If you forgot your password all is not lost. The password can recovered and, for security reasons, emailed to everyone associated with the UserID/Login. If you are not on the email notification list then you will have to contact HCI so your identity can be verified before a password can be recovered. 1 2 3 4 Check your email for the password
Click on ID to sort by ID#. This will bring ID #0 to the first row making it easier to find and select. Locations & sites you are allowed to view will appear here. Click on Select to select the location you want to view. Selecting ID #0 Corporate Reports ONLY will allow you to view the Corporate Activity Reports (CAR)

Use to change the Year Select the appropriate Date Range (It will default to the most recent date range.) Press this button to continue
This is the main Table of Contents page. Clicking on any menu Item will run that report. For instance, clicking on Recordable Hearing Shift (RHS) -- Employee Name Listing will bring up the following report. Allows audiometric & PFT histories to be printed for any employee in any location SYSTEM menu items
How to Obtain Audiometric & PFT History for Any Employee Regardless of Site Location
How to Select An Single Employee To start the selection process, press Select Employee
To Find an Employee Using the Employee HCI# Enter the 9-digit HCI# here Setup the Search screen as shown above to search by HCI ID#: Enter the 9-digit HCI# Last Name is Blank First Name is Blank Press the blue [SEARCH FOR EMPLOYEE] button
To Find an Employee by Name If you know the exact spelling of the last name set-up the screen as shown above: Enter the EXACT spelling of the last name in the LAST NAME field Select the EXACT MATCH radio button Press the blue [SEARCH FOR EMPLOYEE] button
Someone s name might be misspelled or you may not be sure of the spelling. In this case you can use the LIKE MATCH button. For example, if you want a listing of all employees whose last name STARTS with SU, set-up the screen as shown above: - Employee ID is blank - Last Name has an SU (can be upper or lower case) - First Name is blank - LIKE MATCH is selected - Press the blue [SEARCH FOR EMPLOYEE] button
All employees whose last name starts with a SU will be displayed in the select employee table.
Once you find the employee you want to view, simply press the Select button.
An employee has been selected The selected employee will now appear, as shown above. This employee will be used for both the Audiometric and PFT History reports. You do not need to select this employee again. You only need to select another employee when you want to view a report for a different employee.

How to Print/Export a Report Since many clients are very security minded and many IT departments are locking down computers and refusing to allow ActiveX controls to be downloaded to a user s computer, we implemented a Printing Protocol that does not involve downloading any type of control to the user s computer. The report will first be exported to a PDF file and then the user prints from the resulting output. Essentially, printing is EXPORTING to a PDF file. However, in order for the printing routines to work the following standards must be met: 1. The user must have Adobe Acrobat PDF viewer installed on their computer. This is a free download from www.adobe.com. Most computers come with this software pre-installed. 2. Your Internet browser MUST BE SET-UP TO ALLOW DOWNLOADS FROM www.hcihealth.com.
How to Print/Export a Report Press the PRINT icon to start the print/export process. By default, the report will be saved as a PDF. What happens will depend on your browser, how it is setup and what version of Adobe Reader you have installed.
An Example for Chrome and Adobe 9 PRINT A new window or tab should open with Adobe Reader showing the PDF file. This window can then be used to Print or Save the report PDF.
How to Export a Report to another Format Your report can be exported in a wide variety of formats. Press Arrow near Adobe Acrobat and select the export format you want and then press the DISK (export) icon to start the export process.
In this example the Excel 2007 Export Type has been selected.

How to Change Your Contact Information.
How to Change Your Email Address and Contact Information for Report Processing Notifications. Selecting this item will allow you to change your email address and/or contact information associated with your login.
Pressing the EDIT selection will allow you to change the contact and email information. Current contact and email address
After updating/saving your changes, pressing this button will return you to the Reports Selection screen. Make sure you select UPDATE to SAVE your changes. (Cancel will cancel any changes you made.) After selecting EDIT you can now change the contact name and email address.
How to Change Your Password
Selecting this item will allow you to change the password associated with your login. REMEMBER THIS CHANGES IT FOR EVERYONE ASSOCIATED WITH THE LOGIN. DO NOT USE PERSONAL PASSWORDS.
Enter your old (current) password and your new password 2 times (New Password & New Password (verify)) then Press the Submit button.
Possible Error Messages
This is the screen you will see if the password was successfully changed: 1. Changed Password is in the message area 2. Submit button grayed out and not selectable 3. Press to go back to previous screen.
Exiting the Reporting System When you are finished, please select Log Off to exit.
