WELCOME Welcome to MDVIP Connect, a brand new way to live your healthiest life! To help you easily connect with your doctor and make healthy choices every day, MDVIP Connect is designed to deliver a personalized experience that allows you to keep track of important health and wellness information, securely communicate with your doctor, explore healthy eating and much more. Through streamlined communication and intuitive technology, you will have the opportunity to build a strong partnership with your doctor focused on attaining your best health. Whether you are on a desktop computer or connected through a mobile device, you will have convenient access to MDVIP services at your fingertips. This MDVIP Connect User Guide will help you navigate the site and explain how to maximize your MDVIP benefits. This guide outlines: How to reset your log in credentials What to expect when logging into MDVIP Connect for the first time How to navigate through options, settings and features How to change and update your profile, contact and billing information How to securely communicate with your MDVIP-affiliated doctor and his or her staff How to customize your Wellness Wall How to complete your Health Assessment and view results How to explore nutrition tips and recipes designed for your health needs How to find and visit an MDVIP-affiliated doctor while traveling How to contact MDVIP Support Whenever questions arise about MDVIP Connect, please call MDVIP toll-free at 866-602-4081, Monday through Friday between 9am and 10pm Eastern, or email support@mdvip.com. 3

FIRST-TIME LOG IN 1 First-Time Log In With your username and password, you can proceed to MDVIP Connect and use convenient tools to take charge of your health, all in one streamlined location. When logging in for the first time, you will see the MDVIP Connect Tour video. To view, click the play arrow located in the center of the video screen, then click Next to move to the following step. 4

FIRST-TIME LOG IN After completing the MDVIP Connect Tour video, you have the option to upload a profile picture. You can choose to skip this step and upload a photo at a later time using Profile Settings. To upload a profile picture, click Browse to select a stored image on your computer or mobile device, then click Upload to complete the process. Click Next to move to the following step or Previous to go back. After uploading a photo, you will be asked to customize your Wellness Wall. The MDVIP Wellness Wall keeps you updated with the latest health and wellness information in areas that interest you. Personalize your news feed by clicking the corresponding boxes. Click Finish to complete the process or Previous to go back. 5

FAILED LOG IN ATTEMPTS 2 Failed Log In Attempts When log in credentials are incorrectly entered fewer than five times, you will receive an automatic error message. Make sure that caps lock and num lock are turned off on your keyboard and try again. When log in credentials are incorrectly entered five times, you will receive an automatic error message notifying you that you have been locked out of your account for 15 minutes. You can reset your password online after 15 minutes. You can also request a new username by calling the support number. If you have not been locked out of your account but would like to reset your password, you can request new log in credentials through the Forgot Password link on the MDVIP Connect log in screen. 6

FORGOT PASSWORD 3 Forgot Password To reset your MDVIP Connect password, follow the steps below to have a link sent to the email address on file. After you click the link, you can then proceed through two factor authentication to reset your password. To reset your MDVIP Connect password, follow the steps below. Step 1 Enter username Enter your username into the corresponding text field on the Forgot Password screen. Click Submit to move to the following step. Step 2 Two-Factor Authentication An email will be sent to the address on file. Click the link within the email and then select the method that you wish to further authenticate yourself. 7

FORGOT PASSWORD Create a Methods Key Text - You ll receive a text message containing a 7 digit security code. Phone Call - You ll receive an automated voice call to the mobile or phone number on file that will present a 7 digit security code. Zip Code & Date of Birth - Enter the zip code of the address associated to your account and your date of birth in the proper format (DD/MM/YYYY). Click Next, and enter the security code provided. Note: If you choose to enter your Zip Code & Date of Birth, you will not be required to enter a security code. Two Factor Authentication - Enter Security Code Step 3 Complete Password Reset Enter a new password per the guidelines provided. Note: You cannot use your username nor any of the previous 5 passwords as your new password. 8

PROFILE SETTINGS 4 Profile Settings The Your Account navigation within MDVIP Connect is designed for ease of use. You can find and edit all of your information in one secure place, usually with just one click. Profile To edit your profile, click Your Account on the top navigation bar and click Profile. Required fields are noted with an asterisk. If you would like to change your name as it appears on our records, please call MDVIP directly at 866-602-4081, or email support@mdvip.com. If you update your mailing address, it is recommended that you update your credit card billing address as well. Click Save after making any changes to your Profile Settings. 9

PROFILE SETTINGS Billing To edit your billing information, click Manage Payments on the top navigation bar or click Manage Payments from the left navigation under My Settings. MDVIP memberships can be paid with a credit card. To set up payment, click Update on the Manage Payments screen. Click Add New and enter your credit card and billing information into the text fields on the Credit Card Information screen. Please note, each MDVIP Connect account can store multiple cards. To set a specific card as your default payment card, click the check box icon on the Manage Payments screen. To delete a credit card, click the X check box on the Manage Payments screen. 10

