2 WELCOME TO HANOVER HOSPITAL S PATIENT PORTAL. This online tool gives you the flexibility to access your health information and other resources on your time and between visits to Hanover Hospital. MyHealthPortal is available over the internet, which means you can use it from almost anywhere. As a patient of Hanover Hospital, enrolling in MyHealthPortal will allow you to: View Your Lab and Radiology Results View Your Medications Update Personal Information View Appointments Access to MyHealthPortal is secure so you may be confident that your private information is protected. Name: Medical Record: M000 Address: Last 4 digits of Social Security #: 2 This brochure contains the instructions and information needed to sign up for MyHealthPortal. Please be sure to keep this brochure in a secure location since it contains your personal information.

3 ACCESSING HANOVER HOSPITAL S MyHealthPortal The patient portal may be accessed from the home page of the hospital s website. or The following information is required to access MyHealthPortal. Hanover Hospital Medical Record Number or last four digits of Social Security number Personal address In order to enroll in Hanover Hospital s Patient Portal, we need to have your address on file in your electronic medical record. If you do not have an address, you will not be able to access the patient portal. Please contact our Health Information Management Department, Patient Portal office at (717) for portal assistance including the following scenarios. LOG-IN TO MyHealthPortal 1. In order to self-enroll, go to www.hhmyhealthportalenroll.com. You will need to have the following information available.

After enrollment information is entered, you will receive an email with the sign on web address. Please update your user ID and password as soon as possible.

Last Name, First Name as it appears in the hospital s electronic medical record Date of Birth Last 4 digits of SSN# or Medical Record number (Please enter M and all numbers including the zeroes.) address that was provided at registration Note: All information above must match what is recorded in the hospital s electronic medical record. 2. After enrollment information is entered, you will receive an with the sign on web address. Please update your user ID and password as soon as possible. The will expire in 30 days. 3. To continue with enrollment, access the portal login screen using the link from the Next you will see the screen (see picture on right) to set up your permanent Log In ID and Password. 5. Create new logon ID which must be a minimum of 6 characters and include 2 numbers. 4

If unsuccessful, please call the Health Information Management Department, Patient Portal office (717) for verification of information. 11. Click on the Log Off button and it will return you to the screen shown at right. 12. Click on the word here as instructed. This will take you to the Portal Login Screen. 13. Enter the new Login ID and Password that was created in the steps above. 14. Read the agreement and then click on the Accept button. 15. Upon successful login, you will be presented with your MyHealthPortal home screen. From there, you will be able to access your personal health information. 5

6 NAVIGATING YOUR HEALTH INFORMATION The Hanover Hospital MyHealth Portal is a secure web site that allows you to view outpatient appointments (For example, Radiology, Pulmonary, Rehab appointments), view your hospital visit history, view your hospital health summary, view your allergies and conditions, view lab results that were performed at a Hanover Hospital lab location, view outpatient test results, and view your medications. 7 FREQUENTLY ASKED QUESTIONS What should I do if I forget my log-on ID and/or password? Go to and click on the appropriate link. A message will be sent to your personal account with instructions on how to proceed. Who should I call if I do not have an established with the hospital or I do not know my Medical Record Number? Contact the Health Information Management Department, Patient Portal office at (717) As a parent or guardian, will I be able to see my child s patient portal? Parents of children between the ages of newborn and 12 years old will be able to request proxy access for their children s portal. There is no access to a child s MyHealthPortal account for a child between the ages of 13 and 17 years old. Please call (717) for further assistance. Who should I call if I have a question about my test results? If you have questions about your test results, please contact the ordering provider or your primary care physician. Can I download or print my information? You may download and print your health summary from MyHealthPortal. However, it is not recommended that you download personal health information to your PC. You could download information to a thumb drive. With any download, it is best to encrypt the data and secure it with a password. Reminder: For your personal privacy, once you download or print information, you must be careful how you maintain and distribute this information. Can I my information to my physician? Once downloaded, you can use to distribute the information. However, please be reminded that transmissions are not secure and there is a risk of the information being intercepted. In the future, it will be possible to securely transmit your health summary to a participating provider and/or hospital through a Health Information Service Provider (HISP). More information to come as the HISP is just beginning to be developed in the Commonwealth of Pennsylvania. Who should I call if I am having technical problems with the portal?
