SURVEY QUESTIONS Main Topic: Enterprise Imaging 1. What type of organisation are you working for? 2. How familiar are you with the concept of Enterprise Imaging (EI)? 3. Is Enterprise Imaging a strategic priority in your organisation? 4. Do you already use a dedicated Enterprise Imaging solution in your organisation? 5. Do you have plans to purchase a dedicated EI solution? If not, why do you think Enterprise Imaging is not important? 6. Please let us know if clinicians in your organisation can 7. From the top of your mind, what are the top 3 Enterprise Imaging vendors you d recommend to a friend or colleague? 8. In your opinion, which are the biggest benefits of using Enterprise Imaging? 9. How will the environment for ehealth innovation and investment in your country develop over the next 12 months? Enterprise Imaging in this study is defined as Enterprise Imaging refers to a set of strategies, initiatives and workflows implemented across a healthcare enterprise to consistently and optimally capture, index, manage, store, distribute, view, exchange, analyze and make full use of all clinical imaging and multimedia content to enhance the electronic health record. 5 KEY FINDINGS ehealth Business climate: The business trend for ehealth continues to be positive, the outlook has even increased since the beginning of Lessons on Enterprise Imaging (EI) Lesson 1: Enterprise Imaging is a concept many ehealth professionals are aware of but not very well! Many professionals indicate to know something about Enterprise Imaging, but only about one out of three respondents claims to know much about it. There s much opportunity left to educate and shape this market space before it will mature. Companies, but also individuals, that are able to fill this gap with ideas, knowledge, or products will be able to reap the benefits of their efforts. Lesson 2: Healthcare providers are just starting to use dedicated Enterprise Imaging platforms. Only 26% of healthcare facilities in (mostly Western) Europe currently use dedicated Enterprise Imaging platforms. But many more have already made EI a strategic priority in their organisation, with a good share having dedicated investment plans over the next 2 3 years. This will be a growing market segment. Lesson 3: EI is expected to increase efficiency levels, improve care collaboration and add value for the patient. Similar to the motto of the Olympic Games (faster, higher, stronger), Enterprise Imaging promises to be faster (access to images/information), richer (more images/information) and safer (supports collaboration and to take more informed decisions). But unlike the Olympic Games it also promises to be cheaper (thanks to efficiency gains). Well, some of this perhaps still needs to be proven over time but the promises sound convincing and expectations are certainly optimistic. Lesson 4: EI s sweet spots longitudinal image history of a patient and device-agnostic access capabilities. Yes, many clinicians already have access to images from patients that are treated within their healthcare organisation. No, this access doesn t always work through a single user interface / the main application in use. An USP compared to the currently most prevalent model to capture, store and access images is to enable clinicians to see all the images (and videos, waveforms etc.) of a patient in context of related information and documents, chronologically listed, with the ability to easily compare them and be able to easily use mobile devices, where appropriate. Lesson 5: The most often recommended Enterprise Imaging solution vendors in Europe are Agfa Healthcare, Siemens Healthineers and Carestream! But there are quite significant differences between regions and countries, for example Visus in the D-A-CH Region and Sectra in the Nordic Countries was named third most frequent. 5

AWARENESS ABOUT ENTERPRISE IMAGING BY COUNTRY Enterprise Imaging (EI) is not on everybody s mind. While everybody has heard about Electronic Medical Records or PACS, a fair share of ehealth professionals doesn t know much about medical imaging beyond Radiology. There are only few differences between different countries or regions. How familiar are you with the concept of Enterprise Imaging (EI)? [Scale from 1 not at all to 5 I am an expert ; total including other ; labels only shown when >2% ] 1 Not at all I am an expert 5 Mean D-A-CH Region n=117 Italy n=31 Netherlands n=30 Nordic Countries n=64 Spain n=44 Total n= Enterprise Imaging in this study is defined as Enterprise Imaging refers to a set of strategies, initiatives and workflows implemented across a healthcare enterprise to consistently and optimally capture, index, manage, store, distribute, view, exchange, analyze and make full use of all clinical imaging and multimedia content to enhance the electronic health record. 8 Position Organisation AWARENESS ABOUT EI BY ORGANISATION AND POSITION It s not surprising Software vendor employees say they are quite familiar with the concept of Enterprise Imaging. Same for CIO s, no matter for what kind of organisation they are working for. In contrast, professionals working for governmental health authorities or in a position as organisational and corporate governance appear to know the least about Enterprise Imaging. 9 ENTERPRISE IMAGING AS STRATEGIC PRIORITY The majority of ehealth professionals working in health facilities declares that EI is a strategic priority in their organisation (61%), either financially supported (42%) or not (19%). Compared by country it appears that Italian and Spanish health facilities do most often have a dedicated EI strategy. Within the D-A-CH region nearly half of the professionals surveyed are either not aware or declare that their organisation has not yet made Enterprise Imaging a strategic priority. Is Enterprise Imaging a strategic priority in your organisation? [Only participants working in a health facility; total including other countries] Total By country Italy n=10 Nordic Countries n=24 n=203 D-A-CH Region n=80 Netherlands n=16 Spain n=23 9

