1 Microsoft Office 365 Forms Microsoft Forms is a new part of Office 365 Education that allows teachers and students to quickly and easily create custom quizzes, surveys, questionnaires, registrations and more. When you create a form, you can invite others to respond to it using any web browser, even on mobile devices. As results are submitted, you can use built-in analytics to evaluate responses. Form data, such as quiz results, can be easily exported to Excel for additional analysis or grading. Open the Portal and Office 365 Click on the Waffle and then Forms. Remember this is web based so only one click is needed. This is the landing page for your Forms. It is not saved on your OneDrive, but it is stored on Microsoft servers located in the USA. All your existing forms and responses (up to 5,000 responses/form) will be here until you delete them. The results can be exported into Excel for future analysis before removing them from the form or deleting the form altogether. This landing page gives you two options: New Form or New Quiz. 1

2 Click on Untitled Form and rename the document and it automatically saves the entire document to the cloud. The four question types are Choice, Text, Rating and Date. Click Add Question to get started. Click on New Quiz and rename in the same manner. The four question types are the same, but choice is different. You can assign correct answers and points per each question and it scores automatically. Click Add Question to get started. There are four question types each with unique features. Just click add (+) and create your form. Choice aka multiple choice. You may have multiple answers and the question could be required. You can add subtitles and shuffle options or possible answers. Text. It defaults to short answer, but you can choose to make it long. It may be required and also has subtitles. It also has a set of restrictions for math. (Number, greater than, greater than or equal to, less than, less than or equal to, equal to, not equal to, between and not between). Rating. Assign stars or numbers to rate something. The stars or numbers can be labeled and required. It will be averaged as data is collected. Date. Just like it says you can ask what the best date for a study session, meeting etc. 2

3 In the upper right hand corner there are some options. Preview. It will allow you to interact with your form and actually see how the end-user will see and interact with the questions. It can be viewed as it would appear on a computer or on a mobile device. Theme. A limited choice at this point and this theme can only be seen on a computer and not a mobile device. Share. There are many options here: o It can be shared via a link and if you choose only people in my organization you can choose to see who replied. o If you choose anyone with the link it will be anonymous. o Use a QR code o Embed code o You can also share the Form as a template with others and even collaborate as you create. To find Forms shared with you click on Other Forms. 3

4 Three Dots. Choose Settings. o Something you cannot do in Excel Survey is limit the number of times someone can respond. If you choose only people in my organization you can set it to one response per person and this selection will require the end-users name be displayed. o There are other options to be explored here as well. **** The Help section in Settings is a valuable tool if you have questions on the apps use. Remember this app saves automatically and will be stored in the app and not on your OneDrive. 4

5 Responses Tab These are sample responses from a test form about the ski season in Maryland. You can open the data in Excel In the Summary section you can see all responses or individually. Each question is then broken down into itself where you can see the questions and answer and the number of responses. Hover over the chart and it will give you the percentages. If you click on Details you ll see all responses from individuals. The rating averages all responses. Explore other possibilities. Again there can be up to 5,000 responses to each form. The responses can be deleted. To delete responses to use the form again go to the response section and specifically the Summary. Click on the three dots and there is a choice there to delete responses. You can delete an entire form at the Forms landing page.
