CARELINK ENROLLMENT New Patients All Reveal LINQ ICM data downloaded during the Reveal LINQ Mobile Manager insertion workflow is sent to the CareLink network. Patient names will automatically display on the New Patients list of the selected CareLink clinic if the following was true during insertion workflow: There was Internet connectivity and the Reveal LINQ Mobile Manager was authenticated, and A valid CareLink clinic was saved to the Reveal LINQ ICM. Select the box next to the patient s name you wish to enroll NOTE: If the CareLink clinic selected during the insertion workflow is a satellite (child) clinic, the patient will be displayed on the New Patients list of the primary (root) clinic s CareLink clinician website. 5
CARELINK ENROLLMENT New Patients Review/complete all information about the patient, the responsible clinicians, and information regarding the distribution of the Patient Home Monitor. 6
CARELINK ENROLLMENT Satellite Clinics If satellite clinics are part of your CareLink network, an additional Clinic Assignment field will appear in the lower, left side of the Add Patient screen. When a satellite clinic is saved to the Reveal LINQ ICM during insertion, that clinic will be displayed next to Clinic Associated at Implant. The CareLink clinician may: Accept the satellite clinic assignment Assign the patient to a different clinic within the CareLink clinic network NOTE: Satellite clinics do not have New Patient lists; all patients will display on the New Patient List for the primary CareLink Clinic. Clinicians must have access to the primary clinic to access the New Patients list and complete the patient enrollment. If a CareLink Monitor has been ordered, that information displays in the Monitor Status field. If a transmission has been received in CareLink, the monitor serial number will also display. 7
CARELINK NETWORK Transmissions List Reveal LINQ Mobile Manager transmissions are identified as In-Clinic to differentiate them from other transmissions displayed on the CareLink network. 8
CARELINK NETWORK CareAlert Notifications Reveal LINQ Mobile Manager (in-clinic) transmissions that meet CareAlert criteria are displayed as website-only alerts. Email, text, and pager notifications are NOT generated. 9
CARELINK NETWORK Parameter Changes Most device parameter changes made with the Reveal LINQ Mobile Manager (during both the insertion and follow-up workflows) can be viewed in the Quick Look tab on the CareLink network* Additional parameters settings can be found on the Episodes Tab. * Assuming Internet connectivity was available when the Reveal LINQ Mobile Manager was used. 10
