How to Set Up a FMH Mobile Account: Setting up a mobile account is fast, quick, and convenient for you and your provider! If you wish to set up an account follow these simple steps below: If you do not already have a FMH account: 1) You must first receive an email invitation sent from your provider s office to set up an FMH account. 2) Once you receive an invitation, follow the instructions in the e-mail to set up an account. 3) Download the FMHMobile app on your phone (for instructions on how to download the app, please refer to page 3). If you have an existing FMH account: 1) Proceed to download the app via your mobile device s app store. (For instructions on how to download the app, please refer to page 3). ***Please Note- if you set up a Mobile account without receiving an e-mail notification first from CapitalCare, your mobile account will be blank!*** D oh forget to ask your provider s D office for a FollowMyHealth Invitation First! 2

How to download the FollowMyHealth Mobile app: Downloading the FollowMyHealth Mobile app is quick and easy! To install the app, follow the steps below: 1. From the iphone home screen select the App Store icon. 3

2. Once in the App Store, click on the search tab. Here is where you will select Get and Install. 3. Select Search at the top of the screen, and type FollowMyHealth. 4. Click on Get next to the FollowMyHealth app, and then select Install. 4

You should now see the FollowMyHealth app downloading on the last page of apps on your phone! If you are having difficulty finding the app, drag down on the home screen and type FollowMyHealth. 5

How to Sign In: There are a few ways you can sign into your mobile account. This will be dependent on the type of phone you have and/or the method you initially use to sign up with (social media, e-mail or FMH ID credentials). If You Signed-up Through a Follow My Health ID: Click on the big Sign In button with the green FMH logo and enter your appropriate username and password (look at red arrow in display above for reference). If You Signed-up Through your E-mail or Facebook Account: If you have signed up through your E-mail or Facebook account click on the or, sign in with an alternative below the Sign In button. This will redirect you to your Google, Yahoo, Outlook, or Facebook account to sign in and access your Patient Portal (look at yellow arrow in display above for reference). Remember usernames/passwords are case sensitive, so be sure to input the correct information 6

Now you can easily sign in through your mobile app! How to Sign In Using Touch ID: If you are using an iphone 5S or later that has been updated to ios 8 or higher, or an Android device with fingerprint identification it is possible to sign into your FollowMyHealth portal using your fingerprint! To Sign In Using Your Fingerprint To sign in through Touch ID, click on Device Settings, followed by Security, under the More tab. 7

From the Security menu, click Create Passcode, and then enter in a four-digit passcode of your choice. Enter the same passcode in again to confirm your passcode. It is highly recommended that you write your passcode down for future reference. Once you have created your four-digit passcode, you will be brought to the Security menu. From here simply click Enable Touch ID to allow yourself to sign in using your fingerprint! Once you have done this, you will be able to sign in by simply opening the FollowMyHealth app, and placing your finger on the fingerprint scanner (for iphones the scanner is located on the Home Button)! 8

Forgot Your Password? Can't remember your password? Keep in mind that if you have chosen to use your login with Facebook, Google, Yahoo, or LiveID as your login for your FollowMyHealth Universal Health Record, you will log in using the same credentials (username & password) that you use to sign in for those accounts. If you do not remember which method you signed up with, you can e-mail followmyhealthsupport@capcare.com, and a representative will verify that information for you. 9

Resetting Your Password If you still can t remember your password and have a login through FollowMyHealth you can reset your password through these simple instructions below! Remember, if you have a log on through your e-mail or Facebook account you must reset those separate account credentials, you cannot reset your password by clicking the Forgot Your Password tool, this option is only available to patients who sign in using a FollowMyHealth ID. 1. Click the Forgot Your Password button located below the Sign In button (see display reference below). 10

2. You will then be prompted with this screen: 3. Now simply enter your username and select the Reset Password button (see display above for reference). An e-mail will be sent to you with detailed instructions on how to reset your password, and you will be able to access your account! Still Having Trouble? If you are still experiencing any problems with logging on, e-mail followmyhealthsupport@capcare.com for further assistance. Please note that a representative will respond within 3 business days to your issue or concern. 11

How to Request an Appointment Appointments can be requested easily from anywhere in the FollowMyHealth Mobile App by following the steps below: 1. From the Appointments tab at the bottom of the page select the + symbol. 2. Select CapitalCare Medical Group as the organization, and then choose the provider that you would like to request an appointment with. 12

3. Select Next at the bottom of the page, and then select Add Time Slot 13

4. Select when you would like to request an appointment, and then select what time you would like the appointment to be at by sliding the two green arrows at the bottom of the page. Once the desired time slot(s) is selected, press Done on the top right of the page. 5. Select Next to move to the Confirm tab. Under Reason For Appointment, enter in the issue that you are having and why you want to see your physician. Under Comments, enter in any other relevant information. 14

6. Press Send Request at the bottom of the page to send your appointment request to your doctor s office. You can review your requested timeslots here. Please note that by sending a request for an appointment, you are not scheduling one. You will hear back from your office if the requested time is available, but there is no guarantee that it will be. For this reason, it is encouraged that patients request multiple time slots, so that there is a higher likelihood of an open time slot being available. 15

The Home Page Your Action Center will display any updates and/or notifications to your account such as appointment reminders, test results, and messages from your provider. If you look to the right of the Action Center you will see the Recent Activity tab. By selecting this you will see a timeline of updates that have been added to your personal health record. 16

