4 WARNING If your attestation is under review, DO NOT MAKE ANY CHANGES Modifying EHR Program Registration can cause your pending attestation to reset Wait until you have been paid for the previous program year before taking these steps
5 What to Prepare NPPES Web Account username and password Allows access to the EHR program registration system If necessary, call the federal CMS EHR Help Desk at (option #1) to request username and password reset HFS Provider Information Sheet Shows HFS identifiers (business name must be exact) If necessary, call HFS Provider Enrollment at (select options 1, 2, 1) to request a copy 2

7 Registration and Attestation System Visit the EHR Incentive Program Registration and Attestation System ( Click the Continue button at bottom
8 Login Warning Review the warning regarding proper use of the Registration and Attestation System Click the checkbox to indicate your acknowledge these statements Click the Continue button
9 Login Instructions Enter the NPPES Web User Account login/password Individual providers should use their own NPPES Web User Account Practice managers/administrators can use their own NPPES Web User Account if it has been authorized by the provider to act on his/her behalf Click the Log In button
10 Home Tab Click the Registration tab at top (or Registration button in the page body)
11 Registration Tab: Instructions In the table of Existing registration(s), click the Modify button in the row of the provider to update Individual providers will only see their own name Practice Managers/Administrators will see the names of all providers who have authorized them to act on their behalf (or a search by name feature)

13 Registration Tab: Program Questionnaire Select Medicaid from the Incentive Program options Select Illinois from the Medicaid State/Territory dropdown Select your Eligible Professional Type from the dropdown Click Yes if you already have a certified EHR or No if you do not Enter your EHR Certification Number, provided by your vendor or looked up at (optional) Click the Save and Continue button
14 Registration Tab: Personal Information Select the payee TIN type (most will choose Group Re- Assignment ) Enter the Group Name exactly as it appears under the Payee Name section of your Provider Information Sheet Enter the Group TIN Enter the Group NPI Click the Save and Continue button

16 Registration Tab: Business Information Review the Business Address & Phone Number presented Make revisions if information is inaccurate (please note, this will NOT update information stored in your NPPES Web User Account) Enter and confirm an address that will receive all correspondence for this provider* Click the Save and Continue button * Strongly recommended: Create a new, MU-only address for this purpose that can be shared safely and easily to ensure receipt of communications in the event of staff changes

19 Registration Tab: Disclaimer Read the Registration Disclaimer Click the Agree button to accept the disclaimer Click the Disagree button to refuse to the disclaimer (cannot complete registration if you refuse)
20 Registration Tab: Submission Receipt Note your Registration ID on the Submission Receipt page Click Print Receipt to print a copy of the successful Submission

22 I&A System Login Visit the Identity & Access System ( hhs.gov/iaweb/login.do) Log in with NPPES Web User Account ID and password Click Sign In
23 My Profile Click the My Profile tab Click the Add an Employer button
24 Add Employer Search by Organization Name/NPI/City/State/Zip Click the Search button
25 Add Employer Search Results Review search results Click the radio button next to your organization
26 Select Role Enter the address that should receive your request Select role: Authorized Official, Delegated Official, Staff End User
27 Authorized Official Role If you are requesting to be an Authorized Official: You must submit to the EUS help desk a copy of the CP 575* [or approved alternate] for the organization for which you have requested to work on behalf of as an Authorized Official To help expedite your request please write the I&A Tracking ID on the copy of the CP 575 you submit to EUS You will receive an from EUS when your request has been processed * CP 575 is the letter you receive from the IRS assigning an Employer Identification Number (EIN). If you do not have a form CP 575: contact the IRS on from 7am to 7pm.
28 Delegated Official Role If you are requesting to be a Delegated Official: You MUST complete Option A or Option B below before your registration to act on behalf of the Organization below will take effect in CMS applications. OPTION A: Print, Sign and Submit to CMS the Delegated Official Certification for this request, along with the CP 575 [or approved alternate]issued by the IRS for the Organization for which you are requesting to be a Delegated Official. OPTION B: Please have an existing Authorized Official for this Organization approve your request by logging in to this system.
29 Staff End User Role If you are requesting to be a Staff End User: You MUST complete Option A or Option B below before you can act on behalf of the Organization in CMS applications. OPTION A: Please have an existing Authorized Official for this Organization approve your request by logging in to the I&A system. OPTION B: If you are only trying to gain access to your organization's Type 2 NPIs in NPPES, you can contact the NPI Enumerator for assistance.
30 Authorized Official Role Read the attestation statement Check the box agreeing to statement Click Submit button
31 Delegated Official/Staff End User Role Enter information on your organization s Authorized Official: Name Title Phone Click Submit button
32 Add Employer Confirmation Review actions required Upload CP 575 if necessary Click Done button
33 My Connections Log in as Authorized or Delegated Official Click the My Connections tab Click the plus sign next to the organization to expand options Click the Find Provider button to search
34 Add Provider Search by Provider NPI/First+Last/City/State/Zip Click the Search button
35 Add Provider Search Results Click the radio button next to the provider you want to access Select the EHR Incentive Program box (and others as needed) Click Continue
36 Add Provider Enter address where provider will receive request Click the Submit button

38 Provider Approval Provider logs into I&A system using their own NPPES Web User Account Pending approval messages will display on home tab Click Approve All Selected to give access to Authorized/Delegated Official or Staff End User
