Beginning from the BTIS Home Page Welcome to BTIS, home of our exclusive Victory products and legendary customer service! Registering as a Producer is fast, easy and free. More importantly, we do not hold a minimum written premium to have an appointment. To get started, click on the Register As A Producer tab on the top menu bar. 1

The Producer Registration Page You will be redirected to our online Producer Registration form. We will require your Agency information, license number and E&O coverage information to secure your appointment. 2

The Producer Registration Page Once all of the information has been entered, click on the Submit My Information button to submit your appointment application. 3

Producer Agreement Email From: Builders And Tradesmen s Insurance Sent: Thursday, January 14, 2016 8:01 AM To: Agent Jane Subject: Producer Agreement Once your registration application has been reviewed and approved, you will receive an email from BTIS requesting the final items to complete your registration and receive your Agency code and password to login to our site. To begin the E-Signature process, click on the Electronically Sign & Submit the Producer Agreement button. 4

E-Signing the Producer Agreement From: Builders & Tradesmen s Insurance Services, Inc. Sent: Thursday, January 14, 2016 8:01 AM To: Agent Jane Subject: Please sign BTIS Producer Agreement (APP5093790) for Best Choice Insurance Agency Shortly after, you will receive a new email containing the documents needed to E-Sign. Click on the blue link to review and sign the Producer Agreement. Should you need to forward this email to another party to E-Sign, click the link to forward the E-Sign documents. 5

E-Signing the Producer Agreement Y Next you will be taken to the electronic copy of the Producer Agreement where you can review and E-Sign the document. You will have an Options tool on the top left in the case someone else will need to sign, etc. Notice the top right, advising what actions are required for completion. Click the blue Next Required to be taken straight to the E-Signature line. agentjane@insurance.com You will be directed to the end of the document where the signatures are required. The yellow Next icon will help show you where the signature field is for you to sign. Click on the signature field. 6

E-Signing the Producer Agreement The E-Signature pop up will appear. Simply type in your name as you would if you were signing the document in person. You may also choose to draw your signature using your mouse. agentjane@insurance.com Once applied, you will see your E-Signature on the Producer Agreement. To finalize the E-Signature process, click the Click to sign blue button on the very bottom of the document. 7

Producer Agreement Download and Email Acknowledgment Once signing is successfully completed, you will receive this confirmation message. You will also have the option to download the signed copy of the Producer Agreement for your files. BTIS will automatically be sent a copy. From: BTIS Sent: Thursday, January 14, 2016 8:32 AM To: register; Agent Jane Subject: BTIS Producer Agreement (APP5093790) for Best Choice Insurance Agency between BTIS and Agent Jane is Signed and Filed! You will also be notified via email that the Producer Agreement has been E-Signed and filed. You may access a copy of signed Producer Agreement here as well. 8

Request for W-9 Information From: BTIS Inc. Sent: Thursday, January 14, 2016 8:32 AM To: Agent Jane Subject: Please sign W-9 Request for Taxpayer Identification Number and Certification Next you will be required to E-Sign the W-9 Request for Taxpayer Identification Number and Certification that is needed to complete your Agency registration. Click on the blue link to get started. 9

Filling Out the W-9 You will be redirected to an electronic copy of the W-9. All peach-colored fields indicate the places to be completed. You will have the same Options tool at the top left in the case that another party will need to complete and sign. Click on the yellow Start arrow to help guide you through the document for completion. 10

Filling Out the W-9 You will notice that we have pulled some of the information from the online application you submitted. Please review all of the information to ensure it is correct. Once you have reviewed and completed all of the required fields, click on the signature field to begin E-Signing. 11

E-Signing the W-9 Sign your name by typing it in the provided field. You may also choose to draw your signature using your mouse. Review your name below and click Apply when accepted. Once applied, you will see your E-Signature on the W-9 form. To finalize the E-Signature process, click the Click to sign blue button on the very bottom of the document. 12

Download the W-9 and Email Acknowledgment Once signing is successfully completed, you will receive this confirmation message. You will also have the option to download the signed copy of the W-9 for your files. BTIS will automatically be sent a copy. From: BTIS Sent: Thursday, January 14, 2016 8:53 AM To: Agent Jane; echosign Subject: W-9 Request for Taxpayer Identification Number and Certification between BTIS and Agent Jane is Signed and Filed! You will also be notified via email that the W-9 has been E-Signed and filed. You may access a copy of signed W-9 form here as well. 13

Registered Producer Welcome Email From: Builders & Tradesmen s Insurance Sent: Thursday, January 14, 2016 9:06 AM To: Agent Jane CC: Inside Sales Subject: Agency Code and Password Xx123 ABC123. After all required documents have been E-Signed and accepted by BTIS, you will receive an email from our Marketing Team providing you with your unique Agency Code and Password to login to our website! Your registration is almost complete! Lastly, you will need to send our office a copy of your current E&O Declarations page, Certificate or Binder. And that s it! You are ready to start quoting business! 14
