Recrding Heart Sunds and Lung Sunds Our Stethscpe prvide fr the recrding and playback f sunds, using the Audi I/O jack. Using yur stethscpe and an external recrder r playback device, yu can: - Recrd while listening t a patient. - Recrd vice anntatins (dictatins) by simply speaking int the stethscpe. - Playback recrdings, using the stethscpe headphnes fr high-quality audi. See safety precautins fr Recrding. Never recrd frm patients using a recrding device that is cnnected t main pwer, r t any ther equipment. Use nly battery-pwered flating devices. Fr playback with n patient cntact, any equipment may be used. Quick Instructins Recrding and playback can be dne very easily with ur stethscpe. Simply cnnect the stethscpe t the Line Input f the recrding device, listen t the patient, and recrd. T listen t the recrding, turn ff the stethscpe, plug the stethscpe int the Headphne Output f the playback device, and listen. Cmpatible Devices and Listening t recrdings The fllwing devices may be used fr Recrding: - Apple ipd Vide with recrding attachments. (Other ipds d nt prvide recrding capability) - Mp3 players with built-in recrding feature. - Prtable digital recrders. - Ntebk cmputer, perating n battery pwer. Listening t recrdings is best dne using the stethscpe in headphne/playback mde. The reasn is that bdy sunds, especially heart sunds, cntain significant lw frequency cntent. DigiScpe stethscpe are designed t reprduce these lw frequencies accurately. Mst heapdhnes cannt reprduce bdy sunds adequately. T listen t recrdings, perfrm the fllwing steps: 1. Turn ff yur stethscpe, r set the stethscpe vlume t 0. 2. Cnect the stethscpe t the Headphne Output f the recrding r playback device. 3. Play back the recrdings. *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 14

Tlbars Cntrl Tlbar (tls, play buttn, etc.) Thinklabs Tlbar (Imprting and editing heart and lung sunds) Mixer Tlbar (vlume cntrls, input device) Meter Tlbar (Input and Output level meters) Edit Tlbar (editing shrtcuts) Cntrl Tlbar Audi Cntrl Buttns Skip t Start - mves the cursr t time 0. If yu press Play at this pint, yur prject will play frm the beginning. Play - starts playing audi at the cursr psitin. If sme audi is selected, nly the selectin is played. Lp - if yu hld dwn the Shift key, the Play buttn changes t a Lp buttn, which lets yu keep playing the selectin ver and ver again. Recrd - starts recrding audi at the prject sample rate (the sample rate in the lwer-left crner f the windw). The new track will begin at the current cursr psitin, s click the "Skip t Start" buttn first if yu want the track t begin at time 0. Pause - temprarily stps playback r recrding until yu press pause again. Stp - stps recrding r playing. Yu must d this befre making changes, edting, saving r exprting. Thinklabs Tlbar The Thinklabs Tlbar prvides quick access t special features that are useful fr visualizing, manipulating and analyzing heart and lung sunds. Sme f these functins are als accessible via the Thinklabs Menu. Thinklabs Special Functin Buttns Imprt Audi - allws yu t imprt sunds recrdings int the current Prject/Patient. If yu have recrded ne r mre sund(s) frm a patient, use Imprt t pen the sund file. Cntinue t add additinal files that belng t a given patient, s that the Prject cntains all the relevant recrdings made during ne exam. Each file is imprted int its wn separate Track. Nte that Stere recrdings will imprt with bth Left and Right. Use the Track Menu t Split Tracks, then Delete the unwanted track, and then select Mn in the Track Menu. This will cnvert stere t Mn and remve a duplicate track. Amplify - (a Track r part f a Track must be selected t activate this functin) - allws yu t scale a track t be luder and sfter. It is recmmended that yu simply Amplify t the maximum level. This can be dne autmatically by clicking Amplify and then OK, and the Amplify functin will adjust t maximize withut clipping any peaks. If yu wish t cntrl the amplificatin level, enter a value int the db windw. Nte that a signal dubles in amplitude fr every +6dB and halves fr every -6dB amplitude change. Nte that yu can select a small segment f a wavefrm, such as the sunds between peaks, and amplify them significantly t enhance murmurs. Filter - (a track r part f a track must be selected t activate this functin) - allws yu t filter a track r part f a track. There are varius pre-set filters fr manipulating heart sunds and lung sunds, r yu can adjust the Equalizer t filter sund t yur wn specificatins. Fr instance, yu may find that a sharp high-pass filter (allw high, blck lw frequency) enables yu t listen t a particular valve sunds very clearly. Merely place yur cursr ver the Equalizatin Curves, and draw the desired respnse t create a custm filter. Rate Change - (a track r part f a