Site Package Minimum Total Cost includes monthly cost of selected Optus Loop plan plus minimum 10 per month Site Package required for each Optus Loop Site for the life of the plan. An Optus Loop Site is a location defined by a number range and the above features associated with that number range. Multiple `sites' may be at the same physical location. Internet Connection All Optus Loop plans require a fixed andor mobile internet connection, depending on the plan you choose. The cost of Optus Loop plans does not include the cost of your internet plan connection. The type and cost of broadband connection will depend on individual customer needs and requirements. Only Optus Loop IP handsets can be used. Internet usage fees: Customer needs to check with their internet service provider; if they exceed data plan allowance, additional usage fees or capping may apply Optional features can be purchased at additional cost. Serviceability: Service qualification will be required for desk phones. Not needed for Optus mobile devices. In order to receive the Service, you must have a Broadband connection capable of handling 100Kbps on uncongested upstream and downstream bandwidth per call for optimal quality. Optus is not responsible for other service provider broadband issues impacting the Optus Loop product. The Optus Loop service also requires the capability to connect Optus equipment to the Broadband Network. Installation fees may apply if you request a Professional or Remote installation (respectively from 250 and 45 per user) where a self-install option is not available. If you are considering a Professional Installation you are required to first pay a Site Audit fee of 250, which will credited to the cost of the Professional Installation fee if that goes ahead. If you change plans during your contract term to a plan of lower value, you need to go through special pricing unit (call 1300 617 648) or a rate plan change fee of 99 applies if you are moving to a plan of higher value. Month to month customers may change their plan with no fee. Cancellation: If you cancel any or all services within the committed term, you may be charged a cancellation fee. All plans have the ability to purchase IP handsets. Some plans allow the ability to finance IP handsets over contract term. International call rates: can be found at optus.com.auworldsaver. Other rates are available at optus.com.austandardagreements. 4 5

Your phone repayments are reduced by a monthly credit. If you need to cancel your plan there are no plan cancellation fees, simply pay out the full remaining cost of your phone which will include any amount that Optus was going to cover. 6 Business Lease plans allow you to lease an eligible device on a 24 month plan and use it to access the Optus network. You do not own the device and you must return the device to Optus in good working order at the end of your contract. If it is not in good working order, you ll pay a damage fee of up to 499. If the phone gets damaged during your lease and you d like to have it fixed, you need to pay to have it repaired by Optus or the manufacturer (excludes warrantyconsumer guarantee claims). You can offer to purchase the device at the end of your 24 month lease contract for fair market value (which we ll advise at the time). If you don t return the device at the end of your contract term, you ll continue to pay monthly device charges for up to 6 months (as well as your plan fees). At that point, if you still don't return the device, you will have to pay us a non-return fee (the fair market value of the device which we ll advise at the time) and you will own the device. After 12 months you can upgrade the device by paying a one-off fee of 99. To do this you need to return the current device in good working order to Optus and get a new device on a new 24-month plan. If the current device is damaged, you ll pay a damage fee. If you don t return the current device within 14 days of your new contract you ll have to pay for the device at its fair market value. Cancellation: If you cancel at any time within the 24 month contract term you will be charged a plan cancellation fee, pro-rated monthly for your remaining contract term. Additionally, if you cancel in the first 18 months you ll have to keep the phone and pay all monthly device charges (which will be increased by any device credits lost as a result of early cancellation) for the remainder of the contract term. If you cancel in the last 6 months of your 24 month contract, you can either return the device to us or make an offer to purchase it at fair market value (which we ll advise at the time). If you choose to return the device, you ll pay 99 if it s in good working order or a damage fee of up to 499 if it s damaged. International minutes: To standard numbers only. Yes international rates apply for non-included usage. See optus.com.aubizinternational for selected countries. Additional data: Additional data automatically charged at 101GB. If you use more than 150GB we may continue to charge you at the same rates or restrict your data use until next billing period. Data pool: Any unused data in the Data Pool expires at the end of the month. Optus Travel Pack: Gives you unlimited standard national and international talk, SMS and up to 100MB of data to use in Zone 1 countries per day. For list of countries see optus.com.aubiztravel You must activate each day of Optus Travel Pack prior to use and if you activate multiple consecutive days, the data allowance will accumulate for use anytime over the consecutive day period. If you exceed your daily or accumulated usage, standard data roaming charges will apply. Any data provided as part of an Optus Travel Pack is not shareable and will not form part of any Data Pool. Any unused days of Optus Travel Pack are forfeited and cannot be rolled over to the next billing month. International roaming: 105130 plans gives you unlimited standard national and international talk, SMS (excludes MMS) and an included amount of data to use in Zone 1 countries per month. For list of countries see optus.com.aubiztravel If you exceed your included data allowance then standard data roaming charges will apply. Any unused included data for the month is forfeited and cannot be rolled over to the next billing month. For 105130 plans, International Roaming will be switched on when you activate your plan. 7
