Client Details Client : Address : Sunkom Pty Ltd 104 Bronzewing Drive ERI NSW 2250 AUSTRALIA Equipment Description Test Item Details Type: Name: Serial Number : Software Version : Firmware Version : Hardware Version: Manufacturer : Network Port(s) : Other Port(s) : FDX card A101, A102, A104 & A108 V31 AFT-A102-SH Sunkom Pty Ltd Eight port digital line(a108), four port digital line(a104), Dual Port digital line (A102) & Single port digital line (A101) Connectors to PCI or PCI express cards Testing Overview The AFT A108 is an eight-port FXD module. The four-port model, AFT A104 is an unpopulated version of the A108. The AFT A102 is a dual port FXD module. The single port model, AFT A101 is an unpopulated version of the A102. All modules were assessed as being fitted to either a PCI or PCI express card with no isolation provided between the digital network and the internal circuits, therefore classified as SELV throughout, only suitable for connection to SELV circuits. The client has updated the user manual with the appropriate SELV warning and applied a durable label next to the network ports indicating only suitable for connection to SELV circuits. 12 May 2008 Page 3 of 16 ABN 38 076 747 085
