Note: To shut down the device, please hold the Power button for 3 seconds and release. Holding for more than 3 seconds should only be done when directed by Med X Change Technical Support. Wireless Connectivity to EvolutionHD Device 1. Establish Username and Password a. Plug standard keyboard into USB port on front or back of EvolutionHD box b. Upon initial startup, you will be prompted for a username and password. Enter the following and then hit Sign In : Username: admin Password: admin IMPORTANT: EvolutionHD is configured out-of-the-box with two logins: admin and anonymous. The password for both of these logins is the same as the username until manually changed by the user. c. From EvolutionHD box homescreen, use arrow buttons to navigate to Settings icon. Press d. Navigate to the Security icon and press e. Press again f. Create a unique username and password by typing into the onscreen fields g. If you would like this user to have Admin privileges, select the Admin field h. Use arrow buttons until you see Save button at bottom of screen. Select Save and press i. You ve now created the username and password for wirelessly connecting to the EvolutionHD box from the MedXStream mobile app 2. Download MedXStream app to your mobile device a. For Apple ipad or iphone users, download free MedXStream app. Search: medxstream in the Apple itunes Store b. For Google Android phone or tablet users, download free MedXStream app. Search: medxstream in Google Play Store g. On next screen, skip the I.P. Address and Display Name fields. In next two fields, enter the same username and password you initially created in the Setup Username and Password step above h. Swipe Remember Password toggle to On. Then, tap Connect i. Tap Home button located in upper-left corner of the app to return to app homescreen NOTE: After initial set-up, you won t be prompted for password again. However, if your mobile device moves out of local DRS network range, you may need to reselect DRS network again once mobile device is again in range of network. TIP: If, after reconnecting to the DRS network, you receive the message Error: You have not connected to any device yet, perform the following steps: 1. Power off the EvolutionHD box and then power back on. Ensure the box has fully booted up before performing any subsequent steps 2. Ensure your mobile device is connected to the DRS Wi-Fi network 3. Close and re-launch MedXStream app. Tap Select Other Video Recorder 4. Tap Scan located in upper right-hand corner of screen. If more than one I.P. address appears, tap the one that says Online next to it 5. On the following screen, enter the same username and password you initially created in the Setup Username and Password step above 6. Swipe Remember Password toggle to On. Then, tap Connect 7. You have now successfully resynced your mobile device to the EvolutionHD box 3. Connect Mobile Device to EvolutionHD Wi-Fi a. Ensure Wi-Fi is enabled on your mobile device b. Find the wireless router default SSID and password located on the underside of the ZyXEL router on a label c. Go to Settings on your mobile device. Tap Wi-Fi, and then select the network that matches the SSID on the underside of the router d. When prompted for a password, enter the password located on the underside of the router. Mobile device is now connected to the EVO-HD wireless hotspot e. Launch MedXStream app on your mobile device and tap Select Other Video Recorder f. Tap Scan located in upper right-hand corner of screen. Tap on the I.P. address that appears 3

Create New Physician Profile 1. From EvolutionHD box homescreen, use arrow buttons to navigate to Settings icon. Press 2. Use arrow buttons to navigate to Physician icon. Press 3. Press the again to Add a new Physician 4. Plug standard keyboard into USB port on front or back of EvolutionHD box. Using keyboard, type physician name into Full Name field Option A: Sync physician with standard global settings 5. To sync new physician profile with the global settings, use arrow buttons to select Sync to Global Settings and press Enter button to add checkmark. 6. Use arrow buttons to select Save and press Enter. Press Enter again to exit to list of physicians Option B: Customize physician settings 5. To create unique settings for this physician, do NOT check Sync to Global Settings. Use arrow buttons to select Save. Press Enter button and then press Toolbar/Back button. 6. A series of icons will now appear along the bottom of the screen. Use arrow buttons to highlight the specific topic you wish to customize for this physician and press. 