertification is required. NFSTP reserves the right to revoke trainer and/or proctor privileges should you violate any of the procedures outlined in the Exam Administration Manual or the Proctor and Trainer Agreements. NFSTP trainer/proctor requests NFSTP Administration regularly receives requests from individuals and organizations seeking food safety training and/or proctoring. To promote NFSTP and its certified trainers, we have created a training and proctoring request forms. If you are interested in receiving requests for training and/or proctoring, you must contact NFSTP Administration directly (see page 4 for contact information). NFSTP logo usage NFSTP trainers are permitted to use the NFSTP logo for marketing purposes. All usage is subject to approval by NFSTP Administration. Trainers must provide NFSTP Administration with a printed or electronic copy of the logo in use before final marketing materials are printed or posted online. The importance of examination security NFSTP certification is determined by successful completion of a multiple choice examination. The exam has been developed to test a student s food safety knowledge, based on the NFSTP Student Workbook. It is essential that every possible step be taken to safeguard the examination questions failure to do so compromise s the integrity of the entire program and the students it serves. To prevent situations where an examinee could potentially rewrite the same examination he or she wrote previously, we have developed a database of more than 400 food safety questions. Examinations will be available in multiple versions whenever trainers make examination requests. As an exam proctor, you are responsible for ensuring the security of all examination materials before, during and after the testing process, from the time they are received until the time they are returned to the NFSTP Administration office. Paper Examinations Your responsibilities in advance of testing A. Ordering examinations Exam and book orders must be ordered from NFSTP Administration no fewer than fifteen (15) business days in advance of the test or date required by. To order examinations: Online at https://foodsafetymarket.ca/en/ Note: If you are ordering for more than one class, you must complete an NFSTP Request for Exams form for each class. The examinations will be shipped to you via signature-required Expedited Mail Page 5 of 19

Please note that photocopying exams is not permitted and is considered a breach of exam protocol. Trainers, who photocopy, scan or in any other way duplicate an NFSTP exam will have their certification immediately revoked. We will not fax copies of NFSTP exams under any circumstances. Ordering additional exams If you discover that you require additional examinations and you have already received your examinations, you must complete another order form We assume no liability for email that are not received or for exam requests that are not followed up by a confirmation call from the trainer. Maximum Number of Participants As a certified NFSTP trainer your class size is limited to a maximum of 25 participants per class. In the event that the class may exceed 25, you will require an additional NFSTP approved Proctor during the exam portion. Our office must be notified of the increased class size prior to the exam. At that time we will provide information regarding the additional Proctor procedure. Note: You cannot act as a Proctor for a family member or personal friend. B. Receive and safeguard examinations Only you as a certified NFSTP trainer/proctor are permitted to open the examination packages. It is your responsibility to inform your reception or receiving department of this requirement if you work in a workplace in which packages are centrally received. Once you have received the examination package, please check its contents carefully. The examination package will include: The requested number of exams (with security seals). A copy of the NFSTP Exam Tracking Form allowing us to track the exams. Upon completion of your class, this form must be completed and returned. An NFSTP Examination Irregularity Report this form must be filled out and returned with the completed exams. An NFSTP Attendance List this form must be filled out and returned with the completed exams. Once the examination materials have been reviewed and determined to be complete, it is your responsibility to keep the materials in a safe and secure place until the exam is delivered. Please note that if you need to change the examination date or cancel the examination for any reason, you must notify our office immediately. It is acceptable to retain and use any examinations you have ordered and received, provided the new date for the examination is within one month of the original date. If the new testing date is later than one month after the original date, or if the examination session is cancelled, all examination materials must be returned to NFSTP Administration no later than one week after the originally scheduled examination date. C. Secure an appropriate testing room An appropriate testing room must: Be selected, booked and confirmed well in advance of the training session Be of a size appropriate for the number of registered examinees (this includes the provision of sufficient space between each examinee to prevent the viewing of another examinee s responses) Be well-ventilated, well-lit and free from distractions Include an adequate supply of chairs and writing surfaces Accommodate the physical needs of disabled examinees, if applicable Be well-signed to assist examinees in locating the testing room, if the examination will take place in a location not well-known to the examinees Additional location requirements such as occupancy limits may be stipulated by federal, provincial or local laws. It is your responsibility to ensure that these regulations are met when and where they apply. Your responsibilities on testing day A. Identify examinees Examinees must present an acceptable form of identification before they can write the NFSTP examination. Only photo ID with a signature is acceptable this could include a provincial driver s license, a student card, an employee identification card or a valid passport and examinees who do not provide appropriate identification cannot write the examination. It is your responsibility to ensure that examinees are informed of this requirement with sufficient notice. Ensure that all examinees have an appropriate writing utensil (sharpened #2 HB pencil) with which to complete the examination Scantron answer form. At the appropriate time corresponding to the time examinees were told was the testing time the examination room should be closed to prevent interruptions during the testing. Examinees that arrive after the room has been closed are not to be admitted and will be required to retest at another time. B. Inform examinees of exam protocol Before distributing the examination materials, inform examinees of the rules surrounding the examination process, including: The NFSTP examination is a closed-book exam. Examinees cannot refer to any books, their training materials or any other notes. Smoking, eating, drinking and talking to other examinees is strictly prohibited. Page 7 of 19

