1 Completing the Full application form 1.1 Initial application screen Click on Apply now for a full application. 2 P a g e
1.2 Assessment Scopes [2] Please ensure the Full Application option is selected. If you are a Care Service provider please select the care services option and refer to the care services guide. Please read the important CDM 2015 changes and tick here to confirm. [4] If you are a member of a CHAS Recognized Trade Association click here and select the relevant one. 1.3 Declaration and fees [1] If you have been assessed recently by another SSIP scheme member and would like to apply via the Deem To Satisfy route please click on the Deem To Satisfy option and read the instructions on screen, if not please continue. [3] Select the relevant assessment scopes that apply to your business. [5] Please click on the continue button to proceed with the application. You will be taken to the following page, please make sure you read all the information carefully and then complete the forms and continue. [1] Please read The Reasons why a CHAS assessment may be failed and tick the box. The Scope of business will display the Scopes you have selected along with the relevant fees below. [2] Please enter your name & position & other details requested. [3] Please click on the continue button to proceed with your application. 3 P a g e
1.4 Business details page The business details page then needs to be completed, all the information entered on this page must be accurate and supported by the documentation you provide. [1] Any fields marked with a * are Mandatory and must be completed in order to continue. [2] If you are applying with a trading name your supporting documentation must provide evidence of this. [4] Select the work categories you wish to apply via. If you start typing the name of the category then some auto suggestions will appear, click on the correct category then click select in list. You may also manually browse the list and use the tick boxes. All selected categories will show on the right hand side of the screen. If you cannot find the relevant category please select specialist in the list and then use the work category details box to provide a brief description. [6] If you are unable to complete your application at the current time please click the save button and an email will be sent out with a reference and password. You can then continue at a later point. [3] Enter your insurance details here and if you have supporting documentation please use the buttons opposite to upload this. Please see section 4 (Page 11) for further instructions on uploading documents. [5] Once all of the fields have been completed please click the continue button to proceed. 4 P a g e
1.5 Continuation of application [1] Questions that do not have a text box response option will require you to upload supporting documentation. Please see section 4 (Page 11) for further instructions on uploading documents. [2] Some questions may require both a text answer and uploaded documentation. [3] You may save your application on any page using the save button towards the bottom of the screen. [4] If you wish to navigate between pages please use the back and continue buttons on screen and not on your browser. 5 P a g e
1.6 Summary Page Before submitting you will see a summary page which will show a list of all the supporting documentation you have uploaded. You also have the option to upload any more documentation and if anything is incomplete or missing when you hit submit you will see an error message. [1] The sections tabs are all links and clicking on these will take you directly to each section. [4] If any sections have not been completed you will be notified when you try and submit the application. The section towards the top will be displayed in red and links will be provided at the bottom right of the page. [2] All supporting documentation you have uploaded will be listed here, this can be removed using the red button on the far right. [3] If you wish to upload any more documentation please select the section you wish to upload to and then click the upload button. Please see section 4 (Page 11) for further instructions on uploading documents. [5] Once you are happy everything is complete please click on the confirm and submit button to submit your application. 6 P a g e
2 Making Payment 2.1 Payment page You will then be transferred to our secure payment system Civica and will be required to enter your details and make payment. [1] Please enter your details, ensuring the correct amount is being charged. [3] Once you have completed the form please click submit to make payment. [2] If at any point you wish to return to the application form please use the cancel payment button. 7 P a g e
2.2 Payment Confirmation On the payment confirmation page you will need to confirm the amount, after successful completion of the payment you will see a notification page. [1] Click the Confirm button to make payment. You will then see your payment reference Note:- screens are for illustration purposes only for actual fees, Please visit http://www.chas.co.uk/chasfees.aspx [2] Final page with confirmation of payment processed and reference numbers. Your application will be assigned to an assessor and you will receive email confirmation of your assessors details within three working days. Your application has now been submitted to CHAS and will be assigned to an assessor within three working days. Your assessor will then complete the assessment within ten working days, if they require any more information they will notify you of this via email, please see section 3 below if they require this. If the assessment was found Compliant, Eforms will email you a copy of the Approval letter, and also an electronic copy of the CHAS Accreditation Certificate (a hard copy of the certificate and accreditation pack will follow in the post within 7-10 working days). 8 P a g e
3 Resubmission process 3.1 Log in to Eforms system If your initial submission could not be processed then you will have received an email explaining that your assessor requires more information. Firstly you will need to login to your account, your login ID will be provided in the email if you do not already know it. [1] Please enter your Login ID provided in the email. [2] Click on the login button. 3.2 Required actions If you login via the link in the email you will automatically see the following page, if you did not then you will need to click on the applications tab towards the top of the page. [2] Submitted Applications will list any applications you are currently going through as well as all previous years applications, these can be viewed by clicking on the blue link. [1] Any applications that require a response will be listed in the required actions table. Clicking on the blue link will open the application. 9 P a g e
3.3 Leave response for assessor and resubmit application Once you have clicked on the blue link you will see your application, as you click continue through the application you will see any messages left by your assessor. [1] Any section that requires additional information or documentation will have comments requesting this. [2] Once you have provided the requested information or documentation please click on the add a response button and leave a reply explaining this and then continue through the application. Once all the requests have been completed please resubmit your application. Your assessor will now review the additional information and if they require any more information they will notify you of this please repeat section 3 above if they require this. If the assessment was found Compliant, Eforms will email you a copy of the Approval letter, and also an electronic copy of the CHAS Accreditation Certificate (a hard copy of the certificate and accreditation pack will follow in the post within 7-10 working days). 10 P a g e
4 Uploading Documentation If you wish to upload documentation at any point please click on the upload supporting documents button which will be located in various places throughout the application. You will then see the following screen, from here you will be able to upload your documents. Click on the browse button The following box will appear and you will need to locate the correct file on your computer and then click open. [1] Locate the file you wish to upload. [2] Click on the open button. This file will now be listed in the file name box and you can click upload to finalise the uploading of the file. [1] File name is listed here.
