University Statement of Mission & Purpose Statements: How does the new certificate support this statement? B. Assessment of Need/Certificate Planning/Approval (Include Minutes) 1. Needs Assessment and Market Research: Discuss rationale for certificate, including an assessment of need (documented feasibility study: e.g., need, market, job market, career advancement opportunities, survey of student interest data). Provide data and research as an Appendix. 2. Documentation of SACSCOC recognition of proposed certificate, where required. 3. Documentation that all credits in each certificate are eligible to be transferred/applied to Associate s degree program or higher. 4. Documentation that faculty and other groups were involved in review and approval of certificate program (include minutes recording approval). 2 C. Certificate Policies (Include Financial Aid Endorsement) (Graduate certificate: include Matrices Endorsement) 1. Describe admission, curriculum, and completion requirements, in catalog format. 2. Graduate Level. If the proposed new certificate is at the Graduate level, secure the Matrices Endorsement from the Associate Registrar for Graduate Studies and provide as appendix. 3. Describe any special arrangements for grading, transcripts, or transfer policies. 4. Indicate if federal financial aid will be available to students for this certificate. 5. Once full certificate proposal, CCP, and market research is provided for Office of Financial Aid/State Approvals review, the Vice President for Financial Aid will recommend FSOC code and submit to U.S. Department of Education at planned points during the year. 6. Indicate, in writing, the understanding that after the U.S. Department of Education submission, the program will be tentatively eligible for Title IV aid for next semester that is 90 days or more out from date of submission. Title IV eligibility ceases if U.S. Department of Education raises questions during that initial 90-day period. 7. Indicate if any of the courses in the certificate have been in existence for less than one year. D. Curriculum Proposal (Include Institutional Effectiveness Curriculum Proposal Endorsement Form, CCP, State Approvals Endorsement) 1. Provide the following, as appropriate. Include information on any field experiences (internship, externship, co-op class, etc.) to be completed in any of certificate courses. b. Identify potential partner organizations for student field experiences, if students will be required to complete field experience in certificate course. 4. State Approvals. Review any relevant state activities that could affect Office of State Approvals in certificate (staff marketing, travel, location, field experiences, intensives, third party vendors, etc.). 5. If certificate will be offered in the online format, secure an endorsement from the Office of State Approvals (stateapprovals@liberty.edu). 6. List the course numbers and titles of all courses in the proposed certificate. Indicate hours and prerequisites. Note: Courses in a certificate program MUST be in existence for at least one calendar year prior to availability for financial aid. Course Prefix/Number Course Title Hours 7. If proposed change affects a certificate offered in the online format, secure an endorsement from the Office of State Approvals (stateapprovals@liberty.edu). 4. All Programs: If modifying a certificate that affects Liberty University en Español provide evidence that the appropriate entities have been informed of the change. It is the responsibility of the school/department that controls the curriculum to submit changes for programs offered in the bilingual format. F. Faculty Needs (Include Faculty Roster) 1. Provide complete roster (see attached Faculty Roster form) of faculty employed to teach in the certificate. Please note that faculty must have the required teaching credentials for course assignments. If a faculty does not have the required teaching credential, justify the course assignment using the Alternative Justification to Teach a Course form. (Provide Roster and signed completed Justification Form (if needed) as Appendix) 2. Show faculty course load in the new certificate as well as course work taught in other programs currently offered. 3. Provide evidence that adequate, qualified faculty members are assigned to support the certificate. 4. Describe impact of the new initiative on faculty workload. G. Physical Resources: (Residential Certificates Only) Classroom Space, Materials, Equipment Needs (Include Registrar s Endorsement only when special facilities or technology are required) 1. Location of the program being offered. (If off campus, include site approval) 2. Regular classrooms needed, projected through first three years. 3. Labs needed, projected through first three years. 4. Teaching materials, visual aid, software, equipment, etc. projected through first three years. 5. Assess impact of proposed change on existing programs and services. H. Submission of Request Submit request with appropriate signatures and all appendices and endorsements to the Chair of the Faculty Curriculum Council for undergraduate certificates and the Dean of the Graduate School for graduate certificates.
