a) CONDITIONS FOR GRANTING, MAINTAINING, EXTENDING, REDUCING, SUSPENDING AND WITHDRAWING CERTIFICATES Policy The Management of the Certification Body undertakes to provide ongoing clarification of the conditions for granting, maintenance and revoking of certificates to rationalize the relevant procedures defining its approach to clients. Conditions and procedure for granting the certificates are defined in the Certification procedure (and Audit report. The Information for clients informs the clients about the conditions for certification, the procedures, initial assessment of the management system, the recertification. The client can review QM on request. Conditions and procedure for maintenance of certificates are defined in Certification procedure and Report of surveillance audit. Certified organization confirms in the Confirmation of the receipt of the certificate and in the contract its obligation to inform CB about any changes in its management system. Repeated assessment after major changes at the client Major changes at the client s organization which can influence its management system, or a finding that the certified customer failed to meet the requirements of the certification body, require repeated assessment of client s management system by the means of extraordinary surveillance (e.g. in cases of change of ownership, employees, or equipment used by the client) Conditions and procedure for restriction or expansion of the certificates are defined in the Procedure for change of scope of the certification. certification decision date), surveillance audits shall be conducted at least once a year The Client announced to CB (via official letter) the termination of cooperation The Client did not remove the major nonconformities observed during an audit The Client employs in practice different procedures than prescribed in his documentation nonfunction of system or some of element of management system Procedure of suspension of the certificate: If any Member Body discovers any reasons for suspension of the certificate, it informs CMC. On the base of CMC decision, secretary elaborates a draft of suspension, which can then be confirmed by the Manager of Certification Body. The Certification Body gives the Applicant sufficient time, three months from date of suspension maximum, during which the Applicant shall prove that the observed insufficiencies have been removed. In the opposite case the Certification Body shall propose suspension of the Certificate. The procedure of revoking the certificate: The Secretary shall prepare a draft of the proposal for revoking of the Certificate on the basis of CMC recommendation; the proposal shall be approved by the Manager of Certification body. The secretary removes the client from the list of certified organizations. Cancellation of certificate CB secretary cancels the certificate if a certified customer ceased to exist or granting of new certificate (e.g. when the scope of certification changes). The secretary registers the changes to the list of certified organizations. REPORTING OF CHANGES IN A CERTIFIED ORGANIZATION The client is obliged to progressively inform the certification body of all changes which have an influence on the management system following the issue of the certificate, throughout the duration of its validity (e.g. changes relating to legal, business, organisational status or ownership; to organisation and management; changes of contact address and contacts to workplace; significant changes in the system of management and processes). The Manager of the Certification Body assesses the changes in the certified organization and based on the documentation submitted by the organization (and possibly requested additional documentation) decides whether an extraordinary surveillance audit is necessary or the certification can be transferred to new organization without an audit. APPEALS, COMPLAINTS AND DISPUTE Policy for resolution of appeals, complaints and disputes The Manager of the Certification Body undertakes to resolve all appeals, complaints and disputes in a constructive and fast way, in line with documented rules and mandatory rules. The goal is to resolve all issues in a rational way; all concerned parties are immediately introduced to the respective procedures and records of resolution of the cases. An complaint is the expression of the client's disagreement with procedures used by the Certification Body. An appeal is the expression of disagreement with the decision of the Certification Body. The client/interested party shall address any complaint expressed: during a Management System audit to the Lead Auditor who is obliged to solve it within his competence. If LA is not able to resolve complaint to the customer satisfaction, he shall record the situation into the audit documents and calls secretariat`s attention to this problem. Secretariat starts the procedure of complaint (see the next point). The client may address his complaints via correspondence or e-mail also to the Secretariat. The Secretary shall file the complaints, confirms acceptation of the complaint within 5 working days and shall pass them on to the Manager of CB. During solution of complaints Manager of the Certification Body informs the client/interested part via e-mail or post on the subject of the complaint. The Quality Manager shall use the Complaint Survey to initiate the corrective measures and to assess the efficiency of corrective