4 What Is MyHealthRecord? MyHealthRecord is a secure online website that provides patients personalized and convenient 24-hour access to portions of their Family Health Centers of San Diego medical records. Please visit MyHealthRecord at: What Are the Benefits of MyHealthRecord? Improves the overall patient experience. Increases communication between patients and their providers. Available anywhere with an internet access. Available on a mobile-friendly site. Is convenient and secure. Is available in both English and Spanish. Supports preventative care by providing key health educational documents. Empowers patients to be partners in their care. It s free and easy to sign up! What Can You Access Via MyHealthRecord? Schedule, view, cancel or reschedule appointments. Request medication refills. View labs and immunization history. View past clinic visits. View your health summary. View your medication and allergy lists. Send to and receive messages from the medical team. View important information on health topics, such as diabetes and high cholesterol. Please Note: Only current patients 18 years and older can sign up. Currently, patients cannot request dental appointments or contact dental providers via MyHealthRecord. Each patient must create his or her own individual MyHealthRecord account. Patients cannot share addresses with another person so please ensure you are providing your personal address. Duplicate addresses will be rejected by the system. It is not a place for communicating urgent medical information. If you are experiencing a medical emergency, please contact your clinic or 911 immediately. 4 P a g e

5 MyHealthRecord Registration MyHealthRecord uses an address to authenticate and authorize user credentials to allow the use of MyHealthRecord. You must have a personal address to begin the registration process. If you don t have an address, please sign up for one! There are many free providers you can choose from please see our example of creating an account through GMAIL. How to Sign Up For an Address: Go to this link: Complete the personal information: o First and last name, preferred user name, password, date of birth and gender. o You do not have to fill in mobile phone or alternative address. Check the I agree box at the bottom. Click next step. Congratulations! You now have an address and can begin the MyHealthRecord registration process. 5 P a g e

6 Registering for MyHealthRecord Signing up is easy! Simply provide Family Health Centers of San Diego your name, date of birth and personal address and you will receive a link in your inbox to complete the registration process. You can provide this information: To the front desk receptionist at your next visit. By calling (619) By ing MyHealthRecord@fhcsd.org. Once your address is added to your electronic health record, you will receive an welcoming you to MyHealthRecord. This will include a link to a webpage where you will be prompted to enter your address and date of birth. Upon clicking submit, an will be sent with a link that takes you to change your password. After you select a new password, click on Complete Registration and you will be redirected to the MyHealthRecord log in page. 6 P a g e

7 Self Registration As long as FHCSD has your address and date of birth on file, you can self-register from the home page. 1. Click the Register? link from the home page. 2. Enter your and date of birth. 3. Click Sign Up. You will receive an welcoming you to MyHealthRecord. This will include a link to a webpage where you will be prompted to enter your address and date of birth. From there, you can complete your registration. 7 P a g e

8 Password Reset 1. Click the Forgot Password? link from the home page. 2. Enter your and date of birth. 3. Click Sign Up. You will receive an confirming your password reset request. This will include a link to a website to continue with resetting your password. 8 P a g e

9 Logging in to MyHealthRecord After you are registered, you can log-in to MyHealthRecord by following these simple steps: Go the following web address: Note: The url must include https: Enter your address in the [ ] field. Your username is the same address you provided to Family Health Centers of San Diego. Type your password into the [Password] field. If you forget your password, click the Forgot Password link to have an sent to your on file with further instruction. Click the [Login] button to enter MyHealthRecord. 9 P a g e

10 Home Screen Once you are logged into MyHealthRecord, the home screen is your starting point to access a variety of helpful health information. From the home screen you will be able to access all of the main features of the site. You can navigate MyHealthRecord either through the three main icons on the home page or by using the top menu bar. 11 Home Screen Navigation Request an Appointment Enter a preferred range of dates and times with a provider and/or clinic of your choice. View My Health Information View current health issues, allergies, medications, lab results and immunizations. In this section, you are also able to request medication refills. Send a Message Send a referral request or a non-urgent message that will be sent to nursing staff for review. Messages will be responded to within two to three business days. 11 P a g e

12 Upper Menu Tab Navigation Located on each page of MyHealthRecord, the upper menu tab navigation is a convenient way to move through the website Home/FHCSD icon Clicking on this icon or text will take you back to the homepage. Appointments Request an Appointment o Enter a preferred range of dates and times with a provider and/or clinic of your choice. Upcoming Appointments o View any upcoming appointments currently scheduled with FHCSD. Past Visits o View a list of all of past medical appointment dates, including clinic, department, name of medical provider and a copy of the After Visit Summary. My Health Health Summary o Displays a list of current health concerns that are detailed in your electronic health record. Medication o Displays current medications that are detailed in your medical record. o Able to request medication refills. Allergy o Displays a list of allergies that are detailed in your medical record. Lab Results o Displays a list of recent lab results that have been released to your medical record by the provider. Immunization o Displays immunization history from your medical record. Vitals o Displays your vital signs from your last medical visit. 12 P a g e

