2 Applying On-Line Go to You will be presented with the log-in page Click on Apply Online
3 Selecting Region You will be prompted to select a region or division from the drop-down box. If only one region exists, the drop-down will display just that one selection. Click OK
4 Internet Settings Prior to filling out your application, you will need to make sure that your internet settings allow you to view the icons within the software To do this: Select the Tools menu at the top of your screen, and then select Internet Options
5 Enabling Icons Once the dialogue box opens, click on the Advanced tab Under Browsing, make sure that Enable visual styles on buttons and controls in webpages is NOT checked Click OK to continue
6 General Information The general information tab houses demographic information and contact information Please fill out all information within this section to ensure that your application can be processed
7 Calendar Icons Clicking calendar icons opens a calendar, from which you can choose a date. Always select the YEAR first. Then the Month. Then the Day.
8 Adding Phone Numbers To enter in a phone number, click Add A dialog box will open to enter in phone number and comments Click OK once the phone has been added Multiple phone numbers can be added in this section
9 Entering Addresses Enter in your permanent address first. If your permanent address and local address are the same, click on Copy from Permanent Address to populate the local address fields Be sure to provide your address to your recruiter for easy contact
10 Saving General Information Once all information is entered, click Save A box will pop up, instructing you to double check that the SS # is entered correctly- Click OK
11 User ID & Password Once the information has been saved, a User ID/ Password button will appear The address will auto-populate in the User ID field. You can create a different User ID if desired Enter in a password twice, to set a unique password Retain this information to continue to log into this site After the password is set, click Save and Proceed
12 Check List Tab The check list tab will ask you to select your degree You do have the ability to select more than one degree and specialty. Use the drop down box to make your selection
13 Check Lists Once the degree is chosen, a list of specialties within that degree will appear Click on the apply button next to any specialties that you possess Once approved, your recruiter will send you these checklists via to complete (watch for an to log into NurseTesting) Once all specialties are selected, click on Save and Proceed
14 Education Tab To enter in schools attended, click on the New button to open the dialogue box
15 Entering in Schools Attended Select the arrow to view a list of types of schools Complete all open fields Select the icon to view a list of types of degrees
16 Submitting School Use the arrows to enter in the date of graduation Date and Month are optional Graduation year is mandatory Once all information has been entered, click OK to add entry
17 Education cont. To add additional schools attended, select New To change/edit a school, click on the name of the school Select the Work History tab on the menu (left hand side of the screen) to continue (DO NOT click SUBMIT)
18 Work History Select the Work History link, to enter in past employment experience Please enter in at least 7 years worth of experience Select New to open a dialogue box to enter in past work experiences
19 Work History Dialog Box The work history dialog box will open Complete all information relevant to the job including: Facility Name and location Position held Dates employed Shift Reason for leaving Management information Patient ratio Number of beds Once all information has been entered, select OK
20 Work History Cont. To view details or edit a past employment experience, click on the name of the facility To add additional past experiences, select New Once all past experiences are complete, select the Disclaimer tab to continue
21 Disclaimer Select the answer on the right side of the screen If you answer YES to any question, use the box provided to describe details Once all answers are complete, click Save and Proceed
22 Medical Records The Medical Records tab houses all of the medical records that E-Health obtains as part of your employment If the JCAHO Required box is checked, then this is a record that you need to provide You only need to provide EITHER the Hepatitis B Declination or Vaccination, not both You only need to provide the chest X-ray if you have a positive TB Skin Test
23 Uploading Medical Records To upload a Medical Record into the system, click on the Available box, next to the record that you are providing Use the calendar icon in that row to provide the expiration date (if required) for that document (if the calendar icon is not lit up in color, you do not need to enter an expiration date)
24 Uploading Medical Records Select the Upload icon to upload a document A pop-up box will appear Use the Browse button to browse your files and select the document to upload Once the document is selected, click OK If you don t have electronic copies of your records, please fax them to: attention Human Resources Select Save and Proceed to continue
25 Professional Credentials If the JCAHO Required box is checked, this is a credential that you to provide to E-Health Upload electronic copies directly into the system, or fax all professional credentials to: Click Save and Proceed to continue
26 State Licenses The state license tab houses your state license information Use the drop-down menu to select the state Type in the license number Use the calendar to select expiration date If you have an electronic copy, use the upload icon to upload a copy into the system If you do not have an electronic copy, fax a copy of each license along with your medical records and professional credentials
27 Calendar To access calendar, click on the Calendar Tab on the you menu The calendar allows you to indicate days and times when you are unavailable to work
28 Blocking Days To block days that you are unable to work, click on the day in the calendar Then select the Cannot Work button Entering a Reason is optional Click Save to add to your calendar
29 Limiting Availability To limit the hours that you are available to work on a specific day, select the day Using the drop down arrows, select the time that you are available for that day Time is shown in military time Once the hours have been entered, click apply Limited hours will be shown in yellow on your calendar
30 Shift Preferences To select a shift preference, choose the date Use the arrow button to display a list of shifts Select the shift that you prefer Once the shift is chosen, click save to apply to your calendar
31 Limiting Multiple Days To block or limit hours on multiple days: Click on the day of the week to choose all Sundays in a month Click on the arrow at the beginning of a week, to choose the entire week All days chosen will be highlighted You can now use the buttons to block the day, select desired shifts, or specify availability
32 References Submit three references to E-Health To submit a reference, click on new
33 References Continued A reference pop-up box will open Fill out all information Click OK once completed Be sure to fill in all three references
34 Submitting Application Once all information has been entered, select Submit Application!
35 Incomplete Application If you attempt to submit the application without all pieces of information filled out, you will get the following warning: This will tell you any sections that still need to be completed Your application cannot be processed until all pieces are completed Please log back in using your username and password to complete the rest of the application In addition, you received an with your username and password, if you do not remember this
36 Thank you for Applying! You are now finished applying to E-Health Based on their needs, E-Health may send you online checklists, exams, and documents to be signed regarding your employment Please be sure to fax copies of all of your documents, if you have not already uploaded these
