Getting Started: Login MyUFHealth, also referred to as MyChart, is an exciting new way for you to communicate with your University of Florida Health provider and to review information about your healthcare. MyUFHealth uses a secure electronic web portal protected by your own personal ID and password to ensure the privacy of your information. To make sure your information remains confidential, do not share your ID and password with others. Log in to MyUFHealth here: http://ufhealth.org/mychart Enter your MyUFHealth Username and Password. Click the Sign In button. If signing in for the first time, New Users will be asked to accept the Terms and Conditions. To log out, click the Log Out button located in the upper right corner of your screen. 3

Home Page & Basic Navigation The Home page has important information under the You Might Want To section. You can access many common MyUFHealth options using the quick selection buttons The main navigation tabs across the top take you to key areas within your chart, including Home, Messaging, Visits, My Medical Record, and Preferences. The Read your messages bar alerts you if you have any new messages or test results. The To Do section displays any upcoming orders that have been placed for you. The Care Team section displays your list of providers and quick action icons for each provider. By clicking the envelope icon you can quickly send that provider a message. By clicking the calendar icon you can quickly request an appointment with that provider. 4

Messaging The Messaging Inbox contains messages that have been directed to you by your physician or the practice. The inbox may also contain test results that have been forwarded to you by your physician. Read Messages (Inbox) From the Home page, click the Messaging tab. Click the Inbox option (items in bold indicate a new message). Click on the message title (under the Subject column) to read the message. Delete Messages Click the selection box next to the message(s) you want to delete. (NOTE: messages must be read or marked as read before they can be deleted.) Click the Delete button. 5

Get Medical Advice From the Home page, click the Messaging tab. Click the Get Medical Advice option. In the To the office of selection box, choose the physician to whom you would like to send your message. You can message providers you have seen in an office visit within the past 13 months. Select the appropriate Subject from the list of topics Enter a message in the Enter Text Here box below (limit to 500 characters). When you are finished typing your message, click the Send button. Note: This is NOT for urgent medical advice. Please allow two business days to receive a response. All communications sent or received using MyUFHealth are saved as part of your medical record. 6

Letters This page displays any letters your provider has sent to you through MyUFHealth. From the Home page click the My Medical Record tab. Click the Letters option. Click on a letter to view the complete letter. Request Rx Refill You can also use MyUFHealth to request a refill of one of your current medications. To prevent any lapse in your medication supply, send your request for a new prescription at least one week before your current medication supply runs out. From the Home page, click the Messaging tab. Click the Request Rx Refill option. Click the selection box(es) for the prescription(s) you wish to have refilled. Add any notes/comments in the box at the bottom of the window. Click the Continue button. Select a pharmacy from the drop-down list OR select Other from the list and specify pharmacy instructions in the blank box. Click the Submit Request button. Reminder: Please allow two business days to receive a response. 7

Ask Customer Service You can also use MyUFHealth to send a message regarding a billing or account question, a compliment to share, or a question about the MyUFHealth site. Customer Service messages will not be viewed by clinic staff or your provider. Please use Get Medical Advice to communicate with your provider. From the Home page, click the Messaging tab. Click the Ask Customer Service option. Enter the subject of your message in the Subject box Select the type of your message using the Regarding drop-down list. Add the details about your message in the Message box. You are limited to 500 characters. Click the Send button. Reminder: Please allow two business days to receive a response. 8

Sent Messages Click the Sent Messages option. Click on the message subject to open the message review window. Click the Print icon, Delete button, or Back to Messages list button to finish. 9

Visits MyUFHealth allows you to check the time and place of your upcoming appointment or to request an appointment with the practice. Future Appointments Upcoming Appts From the Home page click the Visits tab. Click the Upcoming Appts option. To view more details, to confirm the appointment, or to cancel the appointment, click on the date/time for the specific appointment. o If you wish to cancel the appointment, click Cancel this Appointment. Cancelling appointments from the menu (when available): o Click Visit on the top menu and then click Cancel Appts. o Click the radio button next to the appointment you want to cancel, click Continue. o Select a Cancel Reason from the dropdown list. o Add any comments, as needed. o Click the Confirm Cancellation button. o Beginning a week before your appointment you can confirm you will come to the appointment by clicking Confirm this Appointment. In some cases, you will not receive the reminder call from the clinic when you confirm your appointment in MyUFHealth. 10