13 4 Messages 44 Send a Message o Send a referral request or a non-urgent message that will be sent to nursing staff for review. Messages will be responded to within two to three business days. Messaging Center o View messages received from the health care provider s office, as well as review messages you have sent to your medical provider s office. Download My Health o You can download a copy of your electronic health record, also known as a Continuity of Care Document (CCD), to keep at home or to share with a care provider who does not practice in Family Health Centers of San Diego. You can request two formats one that can be shared with a physician or other health care provider outside of Family Health Centers of San Diego and the other is an easyto-read and printer-friendly document for your files. Request Patient Education o You can request a variety of helpful, educational health documents. Once requested, the document will be available for you every time you visit MyHealthRecord via My Patient Education page. You will also receive an in your Messaging Center with a notification that your request has been approved. This message will also include a link to the My Patient Education page. 5 Most Recent Visit Displays information from your most recent visit to FHCSD, including your vital signs, medications, and any additional follow-up as determined by the healthcare provider. 6 Health Recommendations Currently under development. 7 Logout You are encouraged to use this feature every time you are finished using MyHealthRecord. This will help to ensure your health information is kept confidential. 13 P a g e

14 How to Request an Appointment MyHealthRecord makes requesting an appointment simple! You can reach the appointment request screen via the homepage navigation or the Appointment tab located in the upper menu bar. Appointments will be responded to in 2-3 business days Preferred Dates Select the range of dates to schedule an appointment. Preferred Provider This will default to your primary provider. You may also choose the name of the provider you would like to see from the drop down menu, which lists providers you have previously seen. Preferred Clinic Select the clinic where you want to schedule an appointment. I Would Accept: This option provides you flexibility in scheduling appointments with FHCSD providers and clinics. 14 P a g e

15 Selections Include: If seeing your preferred provider is the most important thing, no matter what clinic the provider is working at that day, please select this option: If going to a preferred clinic is the most important thing, no matter whom the provider is, please select this option: To expand on an appointment request, please select this option: 5 Best Number to Contact Please provide a 10-digit telephone number to be reached at. Enter this phone number without dashes or parenthesis. 6 Reason for Visit You can select from one of the drop down options that best describes the reason for your appointment request. Selections Include: New Problem Visit: Select this if you are experiencing a new health concern and would like to meet with a provider, or if you would like certain testing done. 15 P a g e

16 Problem Follow Up: If your provider has asked you to schedule an appointment as a part of follow-up to a previous visit, please use this selection. Physical Exam: Please use this option to schedule a Physical Exam. Routine Women s Exam/PAP: Please use this option to schedule a routine women s exam/pap based on recommendations from your provider Other: To schedule an appointment for a reason not listed, you can select this option and enter more information regarding the appointment in the request details box. Request Details This space is to provide any additional information regarding the appointment request. This space is not intended for listing health concerns. Information shared in this space is viewed by Family Health Centers of San Diego appointment schedulers and will not be relayed to your medical provider. 16 P a g e

17 Submit Remember to click the submit button to ensure your appointment request will be processed. How to Request a Medication Refill There are two ways to request a medication refill. Via the home page: Click on the My Health Information icon and scroll down to the medications section. Via the upper menu tool bar: Select My Health and then choose Medication from the drop down list. From each window, a medication list will populate. Simply click on the medication that you would like to refill. Once you reach this page, please confirm that this is the medication you wish to request a refill for and select submit. 17 P a g e

18 After submitting, you will receive a confirmation of your request. Review and click Yes if you would like to proceed. Click No if you would like to make changes. Once submitted, you will receive a confirmation screen. Simply click OK to be redirected to the homepage. 18 P a g e

19 How to Send a Secure Message MyHealthRecord allows you to submit a secure non-urgent message to your medical provider or a referral request. Messages will be responded to in 2-3 business days and will be reviewed by nursing staff. You can access this function via the home page icon or the Message tab in the upper menu navigation Select Provider This will default to your preferred provider. You may also select from the drop down to choose to message another provider that you have had an appointment with in the past. 2 Referral Request Check this box if you would like to request a referral and in the text box below, please provide referral request details. 3 Request Details This space is designed so that you can ask non-urgent medical questions to your health care provider medical team. You can also use this space to provide additional information regarding a referral request. 19 P a g e

20 Download My Health You can download a copy of your electronic health record, also known as a Continuity of Care Document (CCD), to keep at home or to share with a care provider who does not practice in Family Health Centers of San Diego. Simply: Go to Messages > Download My Health 1. Click the blue button ( ) to begin the download process. 2. Click one of the two formats that can be used to download and save your CCD. The first format can be shared with a physician or other health care provider outside of Family Health Centers of San Diego by copying the entire file onto a portable drive, such as a USB drive. We recommend you use a password protected USB-drive to keep your information secure. The second format is an easy-to-read and printer-friendly document. You can print out this document for your personal files P a g e

21 Patient Education Patient education is available by request through our patient portal. To request patient education: Go to Messages > Request Patient Education 1. Select the documents being requested. 2. Click the [Request] button. 3. Click Ok to confirm the patient education request P a g e

