26 Using GoFormz Launch GoFormz via Application or Website Login to your account Navigate to the ODOT GoFormz Home Form ALT ID (6-digit project no.) PLN (4-digit) Form Type: Quality Form with worksheets and/or Quantity Form

27 Using GoFormz Appropriate forms and worksheets are downloaded to your device Header information will be prepopulate on all of the forms Forms are dynamic, where selecting Checkboxes will indicate additional fields required to be completed Data stored in the Cloud

28 Using GoFormz Select the Complete button, when the inspection is done and ready to be submitted to the project files PDFs of the forms will be sent to the project SharePoint site If all required fields have not been filled, an error message will be shown instead
