1 Login Screen TM Already enrolled? If you are already enrolled in Student Health Link, just enter the user ID and password you created, then click Continue (see Chapter 4: Logged In). Not enrolled? If you are just getting started, click Register and follow three easy steps toward a better way to access your student health center 24/7 (see Chapter 2: For Newly Enrolling Students). Forgot user ID or password? If you forgot your user ID or password, no worries. Just click the user ID or password link for a reminder (see Chapter 3: Forgot User ID or Password). 4
2 For Newly Enrolling Students TM 2.1 Terms of Use Agreement Read the Terms of Use agreement and mark the I Agree radio button. Also, note the box at the bottom of the window: INFORMATION YOU ENTER HERE WILL NOT BE REVIEWED BY ANY HEALTH CARE PROFESSIONAL UNTIL YOUR NEXT SCHEDULED APPOINTMENT. THIS SYSTEM IS NOT TO BE USED FOR EMERGENCIES OR WHEN YOU MUST SPEAK TO A PHYSICIAN IMMEDIATELY. Mark the I Understand checkbox and click Next to continue. 5
2.2 Student Identification/Verification TM Health records contain sensitive information. For security, we must first verify who you are before creating a user ID and password to access your account. In these fields, enter your student ID (officially assigned by the school, sometimes your Social Security number) and birth date. Click Next to continue. NOTE If Student Health Link cannot find your student ID, contact your clinic. Otherwise, you cannot complete registration. 2.3 Create User ID and Password TM You cannot complete these fields until you are first verified by the system. After successful verification, this section comes alive so you can create your Student Health Link login ID. Your user ID and password must be between 6 12 characters. Numbers and letters are acceptable, but not special characters. Click Next to continue. 6
2.4 Update and Verify This data is pulled from your records in the clinic. Update any information as needed. Changes here are automatically reflected in the clinic records. NOTE Make sure that your school e- mail address is correct. You will receive an e-mail notification that you have a new message from the clinic in Student Health Link. Current Address: Enter the address where you live during the school year. Note the pink shaded areas where information is required. Make sure you also provide your official student e-mail address and primary phone number. My Permanent Address: Your permanent home when you re away from school, usually the address of your legal guardian. Include a description of your relationship to the residence (e.g., parents home, own apartment). Campus Address: Select your campus location (such as residence hall or fraternity/sorority house), and enter campus mailbox and room number, if any. My Emergency Contact: Identify the main person you want contacted in case of an emergency, usually your legal guardian. Choose also which language the person speaks, and mark whether their cell phone accepts text messages. Click Update and verify all information is accurate. If you find any discrepancies, go Back to make any corrections. 7
3 Forgot User ID or Password Fill in the boxes below if you forgot your user ID or password and cannot access your account. If you have forgotten both, request your user ID first, then return to request a password. 3.1 Forgot My User ID For security reasons, all fields are required. Do not skip any fields: School E-mail: Enter your official school e-mail address. Birth Date: Your full date of birth. First Name: Your full first name (no nicknames). Last Name: Your full last name (no nicknames). Then click Continue. 3.2 Forgot My Password For security reasons, all fields are required. Do not skip any fields: School E-mail: Enter your official school e-mail address. User ID: Enter your Student Health Link user ID. Birth Date: Your full date of birth. Then click Continue. 8
4 Logged In 4.1 Secure Messaging Quick Stats This frame shows whether your immunization status is compliant with your school s requirements, and gives you quick links to Immunization Records where you can make updates (see 4.3: Immunization ). However, when you add an immunization to your record, your status temporarily becomes Not Compliant until you submit your paperwork to be verified by the health clinic. Secure Messaging Student messages are private. Simply click a subject from the table to expand your message text. Click again to hide the message. Use the message center to send and receive messages from the health center, just like your everyday e-mail. TM You may store up to 10 messages in your account. If your box is full, new messages will not appear until you delete old messages. To delete old messages, simply mark the checkbox and click Delete under the message text. 9
4.2 My Appointments Schedule an Appointment To quickly find available openings, select an appointment reason from the first drop-down menu. Your choice of providers in the second menu is narrowed to those who can see you for your chosen reason, and your locations are narrowed to the locations where your chosen provider services. Advanced Search Click the Advanced Search On radio button to filter your appointment search by specific dates, times and days of the week. This is useful to busy students who want to exclude times they are in class from search results. Next Available Click Search to see available appointment times. In the Appointments frame below, results appear in the Next Available table, five per page. You can click the numbers across the bottom of the table to scroll forward or backward through available times. Select your appointment choice by marking the radio button, then click Schedule. Booked appointments appear in the Scheduled Appointment table below. Scheduled Appointments Your booked appointments appear here in this table. To cancel an appointment, click the underlined Select link to highlight your appointment, then click Delete. NOTE You can only delete appointments you scheduled online through Student Health Link. Otherwise, call your health clinic to make any schedule changes. 10
4.3 Immunization Records Vaccine and waiver documentation from your doctor can be entered and edited under this tab. Your official immunization status shows as Not Compliant until you submit the necessary documentation to your clinic for review and approval. Once your health clinic confirms your entry, your status changes to Compliant. Status Check what s required from your school, or click Add New to enter vaccine or waiver information from documentation provided by your doctor. Click the Immunization or Waiver radio button to indicate which you are inputing. Note the pink fields where information is required: Immunization Immunization: Select the immunization from the list. Immunization Date: Enter the month, day and year when you received the immunization. Results: Enter the results of the immunization from the list of possible choices. Done in the U.S.: Indicate whether you received the immunization within or outside the United States. Waiver Immunization Code: Select the immunization from the list of choices. This information appears in documentation provided by your doctor. Waiver: Enter waiver information from the list of possible choices. Waiver Expiration: Enter the expiration date of the immunization waiver. If none, simply mark the No Expiration checkbox. Immunization Records The vaccine information you entered above appears here in this table. If you meet the immunization requirement, you are assigned a preliminary status of Compliant, shown in Compliant column. However, your official immunization status will not change until you have submitted the necessary documentation to your clinic for review and approval. 11