track must be selected t activate this functin) - changes the temp (speed) f the audi withut changing the pitch. This will change the length f the selectin. This is useful fr expanding heart sunds s that heart sund splits, S3 r S4 and rapid events such as gallps can be slwed dwn and spread ut. Display Spectrgrams On/Off - allws yu t view tracks with the Spectrgram belw the Waavefrm, r the Wavefrm withut Spectrgram. The Spectrgram shws frequence cntent by shwing Frequency alng the vertical axis and clr as the Intensity f a given frequency. Fr instance, if yu see a bright regin twards the lwer part f a spectrgram, it suggests that there is a high intensity f lw frequency energy at that mment in time. This is mst useful fr seeing murmurs r additinal heart sunds such as S3 in a wavefrm which may nt be visible n the time-dmain plt f the wavefrm. Nte that if yu have a slw cmputer r limited memry, it is best t manipulate tracks (zm, pan, etc.) withut the Spectrgram, and the click Display t shw the Spectrgram nce yu've zmed in. Otherwise yu may find that zming r panning acrss a track is very slw. Mixer Tlbar The Mixer Tlbar has three cntrls, used t set the vlume levels f yur audi device and chse the input surce. The leftmst slider cntrls the playback vlume, the ther slider cntrls the recrding vlume, and the cntrl n the right lets yu chse the input surce (such as "Micrphne", "Line In", "Audi CD", etc.). Use the Recrd Level Meter t set the crrect level. T recrd frm yur stethscpe, cnnect the stethscpe t the Mic r Line Input, and *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 16

select the crrect Input surce fr the stethscpe. Then adjust the Vlume Cntrl n the stethscpe, and the Input Vlume level n the slider. Generally it is best t set the stethscpe Vlume Cntrl t Maximum (10). Hwever, check the recrding fr clipping f the signal which will prduce distrtin and annying sund artifacts. Ideally, set the level t peak at abut 50-75% f maximum. Once yu have made yur recrdings, use Amplify n the Thinklabs Tlbar t maximize signal levels. Changing the Speaker cntrls has n effect n the audi data in yur prject - in ther wrds it desn't matter what the utput vlume level is when yu Exprt r Save a prject - the end result is the same. Edit Tlbar All f the buttns n this tlbar perfrm actins - and with a cuple f exceptins, they're all just shrtcuts f existing menu items t save yu time. Hlding the muse ver a tl will shw a "tltip" in case yu frget which ne is which. Cut Cpy Paste Trim away the audi utside the selectin Silence the selected audi Und Red (repeat last cmmand). Zm In Zm Out Fit selectin in windw - zms until the selectin just fits inside the windw. Fit prject in windw - zms until all f the audi just fits inside the windw. Meter Tlbar The Meter Tlbar is used fr mnitring the input and utput audi levels. Typically it is used t make sure that the ludest vlume is as lud as pssible (fr maximum fidelity) withut clipping r distrting it. The utput (playback) meter is the green ne, n the left in the picture abve, and the input (recrding) meter is in red, n the right. The meters prvide a visual indicatin f the current audi levels ging in and ut f audacity. If yu flat the Meter Tlbar by dragging it ut f the tlbar, yu can resize it and even rient it vertically. *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 17

Nrmally the meters are nly active when yu are playing r recrding audi. Hwever, yu can als mnitr input when yu're nt recrding - t d this, either select "Mnitr Input" frm the input meter's pp-up menu, r else just click n the input meter. If yu have a micrphne r ther input surce attached, yu will be able t watch the level f the audi befre yu start recrding. Each meter shws several characteristics f the audi level at nce: The right hand end f the meter crrespnds t the pint at which the audi will be clipped, and the left hand end is silence Fr stere, the tp bar shws the left channel, and the bttm bar shws the right channel. The brightest part f the bar shws the average audi level (related t the ludness) and the darker part f the bar shws the peak audi level. The peak-hld line t the right f this shws the maximum audi level achieved in the last 3 secnds. Finally, the clipping indicatrs n the far right f each meter will light up if clipping is detected (meaning that the audi was t lud and will sund distrted). If clipping is detected when yu are recrding, yu shuld stp, lwer the vlume f yur input surce, and start recrding again frm the start. If the utput meter clips then yu need t make sme r all f yur tracks quieter using the track gain cntrl. If the level f the input (recrding) surce is t high, yu can try t lwer the input level using the Mixer Tlbar, but if this desn't wrk, yu shuld try t lwer the vlume f the external input surce (e.g., yur micrphne, cassette player, r recrd player). It is pssible, especially if yu have an lder, slwer cmputer, that the Meter Tlbar may interfere with Audacity's ability t recrd r play audi with the highest quality, because yur cmputer is s busy redrawing the meters that it desn't have time t prcess enugh audi. In this is the case fr yu, yu can disable the Meter Tlbar in the Interface tab f the Preferences dialg. *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 18