7. On the next screen, configure the settings for this topic according to the physician s preferences. Be sure to save your work. 8. To customize settings for additional topics (or to change existing profile settings), follow steps 1-2 above. When you arrive at the list of physician profiles, use arrow buttons to select the desired physician profile and press. 9. Press Toolbar/Back button and follow steps 6-7 above 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow steps above (skipping Step 4) NOTE: When prompted to type in physician name, use keyboard found in mobile app by tapping on Keyboard icon. When finished typing, tap Enter icon to exit keyboard function Create New Patient Profiles 1. From EvolutionHD box homescreen, use arrow buttons to navigate to New Case icon. Press. Press again to select New Case 2. Plug standard keyboard into USB port on front or back of EvolutionHD box 3. Using keyboard: a) Select the physician scheduled to perform the patient s procedure by navigating to Performing Physician field and hitting. If physician isn t listed, enter physician name by selecting New button to right of Performing Physician field and hitting. Follow prompts on next screen b) Enter patient information in remaining fields (First Name, Last Name, Patient ID) c) Use arrow buttons to navigate to bottom of screen. Select Save to save patient profile, or Save and Start to save profile and immediately start case. Then press 4. To view patient profiles as a Work List: From EvolutionHD box homescreen, use arrow buttons to navigate to New Case icon. Press. Pre-entered patient profiles now appear. Use arrow buttons to navigate to any profile. Press Enter button to access, edit and/or start case * 1. Launch MedXStream. From app homescreen, tap Connect to Last Video Recorder. If error message appears, click OK remotely from your mobile device. Follow steps 1-4 above (skipping Step 2). NOTE: When prompted to type in patient information, use keyboard found in mobile app by tapping on Keyboard icon. When finished typing, tap Enter icon to exit keyboard function. * The MedXStream app also enables you to create and view patient profiles using a Worklist feature only available through the app. To do this: 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK 2. Tap Worklist icon. Pre-existing patient profiles will appear if they ve been previously entered into the system. Tap any profile to access or edit it. 3. To enter a new patient profile, tap the + button located in the top, right-hand corner of the Worklist screen 4. Complete all fields and tap Save NOTE: To delete a patient profile from this list, swipe left on the profile and tap Delete. Deleting patient profiles from the mobile app also deletes them from the EvolutionHD box 4

Set and Customize Printing Preferences 1. From EvolutionHD box homescreen, use arrow buttons to navigate to Settings icon. Press 2. Use arrow buttons to navigate to Printing icon. Press 3. Use arrow buttons to select desired printing settings within each field 4. (Optional) To include a custom logo on each printed page: Insert a USB memory stick into EvolutionHD box with your logo file saved as Logo.jpg on the root directory. When you arrive at the Logo field within the printing settings in Step #3, ensure box next to the word Enabled is checked. Next, press down arrow button to select the MedExchange logo field and hit. Use arrows buttons to select your custom logo from drop-down menu and hit. Your custom logo is now the default logo for printed pages. NOTE: Do NOT remove USB device from EvolutionHD box until you ve saved your preferences (see Step 8). 5. (Optional) To include a custom header on each printed page: Plug standard keyboard into USB port on front or back of EvolutionHD box. Using keyboard, type custom header text into Header field 6. (Optional) To include a footer on each printed page: Use arrow buttons to highlight Footer 1 field, press, and select from the list of footer options. Up to six footers may be added to each printed page 7. To test your printer settings, use arrow buttons to navigate to Print Test at bottom of screen. Press. 8. Use arrow buttons to select Save button at bottom of screen. Press 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow steps 1-8 above NOTE: When including a custom header on each printed page (Step 5), use keyboard found in mobile app by tapping on Keyboard icon. When finished typing, tap Enter icon to exit keyboard function. Start New Case 1. From EvolutionHD box homescreen, use arrow buttons to select New Case icon. Press 2. Pre-entered patient profiles now appear. Use arrow buttons to select any profile to access and press. Select Save and Start button located at top or bottom of screen and press to start case NOTE: Once you start a case, it will disappear from your existing Work List view on the EvolutionHD box and MedXStream app. TIP: Session Timeout is set to a default of 30 minutes of inactivity. After the session timeout is reached, you will be unable to access the home screen of the EvolutionHD box until the user s password is entered. You may continue to view the live screen and capture images and video once session timeout has been reached. This is designed to protect patient data. The Session Timeout can be disabled or changed by any user with admin privileges by navigating to Settings and then System Settings. 