measures. If the client/interested party does not agree with the conclusions of the Manager of CB, he may appeal against the results of the audit within 14 days from the receipt of the Report to the Secretariat or directly to the Manager of Certification Body via correspondence. The Secretary shall file the appeal, confirms acceptation of the appeal within 5 working days The Manager of CB submits this appeal to the CMC (Coordination and Methodology Commission) together with all necessary documents. The CMC must be organized within 40 days from the date of appeal acceptation. The solution proposed by the CMC shall be accepted. The client must be informed within 14 days from the meeting of CMC. Secretary maintains and file the written materials concerning the complaints (appeals) against the deviations from the procedures described in the CB rules and the appeal against the decisions for three years). The Manager of Certification Body shall pass the materials concerning complaints and appeals and their solutions to the Board for Certification during its meetings. Certification Body accepts and evaluates any signals from the customers to initiate improvements of Quality System through corrective and preventive measures. SURVEILLANCE OVER ORGANIZATIONS General Regular surveillance over the certified organization shall be carried out at least once a year (the first surveillance audit following initial certification is carried out at latest 12 months from the certification decision date); in the case of changes in the Management System of the client which require inspection, an extraordinary surveillance audit may be organized. The scope of the surveillance audit The scope of a routine audit is approximately one third of the scope of a certification audit. During any surveillance audit the audit team must at least: to verify that the points of ISO 9001 are met for QMS: Context of the organization, Responsibilities of management/leadership, Operation, Design, Measurement, analysis, improvement/evaluation to verify that the points 5, 7.3, 7.5, 8 of EN ISO 9001 and EN ISO 13485 are met for QMS to verify a review of action taken for notification of adverse events, advisory notice, and recalls for EN ISO 13485 to verify that the points of ISO 14001 are met for EMS: Context of the organization, Planning, Operation, Management review/evaluation to verify that the points 4.2, 4.3, 4.4.2, 4.4.3, 4.4.4, 4.4.5, 4.5.2, 4.5.4, 4.6 of OHSAS 18001 are met for OH&S to verify that the points 4, 5, 6, 7 of ISO/IEC 27001 are met for ISMS to verify that minor nonconformities from last audit had been solved and removed to check the changes in the system since the last visit to check the customer complaints and their solutions to check the use of the certificate and the certificate mark Vseobecne_informace o_certifikaci_angl.doc 05/16

NEW ASSESSMENT - RECERTIFICATION Each three years, performs full assessment (all points of the standard or its changes) of the whole client s management system, which was certified. The recertification procedure is identical with the certification procedure. It is executed mainly in one stage. Application with Introductory questionnaire is not required. RIGHTS AND OBLIGATIONS OF HOLDERS OF CERTIFICATES The rights of the holders of certificates Certificate holders have the right to: inform potential customers or suppliers of all or partial information about certification use the certification mark and IQNet in promotional materials and documentation according to the criteria of use of these marks (see D-14-03) Copies of certificates The certified organizations may copy certificates (even in enlarged or reduced size) on condition that full contents of the original shall remain legible. Incorrect use of certificates checks the correct use of certificates for its customers. DECLARATION OF IMPARTIALITY OF Management of indicates commitment to ensure the impartiality of its certification activities and to keep all possible threats and vulnerabilities of actual or possible conflicts of interest which could influence certification and surveillance activities of under control. Management ensures coordination and management of employees and auditors and their activities within certification process. Impartiality of CB has been monitored by Coordination and methodical commission which continually reviews risks connected with possible conflict of interest, makes decisions about way of realization of each business case and supervises the correctness of realization and it realises risk analyse according to actual requirement. CB, as its only activity is certification of management systems, does not any consultancy activity. Correctness and impartiality of the procedures of CB is monitored by independent The Board for certification, which resolves even prospective claims. Division of responsibility and authority of the leading auditor and the manager of the certification body (which decides about the result of the certification based on a review of all documents submitted by the leading auditor and the customer) ensure that all decisions concerning certification are unbiased as well. Unbiased performance of audits is ensured by selection of auditors who were never working for the company being certified on a commercial basis. Economic independence is ensured by the fact that the certification body and its auditors are not economically dependent on the findings. 14. 03. 2016 Ing.
