4 Annex C Certification Body Agreement for Cooperative Accreditation by Signatories of Multilateral Cooperative Accreditation Agreement 2003/09/15-Original Date 2003/11/12-Revision 2005/01/03-Revision The agreement to be signed by a certification body to become a mutually-accredited certification body under this MCAA is attached.

5 Certification Body Agreement for Cooperative Accreditation by Signatories of Multilateral Cooperative Accreditation Agreement As a certification body (CB) requesting cooperative accreditation by accreditation body (AB) signatories to a Multilateral Cooperative Accreditation Arrangement (MCAA) for specified programs, the undersigned, as a duly authorized representative of the identified CB, hereby agrees that the ABs named below may share all information related to granting, maintaining, extending, renewing, reducing, suspending, or withdrawing accreditation, including but not limited to assessment reports; nonconformity reports and related correction and corrective actions and reviews; and complaints and the results of investigations ( the information ), provided that the CB is given access, upon request, to all correspondence between the ABs relating to the Information or relating otherwise to the CB and/or its activities. The named accreditation bodies will safeguard the confidentiality of information in accordance with ISO/IEC Guide 61. This agreement shall remain in force unless terminated in writing by the authorized representative of the CB. The termination of this agreement for confidentiality will be applicable only to information received after the date of termination, and only upon receipt of the written termination notice by the named ABs. The CB also authorizes the named accreditation bodies to include in any assessment a review of accredited registrations issued by any of the other named accreditation bodies. The CB also agrees to: i. Notify every named AB of the status of its accreditation, including suspension or withdrawal, by every named accreditation body; ii. Upon request, provide every named AB the results of assessments by every other named AB, including nonconformities and related correction and corrective action; and iii. Upon request, provide every named AB the records of complaints about the certification body or the certification body s clients, and the results of any subsequent investigation and related correction and corrective action. The CB shall appoint a liaison to coordinate oversight activities with the named AB. The CB may be required to pay fees for an AB to plan and coordinate assessments with another AB, or for review and use of the assessments and reports of another AB. If applicable, the CB may be required to pay for the cost of translating an assessment report for another AB to use for granting accreditation to the CB. A copy of this agreement will be provided to each named AB.
