Assessments: FAST FSC Job Aid The Assessments FAST screen allows users to document the Family Advocacy and Support Tool (FAST) Assessment as well as any Prep sessions that occurred. Family Advocacy and Support Tool Navigation Dashboard 1. From the : a. Locate the desired Case and click on the Case ID to bring the Case into focus. The Summary screen will open. Next Steps b. Click on the FAST Assessment Navigation Tile in the pane. 2. From within the Case: a. Click on the Tracking tile. Then click on the Assessments tile. 5/1/2017 FAST Assessment Job Aid 1 of 13

Assessments list screen 1. The Assessments list screen contains a both the FAST Assessment list pane and the Ages and Stages Questionnaire list pane. Inside the FAST Assessment list pane: New Fast a. To start documenting a new FAST Assessment, click on above the FAST Assessment grid. b. To view or update an existing FAST, click on the Assessment Date of the desired FAST in the FAST Assessment grid. c. To view the Respondent Details from the grid, click on the [ + ] to the left of the desired FAST in the grid. To close the Respondent Details view, click on the [ - ] that now appears to the left of that FAST in the grid. Assessment Basics 1. The ratings from the last Assessment will carry over to the next Assessment if the last Assessment was completed less than 8 months ago. 2. Definitions of the ratings will be listed at the top of each section. Hovering the mouse over a selected radio button [ ] will also display the rating definition for that number. 3. Some items, depending on what rating is selected, may cause additional questions such as Is this item a strength? or Is this an item of contention?. These items are marked by a [ ^ ] 4. Some ratings require a justification to be entered in the narrative box. If a justification for the rating is required a red asterisk [ * ] will appear to the right of the Justification narrative box. 5. each section before navigating to another section. If the section has saved successfully a d Successfully message will appear at the top of the screen. 6. Navigating through the FAST Assessment Assessment 1. The pane contains some basic information about the Assessment. This is also where the Assessment is saved and sent for approval (if required). a. Assessment Date: Enter the date the FAST Assessment was conducted with the family. b. Assessment Status: Displays the current approval/finalized status for this Assessment. c. Assessment Category: Indicates whether or not this is the Initial Assessment or a Reassessment. d. Assessment ID: This is the ID assigned to this Assessment by the Synergy system. e. Assessor: Displays the name of the user who completed the Assessment. f. Program: Displays the Synergy program where the Assessment was created. g. Supervisor: If the Send to Supervisor for Approval checkbox is checked this drop-down will be enabled. The user can then select the name of their Supervisor and click as Final to send the Assessment for Approval and Finalization. The Supervisor drop-down will not be visible unless an approval is required. as Draft h. and : as Draft as Final i. Click to save the Assessment prior to Approval/Finalization. as Final ii. Click to Finalize the Assessment. Finalization will lock this Assessment against any further editing. 5/1/2017 FAST Assessment Job Aid 3 of 13

2. The pane contains all of the navigation buttons for the Assessment. Clicking on a button will open that section of the Assessment for viewing or editing. Assessment as Draft Note that the pane must be saved using the button before the pane can become active. Print 3. The pane allows the user to generate a printable PDF version of the Assessment. a. Select the Full Report or the Summary Report using the radio buttons to the left of the Report options. By default, all respondents are selected. Individuals can be selected or deselected using the checkboxes in the multi-select drop-down. Print b. Click to generate the PDF. Important Reminder when previewing and printing: Previews will open in a new window or tab. This window or tab may open in the background and may not be immediately visible. Close all other previews first. Synergy is NOT compatible with Microsoft s Edge browser. Use either Internet Explorer or Chrome. 5/1/2017 FAST Assessment Job Aid 4 of 13

