6 Key questions from Patient Leaders Blurred line between Lifestyle Apps and Medical Apps supporting patients. Some Apps are considered medical devices. How can we ensure technology is really user driven? Is patient involvement valued? How to ensure quality of information apps provide? What about the use of my data? How to ensure apps really benefit patients? Are safe? How should they be tested/certified?
7 Challenges: equity needs versus means Proven value for patient communities in remote areas, low (health) literacy access to technology: does it put people who would benefit most from such support at disadvantage? What reimbursement models for mhealth?
8 Why patient empowerment? Patient empowerment a key response to ensuring patient centred, quality oriented sustainable health and social systems of the future A core value of a modern patient-centred health system (Conclusions of the 2012 Danish Presidency) Reflects changing role of the patient for passive receiver to active user of healthcare services ehealth and mhealth are potentially key enablers for patient empowerment, if embedded and mainstreamed appropriately Trust and Acceptance are core issues
9 collective individual Levels of Patient Empowerment Micro level Individual patient, relationship with health professional, family context (e.g. self-management skills, shared decision-making process) Meso level Health care organisations / environments, care delivery processes (e.g. design of the environment, how quality is assessed, patient feedback systems) Macro level Policies, programmes at national (regional) and EU level (e.g. information to patients, patients rights laws, EU legislation, health professionals training curricula..)
10 Defining Patient Empowerment Patient empowerment is a process whereby the patient gains control over the management of their condition in daily life, take action to improve their quality of life and has the necessary knowledge, skills, and attitudes to adjust their behaviours and to work in partnership with healthcare professionals to achieve optimal wellbeing

14 Chain of TRUST project Patients: ehealth, mhealth and empowerment 92% of patients are willing to play a more active role in managing their own condition 60% of patients would be willing to use ehealth in the short-medium future but only 48% thinks they are ready to handle the additional responsibilities presented by ehealth
15 Chain of TRUST How about health professionals? 70% health professionals would be willing to use e- health in the short-medium future and only 29% believe that their patients will be in the position to use e-health service safely
16 SUSTAINS project on EHR Evidence on Empowerment process Increased patient control over his/her health status and management of chronic conditions Patients are more prepared for face to face visits Quality of consultations/interaction is higher as patients are more knowledgeable about their health (concordance) Patients prepare questions in advance Patients are able to draw HCPs attention to issues important to them Adherence is improved as a result of ownership
17 SUSTAINS project on EHR Resolving the question of patient access to their EHR is an important prerequisite to enable patient empowerment Cultural dimension of ehealth and mhealth If there is no resistance there is no real change!

19 Conclusions / Reflections Fundamental link between the use of ehealth and mhealth technology and patient empowerment connectivity in its widest sense Digital health literacy and confidence is key User driven technology how open are developers, big and small, to working with patients?, how open is industry to involve patients and citizens in the Code development and implementation? Important unfinished business on securing quality and safety, data protection, access and equity