Cell phones and pagers must be turned off and stored in an out-of-sight place, such as in a backpack or under the desk. If this is not possible, they should be handed to the exam proctor for safekeeping during the examination process. The examination consists of 60 questions. A minimum passing score of 70% is required for NFSTP certification (equivalent to 42 correct questions out of 60). There is no time limit for completing the exam. C. Distribute examinations Personally distribute the examination booklets and Scantron answer forms to each examinee. Read aloud the instructions for completing the examination (see below) and remind examinees that a copy of these instructions is included on the cover page of each examination booklet for their reference. You may wish to show examinees a copy of the Scantron answer form as you describe how to complete the different sections. Ask if there are any questions, answer questions if required, and if there are no more questions, instruct students to begin writing the exam. Page 8 of 19

Instructions for completing the NFSTP examination 1. Please remove all items from your desk. 2. On the Scantron answer form provided this is the pink sheet clearly and carefully write in your name and today s date and time. Please write NFSTP examination where the form says subject. 3. Complete all Student Information including name and phone number by printing in the boxes and shading in the bubbles of the Scantron answer form. 4. The Test Form will also be printed on the cover of the exam booklet and is a letter between A and Z. 5. To complete the exam, you must completely fill in the bubble on the Answer Sheet that corresponds to the correct answer. For example, if the answer to question #1 is C, fill in the bubble marked C. These exams are electronically scored and so it is essential that you: completely fill in the answer bubble using only an HB #2 pencil (do not use ink); erase clearly any answer you wish to change; and do not make any stray marks elsewhere on the Scantron answer form. 6. You are allowed to write on your examination booklet. You may prefer to answer all of the questions and then fill in the Scantron answer form, or you may wish to fill in the Scantron answer form one question at a time. 7. To begin writing the exam, open your sealed examination booklet/envelope and commence. 8. Once you are finished writing the examination, please take a few minutes to doublecheck that you have completed the Scantron answer form correctly, so that it accurately reflects your answers. 9. When you are satisfied with your answers, please bring both the examination booklet and the Scantron answer form to me. I will check the Scantron answer form to make sure that you have completed the answer section and your own Student Information Sections correctly. 10. Once I have collected your Scantron answer form and examination booklet, you may quietly return to your seat to collect your belongings. Please take care not to disturb other students who may still be writing the exam. 11. If you have any questions at any point during the examination, please raise your hand and I will come to your seat to assist you. 12. If there are no questions, you may now open the sealed examination booklet/envelope and begin writing the exam. D. Oversee the examination As exam proctor, you must remain in the testing room until all exams have been completed and returned to you. Note: If you have more than 25 participants in a class, you will require one more Proctor during the exam portion of the class. Call our office at 1-888-829-3177 for procedures of additional Proctors. During the examination, circulate around the testing room to discourage cheating and to answer any examinee questions. Please take care not to disturb examinees as they write. Page 9 of 19

