The List of Required Application Documents 2017/18 Page 1 of 6 APPENDIX No. 2 To the CEMS MIM Selection Rules & Regulations for 2017/2018 academic year (2017 Selection) THE LIST OF REQUIRED APPLICATION DOCUMENTS AND GUIDELINES FOR ON-LINE REGISTRATION Before filing In the application form and other documents, please read carefully the Selection Process Regulations available on-line at. www.sgh.waw.pl/cems/ (Last update: 31 October 2016) Application form is submitted on-line and available at CPM website at: http://sgh.waw.pl/rekrutacjawymiana/. Along with that, all candidates MUST REGISTER as candidates for selection at CEMS data base available at: http://www.cems.org/selection/. Along with application on-line, all candidates must submit the following documents: Letter of Motivation (in English) signed in person (unless submitted online)! CV in English according the following format: - Name and Surname - Date and Place of Birth - Education - Achievements (distinctions) - Activities (research, organisational) - Work experience - Languages spoken - Interest and Hobbies - Additional Information Photocopies/Scans of language competence certificates and other relevant confirmations of your proficiency as indicated in on-line application. Master s students of other than eligible majors, who are not graduates of SGH bachelor s studies, but completed studies in economic and/or management in other university must submit in addition a copy of their BA or MA studies diploma confirming the above. 2 nd or 3 rd semester students must submit in addition: Certificate/Committment stating that the student is or will be on leave for one (or two semesters, in case of student who already completed 2 nd semester of studies at SGH) in the academic year 2016/2017 All enclosures placed in a separate punched document pocket (koszulka na dokumenty) with front page (according to the sample presented in Appendix 5) must be dropped into the collection box standing in front of the room 14, Building A no later than on 23 November 2016 by 12:00 hours. Incomplete applications (or submitted after deadline) will not be accepted for evaluation. For your safety, ALL above mentioned documents should be uploaded to the database just in case you miss the deadline for submission of paper documents, or when personal submission is not possible (stay abroad). NOTICE TO SGH STUDNETS: Do not use WSE as school abbreviation. It is incorrect. Our acronym is SGH, while WSE stands for Warsaw Stock Exchange. It is also the abbreviation for some schools of economics in Poland

SGH ON-LINE APPLICATION Log in to the system using your SGH password (akson) Your personal data will upload automatically. Knowledge of English Please choose: Mothertongue if you are citizen of English speaking country or may prove you are a native speaker of English (please observe the specific requirements for the second foreign language applied to native speakers of English). Please submit a scan of your passport or other document confirming the above. University degree if you are graduate of degree programmes that were conducted in English at CEMS member university, or EQUIS/AACSB accredited university, or in any university in the English-speaking country (please provide us with a scan of your diploma and its supplement confirming that the studies were conducted in English). Certificate if you qualify for CEMS based on one of the CEMS accepted certificates (provide us with a scan of the certificate or the internet printscreen of the on-line result) o Certificate: select the one that applies to you o Certificate Level: choose the correct one (in case of TOEFL you need to choose also the exact number of points within the given type of exam) o Awaiting result: choose this option if you have taken the exam, but you are awaiting test result. Please remember to submit the result (at least its on-line result printscreen) to CEMS Programme Manager at SGH by 4 January 2016. In case of discrepancy, please contact the Dean s Office and submit with application a document confirming your current semester. Deans leave 2 nd or 3 rd Semester students only! Please tick if you are on dean s leave, or you plan to apply for the leave in case you are admitted to the CEMS MIM Programme (confirmation of dean s leave or your commitment letter to be attached) Faculty Please choose your major of study at SGH Participation in the exchange programme Please choose, if you are/were participating in any exchanges in the past Language Proficiency (Second Foreign Language) Language: choose the language Proficiency level: Choose the proficiency level as indicated on your certificate or choose options for university degree/mothertongue/secondary education if applies to you. The level of the certificate should be consistent with the clauses and Table in the Appendix 1 to the Rules and Regulations. If you certificate is not on the list provided in the Appendix 1, please choose the level according to your best knowledge. Files Upload files with language certificates/diplomas, CV, letter of motivation, dean s leave certificates or commitment form (pdf is a preferred format). University Selection Please select schools according to your preference, where University 1 is the most preferred and University 29 is the least preferred (you may choose less schools). Please ignore schools which you are not interested in, it means the school you will not accept if assigned (for instance, due to high cost of travel/living, you will not be able to bear). SAVE that part and logout or browse again previous parts. Remember to save the part in which you make any changes!

