18 Detailed Instructions: 6. Attachments Supporting documents to the declaration can be linked via an URL (follow steps 1 and 5) or attached directly in the worksheet (follow steps 1 through 5): 1. Click on next available cell in the File or URL column, E3 in the example. Type in the URL where supporting document can be found or to attach a document, go to next step. 2. Click on the button Attach File to Declaration 3. In the dialog window, browse and select the file you wish to upload. Click OK in the dialog window to upload the file. 4. File appear under File Name. 5. Type in the file name and the file type of the attachment file

Supplier Approval Authorizer Company details and contact information Supplier Authorizer Statement of Acknowledgement Authorizer Acceptance and Signature (electronic signature or print and sign) Macro to check data completeness & accuracy prior to submitting to requestor Macro to enable you generated XML data output in accordance to IPC-1754 Schema. This button is not active in this version of the SRT. Fields with * are mandatory. Once filled, print the form to sign. Scan the signed form and attach it to the declaration via tab 6.Attachements. Group 2: Substances for which a regulatory requirement requires the disclosure, tracking or specific labelling (in addition to what is required by regulatory chemical hazard communication requirements) if present in supplied mixtures/materials or articles.