E. Collect completed examinations It can be expected that examinees will complete the exam at different times. Those who have completed the exam must hand both their Examination Booklets and Answer Forms directly to you. (The Scantron Answer Forms are the pink bubble forms) As examination materials are handed in, please review the Answer Forms to ensure that the examinee has correctly completed the pink form. Check that the participant s have completed all sections of the Answer Form including date of birth, address, name etc. Tampering with an examinee s Answer Form by changing, correcting or erasing any answers is a serious security violation and is strictly prohibited. Once you have collected an examinee s examination materials and confirmed that the Answer Form has been completed correctly, he or she must quietly leave the room. Your responsibility for ensuring the security of examination materials remains in effect following the test. It is strongly recommended that all completed examinations remain in your sight at all times during and immediately following testing. Your responsibilities following testing All completed examinations must be returned to NFSTP Administration c/o Freeborn & Associates immediately to protect exam security and prevent any delays in grading the exams. Specifically, you are required to: Count all examinations booklets (including those not used) to ensure that you have the same number of exams as received from NFSTP Administration. Even one missing exam will be considered a serious security breach and NFSTP Administration will not score any exams from any testing session in which exams are lost. Place the following items in an envelope: all examination booklets, including those not used all USED Scantron answer forms (pink bubble sheets), should be put into the brown envelope that we have provided along with the forms listed below. DO NOT return unused Exam Answer (Scantron) forms as you have paid for these and should keep them for your next session. the completed NFSTP Examination Irregularity Report (must be included even if no irregularities were noted) the completed Exam Tracking form and the Attendance List Seal, sign and date the envelope Securely package the envelope and ship it to NFSTP Administration c/o Freeborn & Associates within 48 hours of the examination. The package must be sent via a shipping service that is traceable and NFSTP Administration must be notified immediately if you anticipate a shipping delay for any reason. Page 10 of 19

Ultimately, it is your responsibility to ensure that completed examination materials are delivered to Freeborn & Associates in a secure and timely manner. If you are unable to send the examination materials to Freeborn & Associates immediately, you must store them in a secure location until you can prepare them for shipping. All examination materials must be sent within 48 hours of the examination and received by Freeborn & Associates within five business days of the examination. Failure to comply with any of these requirements will be considered a breach of exam protocol that may result in having trainer certification revoked. Administration Irregularities An NFSTP Examination Irregularity Report must be completed for every NFSTP exam session, including those in which no irregularities are noted. If you have no irregularities to report, check the no irregularities box at the top of the page and sign and date the form where required. Although these procedures and your own experience will minimize the likelihood of any examination irregularities, irregularities should be anticipated. Irregularities may include examinee illness, examinee misbehaviour or problems relating to the testing location. Below are recommended actions to take should you encounter any such irregularities during the examination process: Page 11 of 19

Irregularity Disturbance during the exam Giving or receiving help during the exam Examinee illness during the exam Decision by examinee not to complete testing Irregularity relating to the testing location (power failure, fire alarm or other emergency) Recommended Action 1. Warn the offending examinee that continued disturbance will result in dismissal from the exam. 2. If the examinee persists in creating a disturbance for other examinees, collect all of the offending examinee s test materials and dismiss the examinee as required. 1. Observe the suspected situation carefully. 2. Warn the offending examinee(s) that the offending behaviour must cease immediately and that continued misbehaviour will result in dismissal. 3. Move or separate the examinee(s). 4. Dismiss the examinee(s) if the behaviour continues. 1. Collect and hold the examinee s exam materials; permit the examinee to leave the room if required. 2. Encourage the examinee to complete the exam if they are able to do so upon their return. 3. If they are unable to do so, write Unable to complete due to illness on the Scantron answer form. 1. Collect all examination materials. 2. Write Decided not to complete on the Scantron answer form. 1. Collect all examination materials and note the time at which the examination was terminated. 2. If possible, immediately relocate examinees and continue testing. or 1. Cancel exam if required. 2. Notify NFSTP Administration as soon as possible if the examination was cancelled. Missing examination materials Trainers should carefully inspect their examination package in advance of testing day to ensure that the appropriate number of examination booklets and answer forms has been provided. If you discover that an examination booklet is missing on the testing day and suspect that it may have been taken by an examinee: 1. Make an effort to ensure that it is returned before any examinees leave the testing room. 2. If this is not successful, note the incident on the NFSTP Examination Irregularity Report and return the remaining examination materials. No examinations will be scored until the booklet has been returned or the irregularity reviewed by NFSTP Administration. Page 12 of 19

