6 SECTION 1 : GETTING STARTED 1.1 PUMP MECHANICS AND THE DELIVERY OF INSULIN Before we begin, let s make sre yo know how inslin is delivered when sing an inslin pmp. The parts that make p the pmp s delivery system are the infsion set, the reservoir, and the pmp Infsion Set The infsion set consists of tbing (1) that carries inslin from the pmp to yo. On one end of the tbing is the reservoir connector (2) that attaches to the reservoir which holds the inslin. On the other end is the insertion site section (3) that attaches to yo. Infsion Set * 1 Tbing 2 Reservoir Connector 3 Insertion Site Section 4 Cannla 5 Adhesive 6 Reservoir Compartment The insertion site section has a small insertion needle that places a tiny flexible tbe called a cannla (4) into yor body **. Once the infsion set is inserted, yo remove the needle, leaving jst the cannla behind. A small piece of adhesive (5) holds the infsion set in place Reservoir The reservoir is similar to a syringe and holds 2- to 3-days spply of inslin. The reservoir fits into the pmp s reservoir compartment (6). Yo will be replacing both the infsion set and the reservoir every 2 to 3 days Pmp Inside the pmp, at the bottom of the reservoir compartment, is a piston. The piston acts like the plnger rod on a syringe, pshing p on the bottom of the reservoir, moving inslin into the tbing, throgh the cannla, and into yor body. The piston is controlled by a mini compter inside the pmp that s able to deliver inslin in very small doses, sometimes as small as nits. It mst be rewond each time a newly filled reservoir is placed into the reservoir compartment. 4 2 Reservoir 6 Pmp *Medtronic Qick-set infsion set shown in illstration. **Some infsion sets do not se a cannla bt have a small needle that remains inserted in the body. 4

20 SECTION 1 : GETTING STARTED 1.9 BASAL PATTERNS Remember, yor body needs inslin so glcose can be moved into yor cells providing energy for yor body. Inslin is needed 24 hors a day, even between meals and dring the night. This is called basal inslin. The pmp spplies basal inslin by delivering small amonts throghot each hor, every hor of the day and night. This allows for inslin to be increased and decreased to adjst for yor body s needs. U/hr 00:00 0:00 24:00 24 Hors Basal inslin amonts mst be programmed into yor pmp. This is done by setting a basal pattern. A basal pattern consists of one or more basal rates being delivered over the corse of 24 hors. BEFORE HER PUMP... Lynn always had to remember to take her injection of longacting inslin at bedtime. Taking it at the same time every night like her doctor asked her to was difficlt. She is at niversity and some nights she wold go to bed early, others she wold be at the library ntil late stdying. Now with her pmp, she doesn t have to worry abot taking an injection. She is getting her basal inslin atomatically 24 hors a day Basal Pattern Setp One Basal Rate Yor healthcare professional will calclate the horly basal rate or rates are best for yo to se when yo start on yor pmp. Yo may simply start with a basal pattern that has only one basal rate. The pmp will deliver that exact basal amont evenly over each hor, 24 hors a day. For example, if yor starting basal rate is 1.0 nit, yor pmp wold deliver one nit of inslin throghot each hor. This means yo wold receive a total of 24 nits of basal inslin every 24 hors. To set yor Basal Patterns, yo will need to go to the Inslin Settings men option and then to Basal Pattern Setp. There are two ways to access Inslin Settings: 1) From the Home screen, select Basal and press to Inslin Settings OR 2) Press and press to Inslin Settings 18

21 SECTION 1 : GETTING STARTED Let s Practice: Setting a Basal Pattern with one Basal Rate Let s set a Basal pattern with a basal rate of 0.75 U/hr from 00:00-24:00 1) From the Home screen, select Basal. 2) Press to Inslin Settings and press. 3) Press to Basal Pattern Setp and press. 4) Select Basal 1. 5) Crrently, the pmp is delivering U/hr. Since yo are setting only one basal rate for all 24 hors, the End time does not need to be changed. 8) Press. 9) Press to and press. 10) Select Done. 11) Verify that Basal 1 is entered correctly. If NO changes need to be made: 12) Select Save. This basal pattern delivers 18 U over 24 hors. If changes need to be made: 12) Press. 13) Press press. Repeat Steps ) Select Save. 19

22 SECTION 1 : GETTING STARTED This basal rate amont entered, U/hr in this example, will atomatically be delivered throghot each hor continosly from one day to the next. U/hr :00 24: U/hr 00:00 06:00 12:00 18:00 24:00 24 Hors 1.92 Basal Pattern Setp Changing a Basal Rate If yor glcose levels are rnning too high or too low, this basal amont may need to be changed. 23 SECTION 1 : GETTING STARTED 1.93 Basal Pattern Setp Mltiple Basal Rates After yo start sing yor inslin pmp, yor glcose readings will help yo and yor healthcare professional determine if yor basal pattern needs to be changed. Not only might yo need to increase or decrease yor crrent rate, yo may also need to add basal rates to give yo different amonts of basal inslin dring certain parts of the day or night. HAVING MORE THAN ONE BASAL RATE... When Jessica was taking inslin injections, her BG readings were always high in the morning. If she increased her nighttime inslin, then she wold have low BGs later in the day. Now that she has her pmp, it is set to deliver more inslin in the early morning so her BGs aren t high when she wakes p, and less inslin later in the day when she doesn t need as mch. Helpfl hint: Most people need more than one basal rate to get the best control with their pmp. Work with yor healthcare professional to get yor basal rates adjsted correctly when yo start on pmp therapy. For example, yor healthcare professional has reviewed yor BG readings and has determined that this basal rate works well part of the day, bt that yo need a lower basal rate, U/hr, between the hors of 08:00 and 18:00. Yor basal pattern wold look like this: U/hr :00 08:00 18:00 00: U/hr 08:00 18: U/hr U/hr 00:00 06:00 12:00 18:00 24:00 24 Hors Now, let s make the changes to yor basal pattern. 21