Upcoming Tests and Procedures From the Home page click the Visits tab. Click the Upcoming Tests and Procedures option. You will see a list of any upcoming tests or procedures placed for you. Schedule an Appointment The Schedule an Appointment page allows you to begin the process of directly scheduling your appointment (not available in all areas) or requesting an appointment. 11

Request an Appointment From the Home page click the Visits tab. Click the Schedule an Appointment option. Select the type of appointment from the available options New Problem Visit, Problem Follow-Up Visit, Annual Physical, or Well Child Check Up. On the Request an Appointment page you will select the desired Want to see, Reason for visit, and Preferred dates. You can Limit times to All available or select Other and indicate your preferred times. Enter any comments for the scheduling staff in the text box at the bottom of the window. Click the Send button. Reminder: Please allow two business days to receive a response. 12

Directly Schedule a Follow-up Appointment When available as an option, you can also use MyUFHealth to schedule your next return visit appointment. This option is not available for all patients and clinics. To direct schedule your next appointment, complete the following steps. There are screencaps below as well. From the Home Page click the Schedule an Appointment Quick Navigation Button. Select the reason for visit for your clinic location Jacksonville or Gainesville Office Visit and click Continue. You may be asked to confirm your demographic contact information. Select your provider and specify any day or time limitations you may have and click Continue. Any available appointments will display. You can use the Next Times and Previous Times buttons to view past or future days of appointments available. When you have located your desired appointment, select the appointment by clicking on the circle to the left of the appointment date and time, and then click Continue. You can then review your appointment date and time, and enter your reason for visit in the Comments box. Click Make Appointment to schedule your appointment. You can view your appointment on the Upcoming Appointments list. ** If needed, you can also cancel any upcoming appointments. 13

Visit History Visit Summaries From the Home page click the Visits tab. Click the Visit Summaries option. View the list of past appointments. View your Appointment Details: o Click anywhere on the row to view more details regarding the appointment. o The Appointment Details window will open showing details of your visit. 15

Admission Summaries From the Home page click the Visits tab. Click Admission Summaries. Click on the row of an admission to view the Your Admission summary. From the Hospital Admissions page you can click on a link to download your admission information to send to another provider. The Download My Record page contains detailed information to download Admission Summary Information. 16

My Medical Record What s in My Record My Plan of Care My Plan of Care provides you a quick overview of several parts of your MyUFHealth account. You can see any goals that you and your provider have set for you, you can review any upcoming appointments or orders that have been placed for you, and you can see your current medications. Care Team displays your list of providers and quick action icons for each provider. By clicking the envelope icon you can quickly send that provider a message. By clicking the calendar icon you can quickly request an appointment with that provider. From the Home page click the My Medical Record tab. Click the Plan of Care option. Test Results Test results released to your MyUFHealth account by your provider or through an automatic process will display. Results for Imaging and Mammography tests now display in the web version of MyUFHealth (written findings, not actual images) and on the MyChart Mobile app. Results for Procedures and Pathology tests will not contain components and may only include comments from your provider. From the Home page click the My Medical Record tab. Click the Test Results option. Click on the row of the test result you wish to view in the Details window. 17

Health Summary From the Home page click the My Medical Record tab. Click the Health Summary option to view your health profile and portions of your patient record. Current Health Issues From the Home page click the My Medical Record tab. Click the Current Health Issues option to view your documented health issues. 19

Medications From the Home page click the My Medical Record tab. Click the Medications option to view a list of your current prescriptions. Requesting updates to your medications: Click the Submit or Review Updates button at the top right corner of the page. Click the green plus sign to request a medication be added to your chart. You will be prompted to complete specific details about your medication, the dose you are taking, and when you began taking the medication. Click the red minus sign to indicate you are no longer taking a medication and request for it to be removed from your medication list. Reminder: Requests to update your medication list will be reviewed during the next office visit with your provider. Updates will not occur automatically. 20