Grading and certification Once received, NFSTP Administration will grade the examination Scantron answer forms within 10 business days. The results of the examinations, along with personalized wallet cards for those who successfully achieved the mark required for certification, will be shipped via Expedited Mail or regular mail to the address provided on your NFSTP Exam Order Form. It is your responsibility to inform your examinees of the scores received, and to distribute the certification cards to those individuals who successfully passed the examination. We will also send to you the class grade sheet as a Microsoft Excel spreadsheet via email. Reporting grading errors If you believe that a grading error has taken place, an NFSTP Appeal Request Form must be submitted within 30 days of receiving the examination results. NFSTP Administration will review the appeal and render a final decision within 30 days of receipt of the NFSTP Appeal Request Form. This form is available online in the Trainer s Forum. Security violations and appeals Any incident that may affect the security of NFSTP examination materials or the examination process will be investigated promptly by NFSTP Administration. NFSTP Administration will notify the trainer involved within three business days if a security violation is suspected or confirmed. Trainers/Proctors who wish to appeal an action resulting from any investigation may appeal the decision by submitting an NFSTP Appeal Request Form within 30 days. Similarly, examinees who wish to appeal for any of the following reasons must submit an NFSTP Appeal Request Form within 30 days of the original notice of denial: denied eligibility to write the examination denied an accommodation for special needs failure to complete the examination due to suspected cheating failure to meet the minimum requirements for passing the examination In all cases, NFSTP Administration will review the appeal and render a final decision within 30 days of receipt of the NFSTP Appeal Request Form. Online Examinations NFSTP offers an online option for delivering the final examination. ALL NFSTP examinations MUST be supervised with an approved Proctor. The online exam option is no exception. Only approved NFSTP Trainers/Proctors will be able to obtain Online Examination materials for the use of Proctoring the exam. The same principles of paper exams apply to the online exams. The major difference is that the Online Exam does not require shipping and handling of paper exam booklets thus reducing cost, time and the chance that a booklet may go missing. Several security measures are incorporated into the NFSTP Online Examination procedure: Students must prove they have completed the NFSTP Training Online exam questions and answers are automatically randomized so no two students would write the same exam. Emails are automatically sent to NFSTP Administration each time a student accesses the Online Exam Proctors must use a unique enrolment key for each student to access the part of the NFSTP web site that hosts Online Exam Proctors must use a secret password to launch the NFSTP Online Exam for the student Only one attempt is permitted per student after which they are locked out. Students are automatically un-enrolled within 24 hours of completing the Online Exam. Your responsibilities in advance of testing A. Ordering Online Exam Enrolment Keys Prior to the exam you must order the Online NFSTP Exam Enrollment Keys using the https://foodsafetymarket.ca/en/. Ensure you allow a minimum of 2 business days notice for your request. Maximum Number of Participants As a certified NFSTP trainer/proctor your class size is limited to a maximum of 25 participants per class. In the event that the class may exceed 25, you will require an additional NFSTP approved Proctor during the exam portion. Our office must be notified of the increased class size prior to the exam. At that time we will provide information regarding the additional Proctor procedure. Computer Testing of Technical Requirements Prior to the Online Exam you must test your computer to ensure it meets the technical requirements for the exam. A sample test is provided in the Proctor s Forum for this purpose. B. Receive and safeguard Online Exam Enrolment keys and Exam Password Prior to the Online Exam you will be emailed enrolment keys and the exam password. Unique Enrolment keys are used by your student to access the secure area on the NFSTP web site where the Online Exam is hosted. DO NOT provide the Online Exam Enrolment key to your student(s) prior to the exam date. Page 14 of 19