Choosing Notification Preferences for Appointments or Updates All notifications for your personal health record are maintained under "Device Settings" (see display reference below). Here you can choose which updates to your health record you would like to receive notifications for! Please note the default notification preference is off, and you must manually turn on Push Notifications to be alerted of changes to your personal health record. To Change Your Notification Preferences Complete these 2 Steps: 1. Click on "More (next to the name on the account), followed by "Device Settings". 17

2. On the next screen, select Notifications, and then change the Enable Push Notifications preference to on. 18

The Messages Tab The Messages tab is a unique feature that contains messages from your healthcare provider/office, and allows you to actually send a message directly to your provider at your convenience! It is located right next to your Home tab. To compose a message, select the icon on the top right of the screen. (Indicated above by red arrow) 19

Once selected, you will be brought to the screen below with the following options: A drop-down for provider name 1 A box for the subject of the message 2 A box for the body of the message 3, which is limited to 1000 characters. Once these three sections are all filled out, you should be able to click the "Send" button 4 o The appropriate staff will respond as soon as possible. 4 1 2 3 Please note: for life-threatening conditions, do not send a FollowMyHealth message to your doctor. Please call 911 or visit your nearest ER. 20

My Health Tab Want to refill your prescription? How about checking your lab results? The My Health Tab can provide you with that information and much more! It is located right next to your Inbox tab (see display reference below). Once you click on the My Health tab you should see this display above. The first page that will appear is your Summary tab. This will show a snapshot view of all of your diagnoses, medications and most recent vitals. 21

My Health Tab Conditions Tab Your Conditions tab is located right under the My Health dropdown menu (see display below for reference). Here you will notice all of your active and past medical conditions conveninetly all in one place. You can actually add your own conditions that you wish to keep track of until your next appointment! (Refer to page 22 to learn how to add a condition). Notice to the far right a blue button (circled in red above). If you click on it, a detailed description of your condition with it s common name, basic information, and links to learn more will appear: 22

You can click here for more information on your health condition. My Health Tab How to add information to your account: FollowMyHealth offers patients the ability to add their own medical information to their personal health record. You can add or delete conditions, medications, immunizations, allergies, results, vitals and/or documents to keep track of important health data such as your weight or blood pressure! Please note if you add or delete any medical information on your patient portal it does not change your permanent health record at your physician s office. To Add Information: 1. Navigate to the section under My Health for the appropriate type of information you want to add. This can be conditions, medications, allergies, immunizations, results and or vitals. 23

2. Click the "Add" button (in red displayed in image below), here we are adding a new vital. 3. This will bring up a box to enter the information you want to add, here we are inputting our blood pressure. Input the required information, then click Save when you are finished (see example image above). 24

My Health Tab Medications: Your Medications tab is located right under the My Health dropdown menu (see display on page 19 for reference). Here you will notice all of your active medications listed, dosing instructions, date received, number of refills, and the provider that prescribed them. You can also add medications if you wish to customize your health record. (Refer to page 22 to learn how to add a medication). Notice to the far right a blue button again. If you click on it, a detailed description of your medication will appear with basic information and links to learn more: 25

To Request a Refill on a Prescription: To submit a request to your provider s office for a refill on a medication: 1. Click on Renew at the bottom of the medication you would like to renew (circled in red on page 23): 26

2. The box below will then be displayed. Note that you can change your pharmacy location by clicking the Select a pharmacy button (circled in red below). After you have reviewed the information click Send. 3. You will then receive a notification from FollowMyHealth that your request has been received. Once your prescription is sent to our office your CapitalCare provider will approve and process your prescription! 27

My Health Tab Allergies: Your Allergies tab is located right under the My Health dropdown menu (see display below for reference). Here you will notice all of your active allergies. You can also add you own allergies that are not listed to customize your portal! (Refer to page 22 to add an allergy). 28

My Health Tab Immunizations: Your Immunizations tab is located right under the My Health tab (see display below for reference). Here you will notice all of immunizations since January 2014, when FollowMyHealth was created. (Refer to page 22 to add an immunization). 29

My Health Tab Results: Your Results tab is located right under the My Health dropdown menu (see display below for reference). Here, you can check all of your lab results and see if they are within the normal range! You can also add any results from tests that are not listed below to further customize your health record. (Refer to page 22 to add a result). 30

My Health Tab Vitals: Your Vitals tab is located right under the My Health dropdown menu (see display below for reference). Here, you can keep track of vitals such as your blood pressure, weight and body mass index. You can input vitals you have taken yourself to further customize your portal. If you input 3 or more vitals a graph will be generated for you to visually view your progress! Note that the graphs are only viewable from the desktop version of FollowMyHealth. (Refer to page 22 to add a vital). 31

My Account Tab: Your info tab contains all of your demographic information such as your name, emergency contact, insurance, and contact information, that you can easily update at any time. The My Account tab is located under the More tab (see display reference below). If you choose to update any of this information including your insurance, it will automatically send a request to your medical provider s office to update their record so there is no need to call the office! 32

Need Help or Support? If you have any questions or concerns regarding your portal account you can access the support button under the More tab or e-mail followmyhealthsupport@capcare.com with your concerns. To request help via the portal: 1. Click More and choose Support from the dropdown menu (see display below for reference). 33

2. Click Contact Support and choose Create Support Request (see display for reference). 3. You will then see this support box below (see image above for reference). Here you can choose whether you want to e-mail support or visit the FollowMyHealth website that has additional resources, such as tutorials and frequently asked questions. 34