Requesting a refill: Click the Request Refills button in the top right corner of the page to request a new prescription for a listed medication that no longer has refills. o Select the medication. o Enter any comments in the text box at the bottom of the window. o Click the Continue button. o Select a pharmacy from the drop-down list OR select Other from the list and specify pharmacy instructions in the blank box. o Click the Submit Request button. Reminder: Please allow two business days to receive a response to a refill request. 21

Allergies From the Home page click the My Medical Record tab. Click the Allergies option to view list of your documented allergies. Requesting updates to your list of allergies: o Click the green plus sign to request an allergy be added to your chart. You will be prompted to complete specific details about your allergy, your allergy reaction, and any comments regarding the allergy. o Click the red minus sign to indicate an allergy should be removed from your list of allergies. Reminder: Requests to update your list of allergies will be reviewed during the next office visit with your provider. Updates will not occur automatically. 22

Medical History From the Home page click the My Medical Record tab. Click the Medical History option to see an overview of your medical history. 23

Immunizations From the Home page click the My Medical Record tab. Click the Immunizations option to view your immunization record. Administrative Info From the Home page click the My Medical Record tab. Click the Administrative Info option to review your documented Patient Information. 24

Medical Tools Lucy Lucy and MyChart Central are products developed by Epic, our software developer, designed to help you manage your medical information more easily. Lucy is similar to your own personal medical record, like a "Medical" file folder in your file cabinet at home. You can create a Lucy chart, add and delete information from it and share this information with providers you would like to have the information. MyChart Central is a central location to view all of your MyChart accounts. For example, if you have a MyChart account at Piedmont Hospital in Atlanta, Georgia and a MyChart/MyUFHealth account in Gainesville, Florida, you may view information from both charts (Atlanta and Gainesville) in one location, MyChartCentral. Download Summary MyUFHealth Download Summary allows you to download or extract information from your MyUFHealth account with the UF Health. The information includes your allergies, current medications, current health issues, results and immunizations. The information in your MyUFHealth Download Summary will display the information you see in MyUFHealth at the time of your download. In MyUFHealth this summary is referred to as your Lucy record. This name and reference is from Epic Systems Corporation, the company that created MyChart (MyUFHealth). The Download Summary page includes a link to more detailed Download My Record instructions. From the Home page click the My Medical Record tab. Click the Download My Record option. Type a password for your record if you would like to password protect. A password is not required. Click the Download button. 25

A File Download window displays (this may not display instantly). Click Save and choose a location on your computer to save the file. You can view the summary or share the file with another clinic or provider. After you save the document to your computer, you can go to the file you saved and open your record to view it. 26

You may want to save your record to a thumb drive so you can share with other providers. At the bottom of your record you will see information on sharing with other providers If you think your information is incomplete or incorrect, let a UF Health Physicians staff member know at your next clinic visit. Contact your clinic if additional assistance is needed with your summary. 27

Download My Record From this page you can download records from MyUFHealth and print or save to a drive to share with another provider. Click on Visit Records. To obtain detailed instructions about downloading your records click on the link as seen below. You can choose to download records in several different ways: From a single visit From a date range You can download all visits You can download your Lucy Summary A list of visits will appear that you can choose from, Admissions will be included on this list. 28

After you select the records you wish to download click the Download button. A message will appear to tell you what you are downloading: Click Download again and you will be asked if you wish to open or save your record. If you wish to share your record, you should save to another location, i.e. on your computer or an external (thumb) drive. 29

Once your file is saved you can navigate to the location you saved the file. It will be saved with the name Health Summary and the date you saved it. Who s Accessed My Record? This page displays each time you or a proxy to your account has logged into your MyUFHealth account. From the Home page click the My Medical Record tab. Click the Who s Accessed My Record option. Each row will show the date and time your MyUFHealth account was accessed by you or one of your proxies. 30