The Exam Password is a second layer of security. Only approved NFSTP Trainers/Proctors are permitted to know the Online Exam Password. DO NOT SHOW THE EXAM PASSWORD TO YOUR STUDENT. Only you as the Proctor can enter the Password when prompted. D. Secure an appropriate testing room An appropriate testing room must: Be selected, booked and confirmed well in advance of the training session Be of a size appropriate for the number of registered examinees (this includes the provision of sufficient space between each examinee to prevent the viewing of another examinee s responses) Be well-ventilated, well-lit and free from distractions Include an adequate supply of chairs and computers Accommodate the physical needs of disabled examinees, if applicable Be well-signed to assist examinees in locating the testing room, if the examination will take place in a location not well-known to the examinees Additional location requirements such as occupancy limits may be stipulated by federal, provincial or local laws. It is your responsibility to ensure that these regulations are met when and where they apply. Your responsibilities on the day of testing A. Identify examinees Examinees must present an acceptable form of identification before they can write the NFSTP examination. Only photo ID with a signature is acceptable this could include a provincial driver s license, a student card, an employee identification card or a valid passport and examinees who do not provide appropriate identification cannot write the examination. It is your responsibility to ensure that examinees are informed of this requirement with sufficient notice. B. Verify examinee completion of NFSTP Training Examinees must provide a Food Safety Certificate of Achievement proving they have successfully completed the NFSTP Online Training. If they have taken the Instructor Led Training in a classroom they must have proof from the NFSTP Trainer that the training was completed. Finally, if they are re-certifying they must provide their expired/expiring NFSTP Wallet Card. Page 15 of 19

C. Inform examinees of exam protocol Before accessing the Online Exam, inform examinees of the rules surrounding the examination process, including: The NFSTP examination is a closed-book exam. Examinees cannot refer to any books, their training materials or any other notes. Smoking, eating, drinking and talking to other examinees is strictly prohibited. Cell phones and pagers must be turned off and stored in an out-of-sight place, such as in a backpack or under the desk. If this is not possible, they should be handed to the exam proctor for safekeeping during the examination process. The examination consists of 60 questions. A minimum passing score of 70% is required for NFSTP certification (equivalent to 42 correct questions out of 60). There is a 90 minute time limit for completing the exam. D. Complete the NFSTP Online Exam When the examinee clicks on the link for the NFSTP Online Exam a window will pop up requiring a Proctor Username and password. As Proctor you must ask the student to look away while you enter the username and password. Note: Passwords change frequently. It is the Proctor's responsibility to ensure they have this password well before the exam date. As exam proctor, you must remain in the testing room until all exams have been completed. During the examination, observe examinees to discourage cheating and to answer any examinee questions. Please take care not to disturb examinees as they write. E. Complete the Certificate of Completion If the examinee is successful the system will allow them to print a certificate of exam completion showing their name, date of the exam, grade and certification identification code. Ensure they understand this is not an official document and that they will receive their NFSTP Wallet Card within 15 business days. Page 16 of 19

F. Complete the Post Exam Report This is the same as the Irregularity Report mentioned previously. Just like the Irregularity Report it must be completed even if there were no irregularities. Once the Post Exam Report is complete you can log out of the NFSTP Online Exam course. Web Proctored Exams NFSTP offers a Web Proctored online option for delivering the final examination. ALL NFSTP examinations MUST be supervised with an approved Proctor. The Web Proctored online exam option is no exception. NFSTP Administration has an agreement with a Web Based proctoring company to provide access to Online Examination materials for the purpose of Proctoring examinees that have completed the NFSTP Training. The process is very similar to that described for NFSTP Online Exams except that the Proctor observes the examinee via a web camera. This is live real time proctoring the same as what happens in a classroom. Web Proctors can see and hear what is happening and communicate with examinees during the exam. Web Proctors can also take control of the examinees computer to assist with technical support; record the computer s screen; record pictures of the examinee and terminate the exam in the event of inappropriate behavior by an examinee. NFSTP Administration will confirm that the examinee has completed the NFSTP Training prior to providing information about the Web Proctored service. Once the examinee s eligibility has been confirmed NFSTP Administration will provide instructions for scheduling the Web Proctored exam to the examinee. Web Proctors require a minimum of 3 days notice to schedule the examinee s request. Page 17 of 19

Maximum Number of Participants A NFSTP Web Proctor cannot proctor more than 8 people at one time. Computer Testing of Technical Requirements Prior to the Online Exam the examinee will be provided with technical support to ensure their computer meets the technical requirements for the Web Proctored exam. The area and room around the computer are scanned using a web cam prior to your exam, so all non-authorized materials must be put away and the area must be clutter-free. No other person can enter the testing room during the exam. D. Identify examinees Examinees must present an acceptable form of identification before they can write the NFSTP examination.