USAGE OF AN ENTERPRISE IMAGING SOLUTION Only about a quarter of health facilities use dedicated EI solutions. A similar share relies on heterogeneous image management solutions that are integrated and behave similar in many aspects to a consolidated environment. Most facilities still rely on multiple departmental solutions that are not very well integrated with each other (45%), disabling their clinicians to be more efficient and provide better care to their patient. This rather non-ideal environment is mostly prevalent in the D-A-CH region. Do you already use a dedicated Enterprise Imaging solution in your organisation? [Only participants working in a health facility; total including other countries, excl. don t know ] Total By country Italy n=10 Nordic Countries n=19 n=170 D-A-CH Region n=64 Netherlands n=15 Spain n=21 Yes, we use a dedicated EI platform No, we have several departmental imaging solutions that are not fully integrated Yes, we use various solutions that are fully integrated and enable enterprise-wide access and sharing for all types of images, videos, waveforms etc. We use something else 10

PLANS TO PURCHASE EI SOLUTIONS A majority of health facilities (69%) not yet using dedicated EI solutions have plans to invest into such products. Most investment plans will not be realised immediately, but within the next 2-3 years. Those who have no plans to invest typically don t have funds available, wait until after they purchase a new EMR or are relatively satisfied with their status quo. Do you have plans to purchase a dedicated EI solution? If not, why do you think EI is not important? [Only participants working in a health facility who are not using a dedicated Enterprise Imaging platform; total including other countries] It is important, but my hospital has even more pressing needs that take priority over this. Because nobody deals with it, there s a kind of "allotment garden mentality" (i.e. each department works on its own) This will become important soon, but will be implemented dependent of the choice of EHR. It is important, monetary issues are the problem. We have other urgent mandatory investments to make. Current solution is sufficient for now. Other priorities are selected currently. Yes, we use a dedicated EI platform Yes, we use various solutions No, we have several departmental imaging solutions We use something else 11

EI PRODUCT AND SERVICE OFFERINGS A relatively high proportion of representatives from software vendors and consulting companies taking this survey indicate to offer, or plan to offer, Enterprise Imaging solutions or services. Whereas this is not a representative study for the European ehealth vendor and consulting sphere, it still suggests that many companies perceive EI to be a lucrative and growing market segment which might be right when looking at the investment plans from healthcare providers on the previous slide. Do you provide products or consultative services with regard to Enterprise Imaging? [Only participants working for an IT software vendor or consulting company] As a partial aspect Data management systems Yes, but only to launch projects related to enterprise imaging (strategy) 12

IMAGE ACCESS AND VIEWING CAPABILITIES Most health care professionals are able to access almost all images directly through the EMR (64%), with the Netherlands leading in this area (nearly 90% of Dutch clinicians have this capability). This might also explain why some ehealth professionals don t really feel the pressure to improve the situation. However, simple access is not everything, in more than half of all cases multiple applications need to be opened in order to access images and associated documents. The real sweet spot for further efficiency gains and EI product and consulting offerings appears to lie in enabling seamless access, with the capability to easily see a longitudinal image history of patients, incl. the possibility to do this by using mobile devices. Please let us know if clinicians in your organisation can [Only participants working in a health facility; multiple answers possible; total including other countries] access more or less all images, waveforms, videos, documents etc. patient-specific directly through the EMR access images from outside the organisation, i.e. remotely easily view images and related reports, notes through the same application easily see a longitudinal image history of their patients access images from mobile devices such as tablets and smartphones ES IT DACH Total n=188 IT DACH NL NL NORDIC IT NL EI Sweet Spot? 15 ENTERPRISE IMAGING BENEFITS BY ORGANISATION Clinicians, using an EI platform and programme, will benefit from having all images in one place, with faster access to more images from almost everywhere and the capability to collaborate easily with relevant stakeholders. This will also be beneficial for patients, especially because they as well will have better access to more types of images and can rely that their care providers are able to take better informed decisions. Overall, benefits related to efficiency gains (especially saving time and money) are mentioned most frequently. In your opinion, which are the biggest benefits of using Enterprise Imaging? [Categorized free text entry; total including other ] Efficiency (cost reduction, less paper, performance improvement, time saving, real time information) Accessibility (access for all users to information, easier management, higher security, less dependencies) Collaborate / Share / Communicate (interoperability, cross-organisational sharing, telemedicine, base for HIE) Patient benefit (avoid double radiation and exams, improve patient safety and satisfaction, quality of care) All in one place (longitudinal patient record, comparability, consilidated diagnotic, one vendor) Other (standardisation, improved workflows, data security, usabilty, big data analysis) Total n=213 Consulting Company n=26 Governmental Health Authority n=21 Heath Facility n=105 IT Software Vendor n=35 15
