POLICY: In order to accurately compute capacity of each mainframe/midrange/server for the Capacity Plan, a Server/Host Information Form is to be filled out for each existing server or host managed by MS. A copy of the form is attached to this procedure. The forms should be completed by the IS Group responsible for the management of the server or host and should be forwarded to the IT personnel responsible for the maintenance of the Capacity Plan. SCOPE: This policy/procedure applies to all Critical and Core systems supported by and its Subcontractors. DEFINITIONS: None PROCEDURE: Completing the Server/Host Information Form This procedure should be followed for filling out the form for each existing server. ACTION 1. Procures an electronic copy of the Server/Host Information Form from the designated shared directory or from the person responsible for Capacity Planning. RESPONSIBLE PARTY IS Functional Group that is responsible for the particular server/host 2. Edits the MS Word document which contains the form, filling in a response for each line item on the form, entering the data either on the same line if space permits, or by inserting a line following the question and entering the response there. 3. Saves the edited document, using the following naming convention: Server Information Form Servername, where Servername is the name of the server or host. 4.