Questionnaires This page displays any questionnaires that your provider may have requested you to complete prior to or after a visit. Completing questionnaires in MyUFHealth will save time at your clinic visit. 31

Wallet Card You should print out a new wallet card periodically as the information in your MyUFHealth changes. From the Home page click the My Medical Record tab. Click the Wallet Card option. Click on the Printer icon to print out your MyUFHealth wallet card. 32

Billing Billing Acct Summary: From the Home page click the Billing tab. Click the Billing Acct Summary option. You can see a summary of your account. You can also link to other billing activities. Make a Payment Click on Pay : o If you have a question, you can click Send a Customer Service Request o You can choose which visit you would like to apply your payment to o You will be prompted to fill in your credit card information, fields are required. 33

If you choose Send Customer Service Request you will be prompted to type in your message. You should expect a response in 2 business days. See More Payments From the Billing Acct Summary Page click the See More Payments link : You can select the time from for which you would like to see payments. All payments made within the selected time frame will appear, identified by payment description, credit card information and payment amount. If you have a question about your payments you can click the send us a customer service request link. 34

Account Details From the Billing Acct Summary Page click the See Account Detail link o If you have a question about your account you can click send us a customer service request o If you are uninsured there is information about Financial Assistance o If you would like to set up a payment plan, click on the Set up a Payment Plan link o To pay your account, click on Pay by Credit Card o o o o If you click the Set up a Payment Plan link you will be taken to the Sign up for a Payment Plan Screen You can select the day of the month to make your payment You can select your payment amount. NOTE: Minimum payment amounts are based on your balance, if you enter an amount lower than the minimum, that amount will not be accepted and the minimum amount will default as your payment amount. You will be prompted to enter credit card information. Your payment will be deducted from your credit card automatically on the day of the month you select. 35

o You can view guarantor demographics o You can see outstanding balance, aging of the balance and total balance o You can see information about outstanding accounts with charge, payment, insurance balance and patient balance information. 36

o You can link to view details for an itemized view of the account o You can see your statements and payments that have been made since your last statement o You can click on a statement row to see statement details 37

o If you have been sent any billing letters you can view them here o o If you would like to make a payment you can click on the Pay by Credit Card You can also click View Recent Payments to be taken to recent payment screen Sign up for Paperless Billing o o o From the Billing Acct Summary page click the Sign up for Paperless Billing link You will be prompted to enter the e-mail address that you wish to receive paperless correspondence. You will be prompted to confirm you will no longer receive statements in the mail. 38

To make a payment for a family member, you will need to have proxy access to their account. At their next clinic visit front desk staff can help you set up proxy access to your family member s MyUFHealth account. 39

Preferences My Family s Records: (Proxy Access to Other Family Members Charts) From the Home page click the Preferences tab. Click the My Family s Records option. To access an account you have been given proxy access to, click on the row for that person s account You can also view a list of the people who have been given proxy access to your account. To access a family member s account for which you have approved access, click on the name of the person shown in the tab at the right side of the page. 40

Demographics From the Home page click the Preferences tab. Click the Change Demographics option. Update your contact information as needed. Most fields are required. Click the Accept Changes button. Your demographics are automatically updated. 41

Security Settings - Change Password From the Home page, click the Preferences tab. Click the Security Settings option. Enter your Current Password, enter the New Password you want to use, Confirm the new password by retyping it. Click the Save Password button. To Change Your Security Question/Answer, enter your Current Password, Choose a security question from the New Question dropdown list, enter your New Answer, and click the Save Ques/Ans button. If you do not select a question from the list, it will remain the same. 42

Notifications From the Home page, click the Preferences tab. Click the Notifications option. To update your Contact Information, Enter your New E-mail and verify the new e-mail by retyping it. Click the Save Changes button. Terms & Conditions From the Home page, scroll to the bottom of the page. Click the Terms and Conditions option to review the terms and conditions for MyUFHealth use.