1. Launch MedXStream app. From app homescreen, and tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow steps 1-2 above Close Case NOTE: Cases cannot be closed unless they have already started 1. From EvolutionHD box homescreen, use arrow buttons to select Close Case icon. Press. The case is now closed and is archived within the system 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow Step 1 above 5

Review Previous Case 1. From EvolutionHD box homescreen, use arrow buttons to select Review icon. Press 2. A chronological list of past cases will appear. Use arrow buttons to highlight desired case. Press to access image and video thumbnails captured during that case. Use arrow buttons to select desired image or video file. Press. TIP: To return to thumbnail view, use arrow buttons to select Done and press. (Optional Label Image or Video file) 3. Once specific image or video file is selected and appears on the EvolutionHD box screen, use arrow buttons until cursor appears in text field at bottom of screen. Plug standard keyboard into USB port on front or back of EvolutionHD box and use to type in desired file label. Once complete, use arrows buttons to select Done and press. (Optional Delete Image or Video file) 3. Once specific image or video file is selected and appears on the EvolutionHD box screen, use arrow buttons to highlight Delete. Press. Confirm Yes by pressing Enter button again (Optional Print Image or Video still file) 3. Once specific image or video file is selected and appears on the EvolutionHD box screen, use arrow buttons to highlight Select. Press. Use arrow buttons to select Done and press. A blue checkmark now appears on the file in the thumbnail view. Repeat for all images/video stills you wish to print 4. Once complete, return to thumbnail view. Press Toolbar/Back button and then use arrow buttons to select Print. Hit 5. Confirm printer settings on next screen. Use arrow buttons to select Print and then press 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow steps 1-5 above NOTE: When labeling an image or video file (Step 3), use keyboard found in mobile app by tapping on Keyboard icon. When finished typing, tap Enter icon to exit keyboard function. Offload Images/Videos to Storage Device 1. Insert high speed USB drive into the front USB port on the EvolutionHD box. IMPORTANT: Each still image takes up approximately 3MB (megabytes) of space. One hour of HD video takes up 12GB (gigabytes) of space. 1000GB = 1T (terabytes). Be sure your USB drive has enough storage space. High speed USB 3.0 drives are the fastest. 2. To save time, insert USB drive prior to the case starting. Images and video files will automatically save to the USB drive during the case. Wait until case completes before removing device 3. To offload images and/or video post-case: From EvolutionHD box homescreen, use arrow buttons to select Review icon. Press 4. A chronological list of past cases will appear. Use arrow buttons to highlight case you want copied to USB and double click the Enter button. Repeat for all desired cases. 5. Press Toolbar/Back button and then use arrow buttons to select Copy. Hit 6. A pop-up window will appear detailing how large a file you are saving to the USB drive and how much total storage space is on the USB drive. Use arrow buttons to select Copy and press Enter button to begin copying. IMPORTANT: The Copy screen has an optional feature to Include Patient Data. When selected, Patient Study Information will be included in a patient text file contained in a secure, encrypted zip file. Viewing the patient data text file will require the login password of the user who created the file. You may still view the exported images and video without entering a password. 7. A progress bar will appear on the next screen. When copying is complete, a list of the copied files will appear. 8. Press and then the Home button to be returned to the EvolutionHD homescreen 9. The USB drive can now be safely removed 1. Launch MedXStream app. From app homescreen, tap Connect to Last Video Recorder. If error message appears, tap OK remotely from your mobile device. Follow steps 1-9 above 6

