NextMD Patient Portal TideLine Clinic Quick Start Guide About the Patient Portal NextMD is a consumer-oriented healthcare website that provides you with a reliable and easy-to-use method of communicating with your medical service provider. With NextMD, you can connect with your doctor through a convenient, safe, and secure environment to: Request, book and/or cancel appointments Receive and review documents Request medication renewals Send and receive secure messages to and from your provider Complete, submit and review online forms Complete, submit and view Instant Medical History forms Request Personal Health Records Update account information Sign up with multiple practices

Enrolling After giving your email address to the staff at the TideLine Clinic, you will receive an email with instructions and an eight-digit token that will allow you to log in and find the AICS TideLine Clinic in the NextMD system. Follow the instructions you receive in the email to set up a user ID and password for your Patient Portal Account

Inbox By default all items from all practices to you and any people you are managing display. To refine the number of inbox items displayed, use the Practice, To, and Type filters located at the top of the Inbox. To reply to a message Select Messages from the Types list Click a message to open it Click Reply, type your response and click Send To View Documents sent to you by the clinic, you need Adobe Acrobat Reader installed on your computer. Select Documents from the Types list To view comments from the clinic regarding the document click the Balloon icon Click the document to view it A File Download dialog box will display and you may choose to Open, Save or Cancel the action The Inbox displays all incoming items from all practices in which you are enrolled. You can access incoming messages, documents, templates and interactive forms. From the inbox you can click links at the top of the page to: Compose Messages Renew Medications Request Appointments Request Personal Health Records

Request Medical Record A Personal Health Record (PHR) is an electronic summary of your health and medical history (allergies, medications, family history, etc.) that you can request from the clinic. When you receive your PHR, you can view it, print it, and/or save it to a storage device. To request a PHR: Click the Personal Health Record link Select a Practice and for whom you are requesting the report if other than yourself Click Request Latest Personal Health Record A message that your request has been successfully submitted displays When the health record is sent to you, it will show up as a new item in your inbox. It will also become available under the link My Medical Record

View Your Medical Record After you have requested and received your Personal Health Record from the clinic you can review it in detail. To review your most recent Personal Health Record: Click My Medical Record If necessary, select the appropriate patient That patient s clinical history is displayed according to the components on the left. Click the different components to see additional detail. To export all of the health information for a particular patient Select the appropriate patient Click Export and follow the promptings from your browser to either open or save the document that is generated

Viewing Appointments Click My Appointments To refine the number of items displayed, use the Practice and Patient drop down filters To view previous appointments, check the Include Past Appointments check box Click a column name to sort the list in ascending or descending order To navigate to other pages click the page number(s) at the bottom of the page Click an appointment to view the details Delete Function Instructions I m not sure how we want them to use this feature There isn t a graphic I can use at this point because there are no upcoming appointments. Use the My Appointments feature to review current and past appointments for either yourself and/or any of dependents. Requesting Appointments You can complete an online appointment request, submit it and receive a response from the clinic to confirm your appointment date and time. To create an appointment request: Click the Request Appointment link Complete the form with the appropriate information Click Submit The Clinic will review your request within 24 hours and you will receive an email notification to check your NextMD Patient Portal for the confirmation or pending appointment request for your proposed dates and times. You may be required to Accept the clinic s proposed appointment to confirm your appointment. If the proposed appointment does not work, you may cancel the appointment request and submit a new one.

Renew Medications NextMD provides an easy way to request medication renewals. To renew your medications Click the Renew Medications link Select the appropriate Practice and Patient Complete the Prescriptions to Renew If no prescriptions are listed, click Select To add or remove existing prescriptions, click Change You may also check the Display any inactive medications to view prescriptions that have expired Check or clear the box next to each medication you wish to add or renew and click Submit Select the Pharmacy of your choice If no pharmacy is listed, click Select or to change it, click Change Enter the search criteria for the pharmacy of your choice and click Search Click the button next to the pharmacy s name to select it If the pharmacy of your choice does not appear, click Add New Enter the new pharmacy information and click Save Select the Reason for your renewal and the Person to whom you are sending the medication renewal request Click Submit

Account Settings allows you to change your user name, your password, your security questions and forgotten password question.