Abut Thinklabs - takes yu t www.thinklabsmedical.cm. Abut Audacity - displays the versin number and credits. If yu cmpiled Audacity yurself, check here t verify which ptinal mdules were successfully cmpiled in. Thinklabs Menu The items in this menu nly wrk when yu have audi selected. Audacity des nt have any real-time effects; yu must select the audi, apply the effect, and then listen t the results. Mst effects have a Preview buttn. Clicking n this buttn plays up t three secnds f audi, allwing yu t hear what it will sund like after the effect is applied. This is useful fr fine-tuning the effect parameters. Amplify - changes the vlume f the selected audi. If yu click the "Allw clipping" checkbx, it will let yu amplify s much that the audi ends up beynd the range f the wavefrm, and is clipped (distrted). The default value when yu pen the effect is t amplifiy s that the ludest part f the selectin is as lud as pssible withut distrtin. Change Temp - changes the temp (speed) f the audi withut changing the pitch. This will change the length f the selectin. This is useful fr expanding sunds s that heart sund splits and ther rapid events can be slwed dwn. Nte that yu must select part r whle track t activate this functin. Equalizatin - Bst r reduce arbitrary frequencies. Yu can select ne f a number f different curves designed t equalize the sund f sme ppular recrd manufacturers, r draw yur wn curve. FFT Filter - similar t Equalizatin, lets yu enhance r reduce arbitrary frequencies. The curve here uses a linear scale fr frequency. Nrmalize - allws yu t crrect fr DC ffset (a vertical displacement f the track) and/r amplify such that the maximum amplitude is a fixed amunt, -3 db. It's useful t nrmalize all f yur tracks befre mixing. If yu have a lt f tracks, yu may then need t use the track gain sliders t turn sme dwn. High Pass Filter - allws yu t filter ut lwer frequencies and keep the higher frequencies. This is useful fr filtering ut heart sunds when yu are nly interested in lung sunds, enhancing valve clicks, etc. This is a quick filter t use, hwever we recmmend using the Filter Buttn n the Thinklabs Tlbar fr better cntrl f the filtering parameters. Lw Pass Filter - allws yu t filter ut higher frequencies and keep the lwer frequencies. This is useful fr filtering ut hiss r white nise, ambient nise, r breath sunds, and enhance heart sunds. This is a quick filter t use, hwever we recmmend using Filter Buttn n the Audacity Tlbar fr better cntrl f the filtering parameters. Plt Spectrum - T use this feature, first select a regin f audi frm a single track, then select "Plt Spectrum". It pens up a windw that displays the Pwer Spectrum f the audi ver that regin, calculated using the Fast Furier Transfrm. The graph represents hw much energy is in each frequency. As yu mve the muse ver the display, it shws yu the nearest peak frequency. This windw can als display ther cmmn functins that are calculated using the Fast Furier Transfrm, including three versins f the Autcrrelatin functin. The Enhanced Autcrrelatin functin is very gd at identifying the pitch f a nte. Imprt Audi - allws yu t add a sund file t the Prject (Patient). If yu have recrded ne r mre heart r lung sunds, and yu wish t wrk n them, r add them t ther sund tracks yu already have pen, use Imprt t imprt the sund file int a new audi track. When yu use Imprt, a Label Track will als be added t facilitate anntatin f the imprted sund.. New Label Track - allws yu t add a new Label Track t a prject. This is t prvide anntatin.. *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 29

