1 Post Completion Optional Practical Training OFF CAMPUS EMPLOYMENT for International Students in F-1 Status POST COMPLETION OPTIONAL PRACTICAL TRAINING: Post-Completion OPT is defined as paid employment which is directly related to your major area of study after completion of studies. For more information on OPT refer to the PowerPoint presentation in the download module. The following are the instructions for completing documents for the Post Completion OPT application. You can find the required documents after the instructions. Money Order A money order for the amount $380. Payable to: DHS/CIS - Department of Homeland Security/ Citizenship & Immigration Services Address: USCIS Attn: AOS 2501 S. State Hwy, 121, Business Suite 400 Lewisville, TX Pictures Instructions to Passport Photos can be found in the download module. On the back of your pictures print your first and last name and I-94 card number lightly in blue or black, ball-point pen only. Your I-94 card is the little white in your passport that you filled out when you entered the United States. The I-94 card number is the eleven digit number on the card. Form I-765 Make sure you Print very neatly (in blue ink only) when filling out this form. Please follow the following instructions when filling out your I-765 form. Question 1: Make sure to write your FAMILY NAME or LAST NAME in CAPITAL LETTERS. Then write your FIRST NAME (i.e. FAMILY NAME, FIRST NAME) Make sure to write your name exactly as it appears in your passport.

2 Question 2: If you have changed your name since you have been in the US, Please write the old name here. Or if you have used another name on other documents please write that name here. If not, leave blank or write N/A. Question 3: Before filling out this form, make sure you are at an address for which you will live at until you receive this card. For example, if you are living on-campus, it is best for you to use someone else s address but only if your name can be placed on the mailbox. Warning: If you change your address by filing a change of address with the post office, the post office will not forward the mail to your new address. The post office is very sensitive about forwarding any sort of government mail. If you are not sure about your address situation, please leave this question blank. Question 10: The I-94 card is the little white card in your passport. The I-94 card number is the 11-digit number on the top. The I-94 card should be in your passport, or you may have it stapled to your I-20. This is the same number you have to write on the back of your photos in blue or black ball point pen only. Question 11: If you have had pre or post completion in the past you will need to check YES. If you have had curricular practical training (paid internship) in the past you will need to check NO. If you have had Economic Hardship before, you will need to check YES. If you have worked on-campus, you will check NO. Depending on which state you were and it depends upon the INS Service Center which would have processed your application. Leave this area blank unless you know which center it was sent to. It would be one of 4; Vermont, California, Texas or Nebraska. If you were authorized for the employment, write Granted. If you applied and were denied authorization for the employment, write Denied. You will also write the dates you were authorized. For example if you were authorized part-time Pre Completion Practical Training you will write:

3 PT Pre-OPT 06/01/2000-9/02/2000. If you were authorized for full-time Post Completion Practical Training you would write: FT PCPT 12/31/ /30/2000. Question 12: Date of your last entry is stamped in red on your I-94. Question 13: The place of your last entry in the US is also stamped on your I-94 card. If you entered at JFK in NYC it will read: NYC, if you entered in San Francisco, it will read: SFR and if you entered in Miami, it reads: MIA Question 14: If you entered in F1 status the last time you entered the USA, write F-1 Student. However, if the last time you entered the USA you were in B-2 or H-1B status, then write that. Question 15: If you are currently in F1 status, write F-1 Student Question 16: Fill in the following: ( c ) ( 3 ) ( b ) Now that you have completed form I-765, please print your NAME, PHONE NUMBER and TODAY S DATE. Paid employment without authorization is illegal. Continuing to work beyond the date authorized on your I-20 is also illegal.