Create and Send Custom Reports (available via MedXStream app only) 1. Develop Coversheet for Patient a. Launch MedXStream app and tap Report Configuration b. Tap Coversheets and then the + symbol in the upper righthand corner c. A new screen will appear. Tap Generate in the lower righthand corner, and then select From template d. Enter desired information into each field by tapping on the field e. When finished, tap Save in the upper right-hand corner f. A screenshot of your newly created coversheet will appear. To edit any section, tap on the desired section and follow the prompts TIP: Information can be added to coversheets such as text, physician signature and more to create a custom patient letter or report. g. (Optional) To add a physician signature, tap the edit icon located just to the left of the Done button in the upper righthand corner. Next, tap the pen icon and using your finger, create a custom signature at the bottom of the page. Other icons in this field can be used to further customize the signature. Once done, tap the circled X icon in this field and then tap Done. If unsatisfied with signature, tap signature to highlight with blue box, then tap trash icon to delete. Tap edit icon again to access pen icon and retry signature h. When finished editing, tap Done in the upper right-hand corner i. Create a name for this custom coversheet by tapping next to the word Name and entering text. When finished, tap Save in the upper right-hand corner. The Title for the template you just created will now appear on the Coversheets sub-menu screen j. To modify a previously created coversheet, tap on the coversheet you wish to modify from the list of available coversheets. Then tap Modify. Tap anywhere in the coversheet to modify and follow the prompts. When finished, tap Done, then Save on the next screen 2. (Optional) Annotate Images/Videos a. Launch MedXStream app and tap Cases b. Tap desired case from list. The next screen displays thumbnails of all images and videos captured during that case. Tap on the file you want to annotate. c. On the next screen, tap the Upload icon in the upper righthand corner and then tap Edit d. Using the drawing and annotation features in the top of the screen, you can now insert text, draw arrows and shapes, as well as customize colors and font size e. When satisfied, tap the circled X icon in this field and tap Save in the upper right-hand corner f. The next screen displays your customized work. To edit further, follow steps c - e again. If satisfied, tap the blue back arrow in the upper left-hand corner to return to the thumbnail view of all images and videos captured during that case 3. Embed Images/Videos in Custom Report a. Launch MedXStream app and tap Cases b. Tap desired case from list. The next screen displays thumbnails of all images and videos captured during that case. Tap Select in the upper right-hand corner and then tap each file you want included in the custom report. These can include any images or videos you may have annotated c. Tap Done and then Report d. On the next screen, tap Coversheet and tap the desired coversheet template e. The selected images will automatically be embedded into the report f. To further customize the report, click any field and follow the prompts g. When the Report is complete, tap Done. Your custom report, including coversheet and embedded files, is now visible (Optional) Embed Physician Voice File in Custom Report h. To embed a voice file in this custom report, tap the Edit icon located in the upper right-hand corner. The drawing and annotation features will appear at the top of the screen, including the Microphone icon (if microphone icon doesn t appear, tap the icon). i. Tap Microphone icon. If pop-up menu appears asking to access the internal microphone, tap Allow j. Tap the location on the screen where you wish to embed the voice file k. To record audio, tap the red dot and begin speaking. When finished, tap the blue dot. To play back, tap the blue triangle button on the left side of the voice file. To delete, tap the circled megaphone icon and then tap the trash can icon. l. When satisfied with voice recording, tap the circled X icon 4. Print, email or open report m. Tap upload icon in upper right-hand corner. Tap Email to email report NOTE: Email will not send until your mobile device is out of range of EvolutionHD wireless router or mobile device is disconnected from DRS Wi-Fi network Tap Print to print report Tap Open In to open report in a different application IMPORTANT: MedXStream app does not save copies of custom reports. The physician and facility should determine the appropriate archiving method that best meets their needs. 7