PROFILE SETTINGS Wellness Wall To edit your Wellness Wall preferences, click Your Account on the top navigation bar and click Wellness on the left side of the screen. The MDVIP Wellness Wall keeps members updated with the latest news on health and wellness topics. Select the topics that interest you by clicking the corresponding boxes. Your Wellness Wall will be updated regularly with articles tailored to your interests. Once you are satisfied with your selected Wellness Wall topics, click Save after making the changes to your preferences. 11

MDVIP CONNECT NAVIGATION 5 MDVIP Connect Navigation Everything you need to manage important information is right at your fingertips in MDVIP Connect. Home: Returns you to the Home page Support: Directs you to the main Customer Support page Your Account: Directs you to your MDVIP Connect settings panel Community: Directs you to our blog and Community page You will find your MDVIP Connect tools and information divided into the categories below. Numbered alerts will appear next to navigation categories to notify you of new or unread content and messaging updates. Messages displays secure messages from your MDVIP-affiliated doctor s office. Wellness Wall features the latest articles according to your interests. Nutrition provides healthy recipes and condition-specific meal plans. Health Assessment offers in-depth health questions with the ability to view the results after completion. Health Records allows you to view documentation from your Annual Wellness Exam. MDVIP Workouts MDVIP Workouts offers a robust library of exercises with instructions and videos. 12

MDVIP CONNECT NAVIGATION Member Benefits gives you the opportunity to learn more about MDVIP member benefits. Manage Payments allows you to pay invoices, enroll in Autopay, and manage payment accounts. My Doctor In the My Doctor box, you can see your MDVIP-affiliated doctor s photo and contact information, send secure messages and recommend your doctor to friends and family. To send a secure message to your doctor, click Send a Message. To recommend your doctor to a friend, click Refer a Friend. Fill in your name and email address, the names and email addresses of up to three friends and your personal message in the corresponding text fields. Click Send Email to complete the referral process. To submit a testimonial about your doctor or your MDVIP experience, click Tell Your Story. If your doctor is active on Facebook and/or Twitter, there will be icons displayed that link you to his or her accounts. 13

MDVIP CONNECT NAVIGATION Annual Wellness Exam The Annual Wellness Exam box displays the recommended date for your next exam. Please call your MDVIP-affiliated doctor s office to schedule your appointment at a time convenient for you. My Health Assessment If you have completed your Health Assessment, click View Results to see your scores on a variety of health factors. If you have not completed your Health Assessment or if you wish to retake it, click Start Assessment to begin. 14

MDVIP CONNECT NAVIGATION Latest Wellness Wall Articles The most recent articles will be displayed on your Wellness Wall throughout the day. To see all of the articles on your Wellness Wall, click View All. Social Media To stay connected with MDVIP on Facebook, Twitter and YouTube, click the corresponding icons at the bottom of the MDVIP Connect page. 15

MESSAGES 6 Messages To make your health our number one priority, MDVIP Connect enables secure messaging between you and your doctor and his or her staff, ensuring that your questions are answered in a timely manner. My Messages Navigation Compose: Create and send a new message Archive Messages: Archive a message in your inbox Messages: Messages Displays all messages sent and received from your doctor and his or her staff Drafts: Displays all messages that have been drafted and saved, but not sent Starred: Displays messages that you have marked as important Archive: Displays messages that have been filed away Search: Search messages for specific content Page Turn Arrows: Click through all pages of messages, beginning with the most recent 16

MESSAGES Note: Unread messages are displayed in bold with a blue dot. A number displayed next to the sent date indicates how many messages are in each email chain. Viewing and Sending Messages Click on a message to view the entire conversation from the beginning. From here, you can reply, star or archive a message and more. To view the subject, refer to the top of the message. To mark a message as important, click on the star icon in the top right corner. To print a message, click on the printer icon in the top right corner. To reply, click Reply to be taken to a new message box at the bottom of the screen. To attach a file to a message, click Choose File next to Attach in the message composition box, and select and upload a stored file. 17

MESSAGES Messages automatically reply to the sender only. To include additional addresses, you can use your Contact List to add recipients. The Contact List icon displays the doctor and staff with whom you can interact. If a recipient s name is accidentally added to an email, delete his/her name from the To box. NOTE: If you are experiencing an emergency, call 911 immediately. Secure messaging should only be used for non-urgent matters. 18

WELLNESS WALL 7 Wellness Wall With MDVIP Connect s Wellness Wall, you are provided with the latest health and wellness information that matters most to you. Navigating the Wellness Wall You can search for specific content within your Wellness Wall using the Search box in the top right corner of your screen. Each article is color-coded by category, making browsing simple and quick. If you would like to see which color corresponds to each category, click on the arrow icon next to the Search box in the top right corner. You can click on any article s thumbnail to view the full article. 19

WELLNESS WALL Navigating Articles You can share an article via Facebook, Twitter or email by selecting the corresponding Share button on the right side of your screen. 20