Light/Dark Clr Scheme - There are tw clr schemes - dark backgrund and light wavefrms (Dark Clr Scheme), r light backgrund and dark wavefrms (Light Clr Schemes). The Dark Clr Scheme is intended fr viewing n a cmputer screen, and the Light Clr Scheme is useful fr cpying the screen t paste int reprts that might be printed. By using a Light Clr Scheme fr printed reprts, yu save tner ink, and create prints that are easier t read. Yu may als find the Light Clr Scheme t be mre effective during Pwerpint presentatins r ther teaching using a prjectr. Other Preferences Sund File Frmats Envelpe Editing MP3 Exprting General Public License Preferences Audi I/O Playback Device - Use this cntrl t select the device that will be used fr playback / audi utput. This is usually nly applicable if yur cmputer has mre than ne sund card. Recrding Device - Select the devce that will be used fr recrding / audi input. Nte that many devices have multiplesurces such as Micrphne and Line in - t select the input surce yu will need t use the Mixer Tlbar. Fr finer cntrl ver audi I/O, pen yur system's Sund cntrl panel r the cntrl panel sftware that came with yur sund card. Recrding Channels - Use this t select the number f channels t recrd simultaneusly. Select 1 fr mn and 2 fr stere. Audacity will supprt recrding mre simultaneus channels, but nte that mst sund cards nly supprt stere, and even if yur sund card supprts mre than 2 inputs, yu need a fast cmputer and a large, fast hard disk t recrd many channels fr a lng time. Mn recrding is nt the same n all cmputers r sund cards. Smetimes recrding mn nly recrds the left channel, and smetimes it mixes the left and right channels. Play ther tracks while recrding new ne - When this bx is checked, Audacity will play existing tracks when yu press recrd - therwise it simply recrds the new track withut letting yu hear what yu've already recrded. Yu can use this ptin t recrd harmnies with yurself r add a vicever. Yu may ntice that when yu play the tw tracks yu recrded tgether, they aren't synchrnized. This is unavidable t a certain extent, althugh Audacity tries t minimize it. T fix it, yu will need t grab the Time Shift tl and slide ne f the tracks arund until it sunds right. Hardware Playthrugh (Mac Only) - plays the audi yu are recrding straight back ut t yur headphnes r speakers s yu can hear it. This ptin is dne in harware, s it is fast and desn't cnsume resurces. Hwever, it is nly pssibly if input and utput are n the same sund device. PC users can d the same thing in their vlume cntrl settings, turning up the relevant audi input. Sftware Playthrugh - Des the same thing as hardware playthrugh, but in sftware. This means that the audi yu hear may be slightly delayed relative t the input, and activating this ptin will use sme mre system resurces. Hwever, it wrks n all systems, even if yu are recrding frm ne device and playing back thrugh anther. Quality Default Sample Rate - This cntrls the sample rate f new prjects. T change the sample rate f an existing prject, use the cntrl in the lwer-left crner f the main prject windw. Default Sample Frmat - This cntrls the default frmat used t stre audi samples. 16-bit takes up the least space and is equivalent t audi CD quality. 32-bit flat takes up twice as much space but is much mre flexible. If yu have a fast cmputer and enugh disk space, yu shuld always use 32-bit flat samples while editing, and then exprt yur final mix as a 16-bit WAV file (the default). *The actual prduct may nt be the same as displayed in the phtgraphs. Real-time sample rate cnverter - Audacity has mre than ne sample rate cnverter that's used when yu have a track that's nt the same sample rate as the prject. This ptin lets yu set the cnverter used during real-time playback, which can be different than the ne yu use during Exprt r mixing. High-quality sample rate cnverter - Audacity has mre than ne sample rate cnverter that's used when yu have a track that's nt the same sample rate as the prject. This ptin lets yu set the cnverter used during Exprt and mixing, which can be different than the ne yu use fr real-time playback. Real-time dither - Dithering is used when cnverting high-quality samples with a lt f dynamic range, t CD-quality samples, with less dynamic range. A small amunt f dithering can make the audi sund a little bit better, but it can als slw dwn prcessing a little bit. This ptin lets yu set the dithering used fr real-time playback. High-quality dither - Dithering is used when cnverting high-quality samples with a lt f dynamic range, t CD-quality samples, with less dynamic range. A small amunt f dithering can make the audi sund a little bit better, but it can als slw dwn prcessing a little bit. This ptin lets yu set the dithering used during Exprt and mixing. File Frmats When imprting uncmpressed audi files int Audacity Make a cpy f the file befre editing (safer) - Selecting this means that Audacity will take lnger t imprt files, but it will always have its wn cpy f any audi yu are using in a prject. Yu can mve, change, r thrw away yur files immediately after yu pen r imprt them int Audacity. Read directly frm the riginal file (faster) - Selecting this means that Audacity depends n yur riginal audi files being there, and nly stres changes yu make t these files. If yu mve, change, r thrw away ne f the files yu imprted int Audacity, yur prject may becme unusable. Hwever, because Audacity desn't need t make cpies f everything first, it can imprt files in much less time. Uncmpressed Exprt Frmat - This lets yu select the frmat that Audacity will use when yu exprt uncmpressed files. 