2 APPLICATION PROCESS DIRECTIONS SPRING 2020 RTE 101 Once the Power Point is completed, please enter your name, Aims student number, authorization code and date into the RTE 101 Spring 2020 Application Process Directions Checklist. Please print this form called RTE 101 Spring 2020 Application Process Directions Checklist. This form is located on the Rad Tech webpage under the Start Today link #5 and should be submitted with your application materials.

3 TO BE ELIGIBLE TO APPLY TO RTE 101 YOU MUST: 1. Apply to Aims if you have not attended Aims within the last year. 2. All holds must be removed from your account prior to submitting your RTE101 application 3. Provide proof of having viewed the Online Orientation Power Point between January 2 and September 30, 2019 by providing the correct authorization code in the printed completion certificate 4. Complete all RTE pre-application coursework with a letter grade of C or higher 5. Have all official transcripts from outside institutions evaluated and applied to your Aims student record before 5pm on September 30, Please note that the state allows 30 days for institutions to evaluate transcripts Failure to submit transcripts in a timely manner may render your application incomplete and incomplete applications will not be processed The timing for transcript evaluations is particularly important if you take a course the summer of the same year you apply. You will need to address this piece promptly after receiving your grade.

4 REQUIRED PRE-APPLICATION COURSEWORK: Pre-requisite Courses 1. HPR 178 General Education Courses 1. BIO 106 OR BIO 201 AND 202 Note: If you elect not to take BIO 106 then BIO 201 and BIO 202 must both be completed successfully to fulfill the BIO 106 requirement. 2. ENG MAT PSY COM 125 (or approved course equivalents)

5 REQUIRED APPLICATION MATERIALS Your application materials consist of the following documents and can be found under the Start Today link on the Rad Tech webpage #5. Note that some of these documents require a signature. 1. RTE 101 Spring 2020 Application Process Directions Checklist 2. Certificate of Completion for Online Orientation Power Point with authorization codes 3. Current signed Background Acknowledgement form 4. Current signed Disqualifying Offenses form 5. Critical Thinking Assignment 6. Grade Verification form: listing the preapplication course information that you wish us to verify to satisfy the application requirements 7. Unofficial Transcripts showing required pre-application courses manually highlighted (these should match the courses you list on the grade verification form). This includes any courses taken at Aims! Your GPA score will be calculated utilizing the courses that you highlight. You should use your transcripts to complete this document with credits received, grade earned and term the classes were completed. 8. Documentation of at least ONE Hepatitis B vaccination or titer proving immunity. Hepatitis B is a series of 3 injections given over a 6 month period. Eventually, all 3 injections will be completed before the Fall 2020 RTE program start so plan accordingly. If you have had the Hepatitis B vaccinations, but cannot provide proof, you must submit a blood titer showing immunity

6 OPTIONAL APPLICATION MATERIALS 1. Medical Experience applicants may submit ONE of the following as proof of medical experience for an additional 0.5 points on their application. Proof of successful completion of a degree or certificate in Allied Health that included a minimum of 8 clinical hours in patient care. Students must provide an unofficial transcript and/or certificate of completion with clinical hours completed to satisfy this requirement. Proof of more than one year paid direct patient care (beyond administrative duties) Students must provide a letter from the supervisor or manager attesting to this employment, written on official letterhead and providing a direct telephone number for verification purposes.

7 IMPORTANT ITEMS 1. Allied Health Advisors or Rad Tech faculty and staff are NOT permitted to review application materials. 2. Students must be at least 18 years old at the start of the RTE Program.

8 APPLICATION DIRECTIONS 1. Application Materials may ONLY be submitted between the first and last days of September The s will be processed in the order they are timestamped. 2. Please scan and your completed application materials from (1.) your Aims account to (2.) This is to be used for applications only!!! The subject line should read: (3.) Rad Tech Applicant The body of the should include: (4.) your full name, (5.) Aims Student ID # and your (6.) contact phone number. 3. A ½ point will be deducted for each requirement on this slide that is not included in your application submission (1-6) 4. See next page for specifics on submitting application documents

9 MORE SPECIFICS FOR SUBMITTING APPLICATION MATERIAL We require that you scan ALL of your documents as ONE file; therefore, you should only have ONE attachment on your that contains ALL of the required documents. If your file is too large for ONE document, please for instructions. If you are missing one or more documents, your application will be considered incomplete and it will not be processed. Our intention is that you will receive an confirmation within 3 business days. Remember, we are not processing applications as they are turned in, so an response will just confirm that we received an attachment, NOT that the application was complete Applications will be deemed complete or incomplete at the time of processing, in October. If you realize on your own that you have failed to submit a document, you may Rad.Tech@aims.edu to ask permission to additional document(s). This courtesy will only be granted one time and a ½ point will be deducted from your ranking score after applications are processed. This one time request many only be used to submit missing documents. It may NOT be used to edit and revise documents that you have already submitted. We will be processing complete applications in October and will contact you by Aims if we have any questions. All applicants will receive letters in November letting them know if they were accepted into the RTE 101 class. The next slide will review the documents that need to be submitted through in one attachment.