The List of Required Application Documents 2017/18 Page 4 of 6 REVISITING THE APPLICATION If you decide to visit your application again (to check if everything is saved correctly or to change some data), please remember to save changes! If you have problems with application, please contact the webmaster of the application. If you are not sure or see that some data, certificates, uploads etc. are not visible in your application, please send an email with a list of problems to CEMS Programme Manager at SGH. If you have problem with uploads, remember, that you have to submit them in paper to the collection box located in front of room 14, building A (ground floor). 2. CEMS SELECTION APPLICATION All candidates, must also register their candidatures in the CEMS MIM Selection Application the following way: Please get to the application website available at: http://www.cems.org/selection/ and make initial registration to get your personal password that is valid within 24-hours after that registration. Please enter as Student ID your SGH 5-digit ID enrolment number ( numer albumu ) just digits!!! Do not enter Polish or other language specific characters. They are visible why you type them, but unfortunately get corrupted when doing any import to Excel file. In order to use such file we have to correct your data manually! Please login to the system as indicated in the confirmation email and fill all the fields in the application in the following format: 1) Name Please enter your Surname in CAPITAL LETTERS 2) First name Please enter your first name in standard format (no capital letters) 3) Date of birth (according to pre-defined format) 4) Place of birth city 5) Country of birth (choose) 6) Nationality (choose) 7) Current Address (Polish format: you may replace m. with / if your house number has no slash) 8) Permanent Address (as above, if you stay outside your home address) 9) Telephone/Mobile: Please use the following format (without spaces) : +<Country code><number including area code if applied> eg. +48123456789 (mobile), +48229990909 (telephone) 10) Email: usually your private email, unless you SGH email is your primary mailbox. 11) Selection year: Please enter 2017 12) Class year: 2017/2018 13) Graduation year: Please enter 2018 (it is the earliest possible graduation year for you) 14) Specialisation: choose correct one 15) Bachelor degree - name of degree: BA, BSc etc. followed by the specialisation; eg. BA in Management) Note: all SGH degrees are BA 16) Bachelor degree School: School name in English 17) Choice of schools (please enter in the same order as in SGH on-line system in case of differences, the assignment is based on the sequence entered in the SGH system) 18) GMAT not required, but if you have the result, you may enter it.

TOEFL enter the number of points obtained b. IELTS, CAE, BEC tick appropriate c. CPE if you have just letter grade (no points on certificate) enter the following values: i. Grade A: 80 ii. Grade B: 75 iii. Grade C: 60 iv. C1 Certificate (Grade D): 55 d. Others i. Tick Mothertongue if English is your mothertongue (citizenship, nationality) ii. Tick University degree if you validate your English on the basis of university degree completed in English (in CEMS school, AACSB/EQUIS accredited school or in English-speaking country) iii. Accredited school course (only if you have certificate of CEMS accredited cultural institute) 20) Other Languages: a. Language 2 i. Enter your mothertongue as Language 2 if it is one of the CEMS languages, (unless it is English) and select mothertongue in the Language Type field. ii. Enter other CEMS language, if you mothertongue is not one of the CEMS languages and select appropriate Language Type (see point 21.) b. Language 3: Enter your second foreign language or mothertongue, depending on your input in Language 2 field. c. Language 4 and more: if you possess documented proficiency in other languages, feel free to enter them. 21) Language Type a. Mothertongue if entered language is your mothertongue (base on your nationality, citizenship or place of birth). You may be asked to sign a special Statement of Honour (in that language) if there are doubts concerning your proficiency level. It is a tool to verify it. b. Secondary education choose that option only if you confirmed your proficiency on the basis of secondary education certificate completed abroad (outside country of your residence/citizenship), or you have submitted official document releasing you from taking language tests in the given language due to completing bilingual secondary school. c. University degree choose that option if you have university degree completed in that language (language of instruction of all courses except tutorials) or you are graduate of language studies (philology, applied lingusistics) d. None leave that field blank if none of the above refers to your case (eg. SGH competence test, CNJO certificate and other certificates these will be added to the database, once you are admitted to CEMS MIM programme by CEMS Programme Manager, on the basis of submitted documents) 22) Document Upload: Please upload your CV, Letter of Motivation and Language proficiency certificates in the application in pdf format. Remember, that despite that, you have to submit them (along with additional documents) also in paper as described in Part 1 of this Appendix! Uploading on-line is just for security reasons, or if you for any reason you will not be able to submit docs in paper!

The system may get overloaded and your data may not be saved! o Once you complete the process of application or make any changes to your application, please login again and check whether the data you have entered have been saved! o If you have doubts whether your certificate/diploma will be accepted or regulations seem not to be clear enough for you contact CEMS Programme Manager at SGH o If you encounter any problems, please contact our webmaster. o If you are afraid you will not be able to submit additional paper documents, please scan them and send by email to august@sgh.waw.pl or send to that email address a notice stating the reason for possible delay. In case of a delay, you will be informed whether we will accept your application (not guaranteed) and under which conditions.