22 To View Patient Education: After you request an education health document, the document will be available for you every time you visit MyHealthRecord. To view this information, Go to My Health > My Patient Education 1. A list of available patient education documents will be visible. 2. Click the patient education link to view the document. 3. This document will open in a new tab and can be saved or printed. You will also receive an in your Messaging Center with a notification that the request has been approved. This message will also include a link to the My Patient Education page P a g e

23 Troubleshooting FAQs What if my information is incorrect? The information on MyHealthRecord comes from your electronic health record at your medical provider s office. Discuss any incorrect information with your healthcare provider at your next visit. I forgot my password or am I am getting the message Invalid Username or Password. If you forgot your password, or are getting that message, it means you need to re-set your password. Please select the Forgot Password link, then: Open your inbox and retrieve the forgot password we sent you Click here to reset your password. Enter in your date of birth, new password, confirm it, and then complete registration When can I expect to hear back from my health care provider if I send a message? Users may access the Patient Portal day or night, however, keep in mind that FHCSD does not monitor the Patient Portal 24 hours per day. FHCSD staff will check the Patient Portal each business day that FHCSD is open. Staff will use its best efforts to respond to all message inquiries between two or three business days. Please remember the Patient Portal is not designed for urgent communications. While it is FHCSD s intent to respond to your inquiries in a timely manner, you are responsible for monitoring whether you have received a response to your communication. If you have not received a response, please contact us by at info@fhcsd.org or call Patient Support Services at Can I view a family member or spouse s medical record through MyHealthRecord? At this time MyHealthRecord does not allow for this feature. You may only view your own individual health information. Can I ask questions for a friend or family member through MyHealthRecord? MyHealthRecord allows access only to your health information. Using your account to ask questions about another person s health could risk the medical care provided to you and allow for incorrect information to be put into your medical Record. Does it cost money to use MyHealthRecord? MyHealthRecord is free for patients of Family Health Centers of San Diego. How will I know when my information is updated? An will be sent to the address that we have on file whenever there is an update to your health information in MyHealthRecord or you receive a new . Why is certain information not shown in MyHealthRecord? Certain test results will not be available through MyHealthRecord and will need to be discussed with your healthcare provider. Please contact your medical provider for additional details or call us at (619) Can I schedule an appointment with, or send a message to, my dentist via MyHealthRecord? At this time, MyHealthRecord is not available for dental appointments, to message dentists or to receive information on dental services. 23 P a g e

24 Can I schedule an appointment with, or send a message to, my mental health providers via MyHealthRecord? Yes, you are able contact mental health providers, schedule mental health appointments and refill medications, if available. Is there a specific internet brower I should use? A browser is a program that allows you to access pages on the World Wide Web. MyHealthRecord was developed to specifically take advantage of features available within Internet Explorer version 8 or 9, or Google Chrome. Is it secure? Is it confidential? Yes! Family Health Centers of San Diego implements a variety of security measures to maintain the safety of your personal information when you request an appointment, referrals, medication refills or otherwise communicate with Family Health Centers of San Diego through MyHealthRecord. Notably, Family Health Centers of San Diego uses a secure server. All supplied information is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our Payment gateway providers database only to be accessible by those authorized with special access rights to such systems, and are required to keep the information confidential. After a transaction, your private information (credit cards, personal health information, etc.) will not be stored on our servers. Why can t I view any information from the FHCSD Reporting in my Yahoo account? We have found that there are some technical glitches with Yahoo! s new Webmail Interface and MyHealthRecord. To view your s, please change your view setting to Classic View by doing the following: Click on Options button in Inbox in the upper left corner Scroll down to and click Options Click on Go Back to Classic Mail Select Go Back to Classic Mail again Now you should be able to see the message Click View full HTML message and click HERE to register Why do I receive the message invalid DOB on my iphone/ipad ios 6 (or) 7 device when accessing MyHealthRecord? iphone settings need to be changed to accept cookies and turn off private browsing mode. To access MyHealthRecord on your iphone/ipad ios 6 (or) 7 device, please do the following: Press Settings Scroll down and Press Safari For ios7: Change Block Cookies to Never Exit out of the settings menu and open Safari > Turn off Private 24 P a g e

25 For ios6: Switch Private Browsing to OFF > Make sure to Accept Cookies It is recommended you choose Keep All for the Close All Tabs? prompt to keep the MyHealthRecord page open Why does the drop down menu boxes disappear before I can make a selection? This may be a browser zoom size issue. Please make sure your browser window zoom level is at 100% by doing the following: Internet Explorer Click on the tools icon Drop down to zoom and change it to 100% (if not already set from the drop down menu). Mozilla Firefox Click on open menu icon Set zoom to 100% (if not already set from the drop down menu) Google Chrome Click on Customize and control Google Chrome Set zoom to 100% (if not already set from the drop down menu) Please help us maintain your security and confidentiality! If you are logged on to MyHealthRecord and you step away without securing or logging off, anyone else who uses that computer will be able to see your personal information. Therefore, it is important to always remember to log off when finished using MyHealthRecord.
