1 How to: Register and Submit a Project Proposal at Important: Please create one project application per submission to mercksupport.com. Up to three project applications are permitted.
2 Access to Charitable Giving at On the Welcome Screen choose the option for Merck Foundation/Office of Corporate Responsibility
3 Access to Sign-on Screen On the Merck Foundation/Office of Corporate Responsibility Screen Click on Apply for Grant
4 Registration/Sign-on On the Welcome Screen First time users choose the REGISTER button Returning users enter username and password, click LOG IN to enter the mercksupport.com Inbox
5 Registration - (If already registered, skip to next slide: Submitting a Request ) Fill out the fields of the Registration Form Create username and password Roll mouse over Help Bubbles for extra guidance Certify and click Upload & Submit button to complete registration Note: All required fields are designated with a red star * Help Bubbles For Non U.S. organizations If No is selected, Organization Tax ID # text box becomes inactive The person named here will be responsible to electronically accept the Letter of Agreement prior to any funds being released The authorized signer address should match the specific signer s name, with the extension indicating the organization (ex. ). If no such extension exists please upload a letter of explanation with the required documentation.
6 Submitting a Request Log into mercksupport.com Click on Submit New Request
7 Choose the MF/OCR as the Business Unit Choose MF/OCR from this dropdown and click Continue Select the Charitable Program Contributions/Donations Request Type from the dropdown and click Save or Save & Continue Choose Charitable Program Contributions/Donations from this dropdown and click Save & Continue
8 Request Form for Charitable Program Contributions Note: Please be certain to choose Charitable Program Contributions/Donations as the Request Type, as there are different input screens and required documentation for each type. Provide answers to all required fields, which are designated with a red star * All information needs to be provided in the English language
9 Select YES to the question Are you responding to a specific Request for Proposal (RFP) or Program? and select MSD Fellowship for Global Health from the dropdown This question provides the opportunity to update the name and of the person authorized by the applicant organization to electronically accept the Letter of Agreement Click SAVE & CONTINUE
10 Complete all fields for the Request form for Charitable Contributions Be certain to include the Country served (Include city/state for U.S.) Note: All monetary amounts must be provided in U.S. dollars Choose July 30, 2018 to October 26, 2018 from these dropdowns
11 Request Form Payee Information Section For U.S. applicants: Select Check to be paid by check Select ACH Transfer to be paid by wire transfer For non U.S. applicants: Select ACH Transfer to be paid by wire transfer You may need to uncheck the Accurate Information checkbox to make the fields editable and re check the checkbox once information is filled out accurately Note: Changes are saved for future use
12 Be sure to include employment status/place of employment or other affiliation for each board member Non U.S. organizations need to upload a document confirming nonprofit status Hit upload after browsing Please ensure that all uploaded documents are in the English language Click Save & Continue