11 cmmn ptins are displayed in the list, but yu can als select "Other" and chse a nnstandard file frmat fr Audacity t exprt. Ogg Exprt Setup - Use this cntrl t set the quality f Ogg Vrbis exprting. Ogg Vrbis is a cmpressed audi frmat similar t MP3, but free f patents and licensing fees. A nrmal quality Ogg Vrbis file is encded with a quality setting f "5". Nte that unlike MP3 encding, Ogg Vrbis des nt let yu set a bitrate, because sme audi clips are easier t cmpress than thers. Increasing the quality will always increase the file size, hwever. Exprt Setup - Use these cntrls t lcate yur MP3 encder and set the quality f MP3 encding. Higher quality files take up mre space, s yu will need t find the level f quality yu feel is the best cmprmise. Fr mre infrmatin, see Exprting MP3 Files.Spectrgrams Yu can view any audi track as a Spectrgram instead f a Wavefrm by selecting ne f the Spectral views frm the Track Pp-Dwn Menu. This dialg lets yu adjust sme f the settings fr these spectrgrams. FFT Size - The size f the Fast Furier Transfrm (FFT) affects hw much vertical (frequency) detail yu see. Larger FFT sizes give yu mre bass reslutin and less tempral (timing) reslutin, and they are slwer. Grayscale - Select this fr gray spectrgrams instead f clred nes. Maximum Frequency - Set this value anywhere frm a cuple f hundred hertz t half the sample rate (i.e. 22050 Hz if the sample rate is 44100 Hz). Fr sme applicatins, such as speech recgnitin r pitch extractin, very high frequencies are nt imprtant (visually), s this allws yu t hide *The actual prduct may nt be the same as displayed in the phtgraphs. Audacity uses this directry whenever yu wrk n a prject that yu haven't saved as an Audacity Prject (AUP file) yet. Yu have t restart Audacity (clse and pen it again) fr changes t the temprary directry t take effect. Interface Autscrll while playing - Scrlls the windw fr yu while playing, s that the playback cursr is always in the windw. This can hurt playback perfrmance n slwer cmputers. Always allw pausing - Nrmally the Pause buttn is nly enabled while yu are playing r recrding. Checking this bx allws yu t set the pause buttn anytime, which allws yu t press Recrd and nt have the recrding start until yu unpause it. Smetimes starting a paused recrding can be faster than starting t recrd in the first place. Update spectrgram while playing - Because spectrgrams are slwer t draw, nrmally they are nt drawn during playback, but this ptin lets yu draw the spectrgrams anyway. Enable Edit Tlbar - Sets whether r nt yu want t display the Edit Tlbar, which has sme cmmn shrtcuts fr editing cmmands. Enable Mixer Tlbar - Sets whether r nt yu want t display the Mixer Tlbar, which lets yu set the vlume levels and input surce. Enable Meter Tlbar - Sets whether r nt yu want t display the Meter Tlbar fr setting audi recrding and playback levels. Quit Audacity upn clsing last windw - By default n Windws and X-Windws (but nt Mac OS), Audacity quits when yu clse the last prject windw. If yu uncheck this bx, Audacity will pen a new blank dcument instead f quitting. T quit Audacity in this case, yu must specifically select Exit (r Quit) frm the File menu. Enable dragging f left and right selectin edges - Nrmally, when yu mve the muse ver the left and right edge f a selectin, the cursr changes t a left r right pinter, and yu can adjust that edge f the selectin independently. If yu dn't like this feature, uncheck this bx, and then clicking will always create a new selectin (unless yu hld dwn Shift t extend an existing selectin). Language - sets the language used by Audacity. Language files are named "audacity.m" and are fund in the "Languages" flder n Windws and Mac OS X, r in /usr/share/lcale r /usr/lcal/share/lcale n mst Unix systems. Audacity will detect new languages the next time yu start it. Keybard This panel lets yu change keybard shrtcuts. All f the cmmands that appear in Audacity menus appear n the left, alng with a few ther buttns that can get keybard shrtcuts. T change a cmmand, first click n the cmmand yu want t change. Then type the new keybard shrtcut n yur keybard. Verify that the crrect shrtcut appears in the bx belw. If it's what yu want, press the Set buttn. Or t get rid f a keybard shrtcut, press Clear. T reset t Audacity's defaults, press the Defaults buttn. This will get rid f any changes yu have made. If yu have custmized yur keybard layut and want t share it with smene else, yu can press Save... and save yur cmplete keybard layut as an XML file that yu can share. T lad an existing layut, press the Lad... buttn and lcate the XML file. *The actual prduct may nt be the same as displayed in the phtgraphs. Page N. 33
