Module 9 CMR Editing: Notes Title Slide Hello and welcome to the introductory tutorial on accessing EpiTrax, the state of Kansas electronic disease surveillance system. This training is being provided by the Kansas Department of Health and Environment. 2 Subsection Title Slide You are beginning Module 9 CMR Editing: Notes. 3 In Module 9, the objectives are for the user to add a new note, sort and view notes, and add an attachment to the case. Please note: you can pause this recording at anytime to accomplish the tasks requested. 4 The first activity of this module is to log into EpiTrax Training system and open a CMR in edit mode. 5 6 After opening a CMR in edit mode, the user will see the event header with the CMR described by patient, disease, jurisdiction, and event status. Below the event header will be the tabs with the Demographic tab open. Clicking the tab Notes will allow the user to access the notes that were added to the system. There are 3 sections to the Notes Tab: Attachments, Notes, and Tasks. Tasks will be covered in another module.
7 Notes can be listed by all, administrative, clinical, and brief. Administrative notes are usually system added notes indicating when a CMR was edited or routed. 8 Clinical notes are entered by a user to record data that cannot be entered on any other tabs. To enter a new note, type your note into the New notes text box. If the note is an administrative note mark the Is admin box to mark it as such. If you do not click the Is admin box the note by default will be categorized as a clinical note. Admin notes can be descriptions about editing or routing cases. 9 After a note is completed, Save & Continue must be clicked to save the note. The note will be placed up in above the earlier notes and it will be automatically stamped with a date and time that it was created. 0 A note that has been saved is permanent. It cannot be deleted. However, the user can strike through the note to indicate that it is not longer valid. To strike though a note, click the Strike Through box located to the left of the note.
Brief notes are added to help communicate to other users as to why a case may still be open or was reopened, why it was routed to another jurisdiction, or what is needed to complete the case. These notes are usually entered form the Event header. 2 The notes are sorted by clicking the red lettered links under the event notes. Return to the CMR that was opened in the Edit Mode. Add a new note to the CMR. Identify and if desired use the Is Admin and Strike Through boxes. Sort the notes by All, Administrative, Clinical and Brief EpiTrax allows the user to attach a file to the CMR. To attach a file, click on the Add Attachment in the red-lettered links below the Patient Name in the event Headings. 2 This will bring the user to the Add Event Attachment page. An abbreviated patient header appears at the top of the page with the patient name, jurisdiction, and links to show or edit the CMR.
3 Below the header the user will find a section for adding a new attachment. The File field is used to locate and attach the desired file by locating it with the Browse button. 4 The Category drop-down field is used to select an appropriate category for the file to be attached. 5 After clicking the Create button, the file screen will refresh with the file selected under the Existing Attachments. The file can be opened or deleted from this screen with the red text, located on the right. 6 To return to the CMR, click the red lettering in the patient header.
7 When the screen refreshes, the Morbidity Event will be seen. The attachment will now be found on the Notes tab, where it can now opened or deleted. Return to the CMR that was opened in the Edit Mode.
