1 Introduction to The Cochrane Library

2 What is The Cochrane Library? A collection of databases that include high quality, independent, reliable evidence from Cochrane and other systematic reviews, clinical trials and more. Brings together research on the effectiveness of healthcare treatments and interventions from around the world. Based on the practice of evidence-based medicine Used by clinicians, policy makers, researchers, educators, students, patients and others. Evidence-based medicine is The conscientious, explicit and judicious use of current best evidence in making decisions about the care of individual patients. 3 Who develops the Cochrane Library? The Cochrane Library is published by Wiley for The Cochrane Collaboration. The Cochrane Collaboration is an international not-forprofit organization: Aim: to help people make well-informed decisions about health care. How: by preparing, maintaining and promoting the accessibility of systematic reviews on the effects of healthcare interventions.

4 The Cochrane Collaboration In early 2010 there were 27,724 contributors from 110 countries. Member Collaborative groups Review of The Groups Cochrane - maintain Collaboration and update include: Cochrane Reviews. Cochrane Centres - support people in their geographic and linguistic area. Fields - bring together healthcare issues impacting across review groups. Method Groups - develop the methodology of Cochrane Reviews. Consumer Network represents the interests of healthcare consumers.

5 The Cochrane Collaboration global community The Cochrane Collaboration, through the Cochrane Library, facilitates a global community approach to healthcare by promoting education, research and accessibility to everyone. The Cochrane Collaboration reacts quickly to current health issues, such as the Haiti and Chilean earthquakes, making relevant reviews available for free. In addition they set up specific task forces to investigate and research high impact current issues such as the recent Swine Flu and the Indian Ocean tsunami in December

6 What can you do with The Cochrane Library? Use: For improved patient care and outcomes. Based on the best available evidence which have been proven in practice. 7 The Cochrane Library databases 1. The Cochrane Database of Systematic Reviews (Cochrane Reviews) A systematic review investigates the effectiveness of interventions for prevention, treatment and rehabilitation in health care. Includes reviews of diagnostic test accuracy studies. Now updated monthly! 2. The Database of Abstracts of Reviews of Effects (Other reviews) Complements Cochrane Reviews by quality-assessing and summarising reviews which have not been carried out by The Cochrane Collaboration.

The Cochrane Central Register of Controlled Trials (Clinical trials) Includes details of published articles taken from bibliographic databases such as MEDLINE and EMBASE - and other published sources. 4. Health Technology Assessment Database (Technology Assessments) Brings together details of completed and ongoing health technology assessments from around the world.

9 The Cochrane Library databases 5. Methodology Register (Methods Studies) Presents a bibliography of publications that report on methods used when conducting controlled trials. 6. NHS Economic Evaluation Database (Economic Evaluations) Systematically identifies economic evaluations of healthcare interventions from around the world, appraises their quality and highlights their relative strengths and weaknesses.

10 The Cochrane Database of Systematic Reviews The gold standard for systematic reviews Combines results of the world s best medical research studies Adhere to a strict process to make them more comprehensive, thus minimizing bias and ensuring reliability Updated regularly - ensuring that treatment decisions can be based on the most up-to-date, reliable evidence.

11 What is a systematic review? A systematic review identifies an intervention or treatment for a specific disease or other problem in health care Identifies and includes all up-to-date and relevant studies and trials Determines whether or not the intervention or treatment works

12 What is a protocol? The plan or set of steps to be followed in the systematic review Should describe the rationale for the review, the objectives, and the methods that will be used to locate, select, and critically appraise studies, and to collect and analyse data from the included studies

Clinical guidelines, graded evidence summaries, images, audio samples and videos to guide in applying evidence in practice. Health Economic Evaluations Database What are the costs involved? Provides comparative analysis of costs and consequences of health interventions to aid decision makers.

16 Browse Cochrane Systematic Reviews by topic or expand Other Browse Options for further browsing choices. Search for trials Browse the other resources in the Cochrane Library

17 Click Advanced Search to access Advanced Search, MeSH Search, Search History and Saved Searches.

18 Search tips Use *, and Boolean operators AND, OR, NOT to refine or broaden your search.

19 Limit your search to a specific field from the drop down menu. Specify Product, Record Status or Date Range

20 If no product specified, Cochrane Reviews results are displayed if available, otherwise the next database with results is displayed. By selecting specific criteria you can retrieve more relevant results. Click on Edit Search to refine your search further.

21 If you are registered and logged in you can save your search query. Further sorting options.

23 The Medical Subject Heading (MeSH) search is based on the National Library of Medicine's controlled vocabulary thesaurus of medical subject headings index. MeSH terminology provides a consistent way to retrieve information that may use different terminology for the same concepts (see )

24 Searching for a MeSH indexed term allows focusing of the search to records that are in fact about that specific topic; For example a text search for bowel (AND) cancer may bring up a record that includes the terms bowel and cancer but is in fact discussing treatment of irritable bowel syndrome. If the database contains shorter documents with just an abstract and title for example searching for a MeSH indexed term can ensure that relevant documents are retrieved since the term may not be specifically mentioned in the title or abstract.

25 Search tips Enter your search term and click Thesaurus or Go To MeSH Tree options to view related MeSH index terms. Note: Not all records in The Cochrane Library have MeSH indexing. Thesaurus entries for the term kidney

26 When ready click on View Results Select qualifiers to refine your search. Tree structure showing all MeSH headings referring to kidney

27 Search Combine searches tips to maximise search efficiency. And refine searches further by product, status or date. View a list of searches from your current session on The Cochrane Library.

28 To use the Saved Search feature you must be a registered Wiley Online Library user and logged in. Run the search, or export the search query in plain text format. Activate or stop an alert. Delete selected searches.

30 An example Cochrane Systematic Review Download PDF versions. The Standard version includes only a summary of the Data and Analyses, whereas the Full version includes full details. From the Table of Contents you can quickly link to all sections of the review or protocol, including the Plain language summary, Background, Objectives, Methods and Results

31 In the Results section you can find out more information on Characteristics of included, excluded and ongoing studies.

32 The References section includes links, where available, to electronic sources of referenced documents.

34 Example of a figure Odds Ratio Table Describes the odds of an experimental patient suffering an adverse event relative to a control patient. Size of blue or green square relates to weight %, and diamond indicates overall odds.

35 Download the citation, or abstract and citation, for this article in plain text format.

36 Select Citation or Abstract and citation.

38 Links to browse and search options. To view a record, click on the title. Download citations for one or more results.
