GENERAL INFORMATION The instructions in this guide will provide you with the necessary information to complete your 2012/2013 French Language Services (FLS) report. This user guide is intended for non-identified agencies only. Non-identified agencies have the obligations in their M-SAA / H-SAA to report on FLS with respect to how they address the needs of their local Francophone community. The completion of this report, designed as a survey, will allow non-identified agencies to be in compliance with their accountability agreement with the LHIN, regarding reporting on French Language Services. This FLS report is in an Excel format and includes Macros. Make sure to enable macros in order to be able to complete this file. Refer to the most recent CATSRI User guide (Appendix) for detailed instructions. MAIN MENU The four following steps will have to be completed in step order to finalize your 2012-2013 FLS annual report: Step 1: Provider Details. Step 2: FLS Data Input for Non-Identified Agencies. Step 3: Select Services provided. Step 4: FLS Report. Click on the name (In Blue) of each step to be directed to the page to complete. Start with step 1 up to step 4. When you open the 2012-2013 FLS report, you see the following page; this is your main menu: 3
STEP 1: PROVIDER DETAILS In this section: Complete the Details column by typing the requested information in the yellow cells, and by choosing the answers from the drop down lists offered in blue. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu at the top left of the page. When the page is completed, click on the hyperlink Continue to Step 2 of initial Setup>>> at the bottom left of the page to complete step 2. 4
STEP 2: FLS DATA In this section: Complete the Data column to the best of your knowledge by typing the requested information in the yellow cells. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu at the top left of the page. When the page is completed, click on the hyperlink Continue to Step 3 of initial Setup>>> at the bottom left of the page to complete step 3. 5
STEP 3: SERVICES SELECTION SCREEN In this section: Enter an x under the Funded Services column for all the services funded by the LHIN in your agency. The lists of services are extracted from Ontario Healthcare Reporting System (OHRS). It is possible to switch listing from hospital services to community and LTCH services by clicking on the hyperlinks Link to Community and LTCH Services>>> (illustrated below) and Link to Hospital Services>>>. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu. When the page is completed, click on the hyperlink Continue to Step 4 of the Report>>> at top of the page to complete step 4. 6
STEP 4: REPORT This report is built as a survey. Please, answer the series of questions (1.0 to 7.0). Start with question 1.0, and the next question will appear after you select the answer to question 1.0, and so on. For each question, select the answer from the drop down list. You will have the opportunity to explain your answer in the Additional comments column. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu. ACRONYM TABLE & DEFINITIONS Acronym Table FLS French Language Services H-SAA Hospital Service Accountability Agreement L-SAA Long-Term Care Home Service Accountability Agreement LTCH Long-Term Care Home M-SAA Multi-sector Service Accountability Agreement Definitions Active Offer To ensure that members of the public know they have the right to use the official language of their choice, the institutions are required to actively offer services in both languages. The requirement consists of informing the public that services are available in both official languages. Non-identified agencies are health service providers that have not been identified by the LHIN to provide services in French to the public. However, non-identified agencies might deliver or aim to deliver services in French to their clients for many different reasons such as: - The services are requested in French by the patients; - A large population of Francophone is present in their catchment area; - The agency already has the capacity to deliver services in French; - The agency provides a unique service in a designated area or in an area where Francophones are present; Non-identified agencies need to report on FLS to the LHIN with respect to how they address the needs of their local Francophone community. 8
