1 Follow My Health How To s IF you want to Request an appointment: THEN 1. From your Home screen, click Request Appt or Request (See example below.) NOTE: If you are unable to request an appointment as desired, please call 503 399 2424 for assistance. 2. Select the provider with which you wish to schedule an appointment from the drop down menu. 3. Enter your reason for visit and select your available times. When finished with all comments, click Submit. (See example below) Request a referral to another provider: 1. From the My Info tab, click the drop down arrow. 2. Select Providers from the drop down menu. 3. Click the drop down arrow next to What do you want to do? and then select Request a Referral. 4. Select your primary care provider.
2 5. In the Referred To box, type the provider s full name and location that you wish to be referred to. In the Reason for Referral box, state why you want to be referred and add any additional information in the Comments box. The date will default to today. You will need to select the next day, for example it would be the 28th as shown in the example below. When finished, click Send. (See example below) Request a medication refill: 1. Click the My Health tab. 2. Click the Medications tab. 3. Click on the illuminated pill bottle for the medication you wish to be refilled. (See example below.) 4. In the popup window, you may change your pharmacy if needed, enter comments and when finished, click Send. (See example below).
3 Send a message to your provider: VERY IMPORTANT NOTE: If you are experiencing any symptoms that require medical attention, please contact the clinic at 503 399 2424. The patient portal should not be considered a means of seeking medical care for current symptoms. 1. Click the Inbox tab. (Or you may do so from the top of your screen as shown below). 2. From the Inbox, click Compose. The Emergency Message Disclaimer will pop up. After reading the message, click OK. 3. Fill in the Subject and the Body of the message. When finished, click Send. (See example below) Add another Authentication Method: (Facebook, Google, Yahoo, or LiveId) 1. From the top right side of your screen, click My Account and from the dropdown menu, select Preferences. 2. Under the section titled Login Preferences, choose your other desired logins. Only one username/password of each type is allowed. (See example below).
4 3. Click Connect for your desired connection. Example shown below is for a Yahoo login. Enter your email addres s and password for that account. Click Sign In and your authentication method has been added. Delete my account permanently: 2. From the drop down menu, click Preferences. 3. Under the section titled Account Preferences, click the Delete you UHR button. (See example above). 4. Click Yes when responding to the Warning. See example below. Update your health information: Please contact the Salem Clinic directly (503 399 2424), as information that you enter into Follow My Health WILL NOT be viewable to your provider.
5 Request support for the patient portal: 2. Select Support from the drop down menu. (See example below). Change your forwarding email address to receive updates: 2. Select Preferences from the drop down menu. (See example below). 3. Under the Notification Preferences section, in the box under the Contact Information and Email Address, type in your new email address for which you would like your updates forwarded to. 4. When finished, click Save Changes. (See example as shown below). Turn off every message that is an update to your FMH record: NOTE: You may still receive appointment reminders, etc. 2. Select Preferences from the drop down menu. (See example below). 3. In the section which states Updates to Health Record, uncheck the box next to Email and/or Text Message. 4. When finished, click Save Changes. Make a payment Currently we are not accepting payments through Follow My Health. Contact Salem Clinic directly at 503 399 2424.
6 View your Clinical Summary for your visit: NOTE: You will see your provider s treatment plan during a visit. 1. Click the My Health tab. 2. Click the Documents tab. 3. Search for the Clinical Summary document. Once found, under the Options section, click the document with the magnifiying glass. (See example below). The document will display. Print your results : 1. Click the tab My Health. 2. Click the tab Results. 3. To print ALL results, simply click Print. 4. To print individual results, click the document with the magnifying glass next to the result. (See example below). Update my personal information 1. Click the My Info tab. 2. Select which tab to update information. 3. Edit in section desired. When finished, click Save. (See example below).
