1 Click the Registration Button to begin the process of registering to the BTE App. 1

2 Select a Program: Select the Recognition program that you wish to submit your data to and click the Next button. 2

3 Select a Network: Select your network from the available list of networks or create a network. Click the Click Here link to create a network. A network represents the practice or organization that the clinicians are associated to for the purpose of registering for BTE Recognition. Once you have created or selected a network, click the Next button to continue. 3

4 Create Network: To create a network, complete the required fields on the Create Your Own Patient Care Organization Network form. Required fields include, network name and physical location address. Once you complete the form click the Create button to continue. 4

5 Add Members: The next step in the registration process is to add and enroll members who will be submitting their data. Available Members: Lists members that are registered to your network and are available to submit for BTE recognition. In order to submit they will need to be moved to the enrolled members table by clicking the green plus sign. Enrolled Members: List the members that are selected to be submitted for BTE Recognition. To remove a member from this table click the red minus sign and they will be moved to the available members table. If you do not see who you are looking for, click on Click here to create a new member. 5

6 Add Member: To add a new member, complete the Add Member form. When you are adding existing members you will be asked to complete a few additional fields. They will be the members NPI number, Medical License and DEA number. 6

7 Registration: You are able to enroll up to 20 clinicians to submit data for in this setup, if you have more than 20 members please contact our support team at Once you have completed enrolling your members click Next to proceed. 7

8 Registration: From this screen you will be asked to select if you wish to be score individually or as a group. You are able to be scored as a group if you have registered more than two clinicians. Practices that are scored as a group, share the same results. Individual assessment are $145/clinician and a Group assessment is $395/group. 8

9 Payment: Complete the required fields indicated by the red asterisk in the payment screen to process your registration. 9

10 How it works: Once payment is complete you are ready to begin the process of submitting for recognition. Answer a few questions Add patient data Submit for scoring 10

11 Program Setup: You will be asked a few questions regarding your EMR system. This page will let you know what your reporting period dates are and what the submission date is set at. You are able to download the submission guide and data template prior to clicking next to ensure you understand the process. Once you are ready to submit click the Next button and this will lock you into your reporting period and you will have 90 days to submit. 11

12 Add Data: The next step is to add your data. You have two options to enter your data for submission. You can upload using the provided upload template (option 1) or manually enter your patient data with the online for (option 2). From this screen you will be able to see if your data is ready to submit and what your patient progress is. 12

13 Upload Data: If you have chosen to upload your data you will need to download the data template to enter your data. There is a definition file available for download that will guide you through what each field requires. 13

14 Upload Data: Once you have completed entering the data into your data template you are able to upload the file. 14

15 Review Data: Clicking the review data button on the add data screen will show you if you have any errors or warning in your file. In order to submit for scoring you must correct all errors. Warnings can be evaluated for further action or be left as is. 15

16 Add Data: If you select to enter your data manually select Option 2 and click the Add Patient button. 16

17 Add Data: When entering your data manually you will be presented with a Patient Data form in which you will input your data. 17

18 BTE Progress Monitor: Once you have reached the patient minimum for submission the BTE Progress monitor will let you know that your data is ready to submit. Click the Next Button to proceed. 18

19 Submit for Scoring: Once your data is complete and you are ready to be scored click Next. All results will be considered final. 19

20 Submit for Scoring: You will need to certify the method by which you are extracting your patient data. Once you have attested and clicked the appropriate boxes click Next to proceed with scoring. 20

21 Report: Your data will be scored which will take several minutes to process. Upon completing of scoring you will be shown a Report page that informs you of your assessment level and recognition level. 21

22 Performance Tab: This allows you a view into each measures to show where you performance stands and how you compare to others. It allows you access to resources on how you can improve for each measure. 22

23 Improvement Tab: This provides you with a list or resources that you can access to continue your education. 23

24 Members Tab: This allows you to view the members that you have enrolled in the program. You will be able to enroll new members and delete existing members. 24

25 Manage Tab: This allows access to administrators of a practice the ability to grant administrator rights to other members. 25
