At that time, there were 5 customers we had not received the return receipt cards on from the post office, and we informed you we would follow up on those notices. We have since that date received the certified return receipt cards on 3 of the customers, those being 1. Yvonne Wilson 2. David & Stephanie Rinehart And 3. Larry Presley. We are attaching copies of those cards, proving receipt. We did however, have 2 notices returned to us by the US Postal Service where they were not picked up. We are attaching copies of the envelopes that show this information from the Post Office. (The originals of these are in the file and are available if you should need them.) In talking with the Public Service Commission, we were instructed to send those notices out to these 2 customers again, this time, by regular mail. We have, therefore, r ed those notices today (regular mail). You should be aware, we have made every effort to make sure all customers are aware of the hearing scheduled on this Case. We have not only advertised the meeting in the local newspaper, but we have also put a copy of the newspaper publication in each customer s billing on That would include the customers who did not sign the receipts when the first notifications were sent by certified mail.

11 APPENDIX A Tariff Form No. 10 Case No W-19A OSTEGO COMMUNITY WATER SYSTEM PUBLIC NOTICE OF INCREASED RATES On October 26, 2006, the Ostego Community Water System filed a Rule 19A application with the Public Service Commission to increase its water rates and charges. The Staff of the Public Service Commission has reviewed the application and recommended an across-the-board increase of % to the Applicant's current rates and charges. The present and Staff-recommended rates are as follows: Monthly Flat Rate Tap Fee CURRENTLY CHARGED $ $ STAFF RECOMNENDED $ $ Additionally, Staff has recommended a 10% delayed payment penalty for late payments and a reconnection fee of $15.00 for reconnection of water service when service has been terminated. Any person'who will be affected by this rate change has a right to protest or to be heard as to why the change should not be allowed, either wholly or in part. Anyone desiring to protest the changed rates and charges must do so, in writing, within ten (10) days after the date of publication of this notice. All protests should be addressed to the Executive Secretary, Public Service Commission of West Virginia, P.O. Box 812, Charleston, West Virginia, To help the Commission determine the justification for additional review and investigation, protests must be specific as to the reasons that the changed rates and charges should not be granted. In addition, anyone desiring a hearing in this matter must demand a hearing in the letter of protest.