HEALTH RECORDS AND HEALTH ASSESSMENT 8 Health Records and Health Assessment Health Records At home or on the go, monitoring your health is simple with instant access to your personal health records. You can view your personal health records at any time using MDVIP Connect. To save or print a hard copy of your health record, click Download Health Record. If you do not yet have a health record on file, please contact your doctor s office. If you have completed your Health Assessment, click View Results to see your scores on a variety of health factors. If you have not taken your Health Assessment, or if you wish to retake it, click Start Assessment to begin. Health Assessment The Health Assessment takes approximately 15-20 minutes to complete. You will see a progress bar above each question noting percent complete. You can save and return to your Health Assessment at a later time if you are not able to finish in one sitting. 21

HEALTH RECORDS AND HEALTH ASSESSMENT Health Assessment Results When you have completed your Health Assessment, you and your doctor will both receive an in-depth results report. You can then work together to devise a health and wellness plan according to your needs and goals at your next appointment. 22

HEALTH RECORDS AND HEALTH ASSESSMENT When viewing results, you will see a report divided into five sections: 1. My Overall Health Score Your overall health score is factored on a scale from 1 to 100, and each question is individually weighted. You will see a brief synopsis of your score and how you can maintain or improve your health and lifestyle factors. 2. My Score Breakdown Your score breakdown displays individual scores for 10 key categories. When you click on a category, you will see an analysis of your score. 3. Things You re Doing Well This provides insight into your positive health and lifestyle factors. 4. Things You Need to Improve This provides a snapshot of actions you can take to improve health concerns and overall lifestyle. 5. My Health History This displays your individual scores in the 10 key categories for each Health Assessment you have taken. To see how making changes to your lifestyle can improve your score over time, click Improve Your Score. If you have any questions regarding your results, please contact your MDVIP-affiliated doctor. 23

NUTRITION 9 Nutrition Home Page Eating right is easy, stress-free and delicious with MDVIP Connect s personalized 28-Day Meal Plans. Discover healthy recipes and meal plans tailored to meet your specific health and lifestyle needs. The Nutrition home page will display a featured recipe each week with a short description and photo. To view the complete recipe, click View Recipe. To save the recipe, click Add to Recipe Box. 24

NUTRITION Recipes Click the Recipes tab to search for recipes by Health & Diet Considerations, Main Ingredient or Type of Dish. To expand and select additional recipe preferences, click the category name you want to expand. You can also search for specific items using the search box on the top left side of the screen. Click the heart icon next to any recipe to save it to your My Recipe Box. To view all saved recipes, click View My Recipe Box on the left side of screen. The entire catalog of recipes will be displayed in alphabetical order. Use the forward and back arrows in the bottom right corner of the screen to navigate through the catalog. To save or learn more about a recipe, click on the photo. Meal Plans Meal plans can help you make healthier choices on a daily basis. Click the Meal Plans tab on the home page to find 28-day plans specially designed for weight loss and heart health. Each meal plan includes instructions and tips, recipes and weekly shopping lists. 25

NUTRITION To find your perfect meal plan, click Begin Meal Plan to get started. On the Select a Meal Plan screen, click on the ideal meal plan for you and click Next to move on. To determine the recommended caloric intake needed to reach your weight goal, use the text field to enter your current weight. Next, using the corresponding bubbles, select whether you would like to lose weight or maintain weight. If you select lose weight, use the drop-down menu to select how many pounds you desire to lose per week. If none, select 0. You will then receive a caloric intake recommendation and corresponding meal plan. To start a plan at a different caloric level, use the drop-down menu. Once satisfied with your assigned plan, click Next to move on. 26

NUTRITION When your meal plan is ready, you can download the entire 28-day plan by selecting Download Entire Meal Plan, or select individual weeks and download associated information by selecting Download Meal Plan, Download Shopping List and/or Download Recipes. All meal plans download in PDF format. 27

MDVIP WORKOUTS 10 MDVIP Workouts Perfect Fit Perfect Fit MD is an online tool that provides detailed 3D video descriptions of hundreds of pre-designed workouts. 28

They are focused in the 4 areas: Balance Seated Exercises Weight loss Stretch & Strengthen 29

MDVIP WORKOUTS Weight Loss and Stretch and Strengthen workouts are available in beginner, intermediate and advanced levels. Each exercise within a workout has a unique video with safety warnings, spotting procedures, special instructions, equipment required, progression and regression of the exercise, the primary purpose, procedures and techniques of the exercise and a complete listing of the muscles targeted. 30

MEMBER BENEFITS PROGRAM 11 Member Benefits Program One of the many benefits of being an MDVIP member is that you are able to enjoy the extensive Member Benefits Program. For example, if something urgent comes up while traveling, you can search for an MDVIP-affiliated doctor nearby and get connected through your personal MDVIP-affiliated physician. To find an out-of-town doctor, you can use the Find an MDVIP Doctor search box at the top of the Member Benefits Program page to search by location or by name. If you are traveling out of the country, click the Traveling Internationally? link to find tips for staying healthy abroad. Navigating Search Results Search results will display details on the local MDVIP-affiliated doctors in the area and related contact information. 31