FAST Assessment Respondents Respondents 1. Click the button in the pane. Household Member Respondents a. : All of the potential Household Member Respondents are listed in this pane. Select Household Members to include as Respondents by checking the checkbox to the left of each included individual. To select all of the Household Members, check the checkbox at the top of the grid. b. FAST Respondent Type: For each selected individual, indicate whether they are a Caregiver, Child, or Both by selecting from the drop-down. Support Respondents c. : All of the potential Support Respondents are listed in this pane. Select Supports to include as Respondents by checking the checkbox to the left of each included individual. To select all of the Supports, check the checkbox at the top of the grid. d. Click below the Respondents panes to save the selections. Respondent Data d Successfully i. FAST Assessment Prep Prep 1. Click the button in the pane. The date, location, and participants of a prep meeting must be entered and saved before the actual specifics of the Prep Meeting can be entered. New 2. Creating a new Prep: click to open the New Prep pop-up: a. Meeting Prep: This will automatically be entered by Synergy based on the number of Meeting Prep s that have already been saved. b. Prep Meeting Date: This defaults to today s date but can be updated. Enter the date the Meeting Prep took place. c. Location: Select the location where the Meeting Prep took place from the drop-down. d. Date Entered: This is the date that the Meeting Prep information was entered into the system and is filled in automatically. e. Included in Prep?: Select all of the Respondents who participated in the Prep Meeting using the checkboxes to the left of the Respondents names. f. Click to save the information entered. Clicking Cancel will close the New Prep pop-up without saving any information. 5/1/2017 FAST Assessment Job Aid 6 of 13

3. Documenting the specifics of the Prep Meeting: a. Select a Meeting Prep from the grid by clicking on the name of the Meeting Prep. The Meeting Prep details text fields will open up. i. To edit the Respondents, Prep Date, or Meeting Location, click the Edit Icon [ ] to the right of the Meeting Prep. ii. To delete a Meeting Prep, click the Delete Icon [ ] to the right of the Meeting Prep. IMPORTANT NOTE: There is no Delete Confirmation pop-up when deleting a Meeting Prep. b. Enter information in the narrative for each Meeting Prep question and click. Prep Assessment d Successfully i. FAST Assessment Family Family 1. Click the button in the pane. Family Together a. Rate each item using the scale at the top of the pane. b. Enter the Justification for items as needed. c. Some items, depending on what rating is selected, may cause additional questions such as Is this item a strength? or Is this an item of contention?. These items are marked by a [ ^ ] Family Together d. Once the pane has been completed click. Family Assessment d Successfully i. FAST Assessment Adult/Caregiver Adult/Caregiver 1. Click the button in the pane. 2. Select an Adult/Caregiver by clicking on that individual s name in the from the Adults/Caregivers list. That individual s line in the grid will turn grey to show that it has been selected. 3. Complete the Adult/Caregiver Strengths, Adult/Caregiver Functioning, and Adult/Caregiver Trauma Experiences sections for each individual. a. Rate each item using the scale at the top of the pane. b. Enter the Justification for items as needed. c. The user should receive the message at the top of the screen before continuing on. FAST Assessment Child Child 1. Click the button in the pane. 2. Select an Adult/Caregiver by clicking on that individual s name in the from the Adults/Caregivers list. That child s line in the grid will turn grey to show that it has been selected. 3. Complete the Child Strengths, Child Functioning, Child Status Trauma, and (if the child is under age 5) Early Childhood sections for each child. Click child in the grid, otherwise information entered will be lost. before moving on to another a. Rate each item using the scale at the top of the pane. b. Enter the Justification for items as needed. c. Some items, depending on what rating is selected, may cause additional questions such as Is this item a strength? or Is this an item of contention?. These items are marked by a [ ^ ] d. Once the information has been completed for an individual click before moving on to the next individual. ii. FAST Assessment IPV and UNCOPE NOTE: The IPV and UNCOPE portions of the FAST assessment are not currently completed by Family Support Center staff. These sections of the FAST will not be editable. FAST Assessment PHQ-9 PHQ-9 1. Click the button in the pane. 2. Select a Household Member by clicking on that individual s name in the from the PHQ-9 list. That individual s line in the grid will turn grey to show that it has been selected. 3. Enter the date and score of that individual s Patient Health Questionnaire (PHQ-9). a. Click before moving on to the next individual. b. To add another PHQ-9 score for an individual, click. i. The user should receive the message at the top of the screen before continuing on. FAST Assessment Notes Notes 1. Click the button in the pane. 2. Use this section to document any additional information that applies to this family for the FAST Assessment. Check the checkboxes for each applicable item and enter a narrative. 3. Once the information has been completed click. Notes Data d Successfully i. Saving as Final and Requesting Approval 1. When the Assessment has been completed maximize the pane and click as Draft. Assessment 2. If the Assessment is ready to be finalized, click in the pane at the top of the screen. If Supervisory Approval is required, the Send to Supervisor for Approval checkbox must be checked and a Supervisor must be selected prior to clicking as Final. as Final Assessment as Final a. Upon clicking a confirmation pop-up will appear: Clicking Clicking Yes No will save the Assessment as Final. will close the confirmation pop-up without saving the Assessment as Final.
